Outline of service:
Gaddum and Manchester Mind have come together to provide a new and innovative service which will support people to leave inpatient mental health settings and return to their community with the support they need to recover and remain well.
As a team, we will deliver psycho-social interventions, advocacy, housing and welfare rights advice and peer support. There will also be tailored support to young people (those under the age of 25). All we do is underpinned by listening and ensuring people feel heard – because we know that when people are listened to, they recover. The team will be working with colleagues within the mental health, primary care, inpatient and community sectors.
Job summary:
As a Peer Support Worker, you will have lived or living experience of receiving mental health care or treatment. As an ‘Expert by Experience’ you will support peers (people with similar experiences to your own) who are ready to leave hospital after being treated for their mental health needs. Receiving support from your colleagues, you’ll develop and manage your casework practice and hold a small case load.
Walking alongside people prior to discharge from mental health hospital, you’ll support people in the discharge process to get the care they need, and then into a community setting where they can continue to recover and begin to explore their aspirations. Ultimately, people will feel connected to their communities – to the right services, friends and family.
This role will work with a small but intensive caseload (max 12 cases for a full-time peer support worker) to focus on:
· Repatriate: when people are sent to an out of area placement (OAPs) advocate to get them back to Greater Manchester so they can recover in their community.
· Discharge: Supporting people when they’re ready to be discharged from inpatient mental health services to get back to the community and recover.
· Prevent re-admissions: Work with people, post-discharge, to ensure they continue to get the support they need to make informed choices, stay well and ultimately thrive.
Main Duties and Responsibilities
· To provide bespoke support to people that are ready to be discharged from inpatient psychiatric care to return to the community, building (with support) packages of care so they have a positive and sustained recovery.
· To work alongside people post-discharge, in their community setting (e.g. home or supported accommodation) to ensure they get what they need to stay well.
· To effectively manage a small but intensive caseload (max 12 cases for a full time worker), being flexible to people’s needs and short-term changes in schedule.
· To work in a person-centred, trauma informed, empowering and culturally appropriate way that enables people to feel listened to and heard.
· To help people engage with services and make sustainable links with their communities.
· To use and develop your knowledge of community care, policies, legislation and services to get people the support they need to recover and stay well.
· Identify when cases need escalation or onward referral and keep your line manager aware of case developments.
· Independently visit people in inpatient and community settings in Manchester and across Greater Manchester, adhering to lone working and risk management procedures.
· Occasionally visit people out of the Greater Manchester area who need to return to their community in Manchester (all travel expenses will be paid and non-commute travel time will be part of your working day).
· Build relationships with colleagues within services (e.g. inpatient staff, social care, CMHTs etc) to enable improved collaborative working
· Take part in multi-disciplinary meetings, representing the views of people, and their aspirations on returning to community settings.
· Support the development of this new service, drawing on your role as an ‘Expert by Experience’ and using what you hear from the people you support to contribute to the development of the services by reviewing and contributing to improvements.
The post holder will be required to undertake other tasks as reasonably directed by the Programme Management and Senior Management Team, which will usually be commensurate with the skills and experience of the post-holder.

We are seeking three confident and experienced Modern Slavery Case Worker Assistants with a wide range of skills and importantly, a ‘can do’ approach.
As a Modern Slavery Case Worker Assistant, you will work as a member of a close and supportive team providing a safe environment for vulnerable adults from outside the UK who have experienced trauma and exploitation. You will provide high quality practical and emotional support to the residents within a risk assessed safe house setting. The role will involve assisting clients with increasing practical skills and self-confidence to empower and enable independent and safe living.
This service is open 24 hours a day, seven days a week. Applicants must be willing to work flexibly, which will include on-call duties. This role will work between two properties approximately 4 miles apart on a rota basis so you must be able to travel to both properties.
You must have experience of working with vulnerable adults, the ability to work within a ‘risk associated’ environment and, ideally, experience of working within a residential or community setting with non-UK citizens.
Administration / IT skills and health and safety awareness are essential. Modern Day Slavery, Immigration and knowledge of the asylum process would be desirable.

IKWRO is a registered charity which provides advice and support to Middle Eastern, North African and Afghan women and girls living in the UK, who have experienced, or are at risk of all forms of “honour” based abuse, including; forced marriage, child marriage and female genital mutilation (FGM), or domestic abuse.

We are looking for an experienced, skilled and friendly adult counsellor, dedicated to improving the mental health and emotional resilience of our community.

You will be delivering 1:1 complex case counselling and assessments within face to face and remote appointments for our Stockport Homes Group counselling service based onsite at Beacon House or at Stockport Homes Group main office.
Beacon Counselling and Stockport Homes Group work in partnership to provide effective, high quality short-term therapy to Stockport Homes adult tenants through a self or supported referral pathway.
Clients referring into this service will be assessed initially by this role and allocated into the appropriate pathway within the programme.
The role requires 10 client hours per week including assessments, with a 12 hour per week contract. You may be required to work evenings to meet the demand and needs of our clients.

Although training to use our systems will be provided, you will have some digital competence and understanding to fulfil the administrative requirements of the role.

If so, Hope4 has an exciting opportunity. We are seeking a Senior Key Worker to join our team and play a central role in delivering our vital homelessness support services in Rugby. Hope4 aims to end homelessness and food poverty one person at a time. We need an ambitious, compassionate person to join us. About the role
You will work as part of our small staff team along with local accommodation providers, seeking to place clients directly into accommodation. You will work with them to develop outcome-focused support plans that reflect their needs, personal strengths and goals, to make sure that they can sustain their accommodation and thrive in their community.
You will coordinate support around your clients and act as their Key Worker to help them sustain their accommodation and move forward with their lives. This will mean coordinating interventions provided at the Hope Centre by our staff or other agencies, or in the community so that the right support is provided at the right time, enabling clients to progress on their journey to independence.
About you
You will:
- need a relevant background that includes experience of working with those who are homeless or on the margins of homelessness.
- understand the needs and obstacles faced by those experiencing homelessness.
- be experienced in supporting people who are vulnerable and/or could have multiple disadvantages and complex needs.
- be able to demonstrate that you are a team player who is flexible, proactive, compassionate and dependable.

Hope4:
At Hope4 we believe that everyone has the right to have a place to call ‘home’, somewhere that is safe, secure and sustainable, and enough food even when in a crisis. We also believe that everyone should have the chance to realise their hopes and ambitions, and as a charity our clients are at the heart of what we do. Although Rugby is a small market town, in 2024 we supported over 180 people at our Day Centre and provided over 6000 food parcels at our Foodbank. The Hope Centre clients access a holistic range of services from the immediate need of a sleeping bag to showers, food, clothing, computers, mental and physical health care, and Key Workers working on a tailor-made plan enlisting the help of housing providers. Our staff and more than 80 volunteers are crucial to making this happen.
The charity is founded on Christian principles by the local churches and works to address homelessness and food poverty in the Borough of Rugby. Our staff and volunteers support these values and ethos irrespective of belief or background.

Job Title: Senior Key Worker
Reporting to: Service Delivery Manager
Location: Hope Centre*, 8 Newbold Road, Rugby, CV21 2LJ. *some off-site working is anticipated.
Salary range: Full-time, £31,000- £33,000 depending on experience.
20 days holiday a year (plus birthday and bank holidays, increasing a day a year of service to a max of 25 days a year).
Hours: Full time, Monday to Friday. Some evenings, bank holiday or weekend work may be required, and time off is given in lieu of extra hours worked.
Overall role purpose:
As our Senior Key Worker, you will work closely alongside the Service Delivery Manager to oversee the smooth running and operation of the Hope Centre in terms of its operational efficiency and standards.
You will be the main point of contact for staff and external agencies when the Service Delivery Manager is not available.
Key Responsibilities for a Senior Keyworker
1. Case Management and Support:
o Be responsible for the triage and support of a case load of clients who have multiple needs including mental and/or physical health issues drugs and/or alcohol dependencies, a history of multiple exclusion and social isolation, and who require an intensive case management approach.
o Deliver high quality support including assessing housing, benefits, healthcare, and employment needs. enabling clients with multiple needs to access and engage with the help they require., Supporting them to develop their plans and achieve their goals and to ultimately leave homelessness for good, including assessing client needs, risks and strengths
o Coordinate and support access of clients to relevant resources and partner agencies both at the Hope Centre and in the community ensuring effective communications between all parties.
o Liaise with partner organisations as necessary including meetings with other professionals concerning clients.
o Advocate for clients and lead on access to appropriate external organisations to ensure appropriate, timely support to help end their homelessness.
o Lead on first stage escalation of client issues/challenges.
o Maintain accurate and up-to-date records of client progress, interventions, and outcomes.
2. Team Leadership and Mentorship:
o Lead case review meetings, ensuring a coordinated and consistent approach to client care.
o Provide support to colleagues and volunteers to contribute to the development of skills and experience in the team providing guidance on best practices.
o Support team members, including volunteers, in their work by sharing your knowledge and skills and working collaboratively and constructively together in the interests of members.
o Supervision of volunteers where required.
o Participate positively in team meetings and group work sharing ideas and respecting colleagues’ contributions to build and sustain an inclusive team.
o Be the main point of contact for staff and external agencies when the Service Delivery Manager is not available.
3. Coordination with External Agencies:
o Act as a key point of contact for multi-agency meetings and initiatives focused on tackling homelessness.
o Liaise with partner organisations as necessary including meetings with other professionals concerning clients.
o Build and maintain strong relationships with local authorities, housing providers, and support services to secure resources for clients.
4. Crisis Intervention:
o Provide immediate support in emergency situations, including support to securing temporary accommodation and addressing urgent needs.
o Support Safeguarding concerns, ensuring all cases are handled in line with policies and procedures.
5. Program Development and Advocacy:
o As part of a team ensure the delivery of Psychologically Informed Environments and Trauma Informed Care that:
• promote client engagement and maximise inclusion and safety
• support wellbeing and the development of resilience and interpersonal skills
• are person centred helping people to recognise and build on their strengths
• motivate people and encourage them to identify and work through the changes they need and want to make supporting them to recognise their progress
• provide routes into appropriate learning and skills opportunities and ensure that clients have access to the community-based specialist services and support they need
• recognise the prevalence of, and impact of adversity and trauma on a person, understanding that individuals have all too often been disempowered, disconnected and excluded, and being careful not to repeat this
• recognise the value of compassion and connection in creating safety
o Contribute to the development and improvement of homelessness programs and services across the town and county.
o Act as an advocate for clients to ensure they are engaged with, or continue to receive assistance from other specialist services, working to address barriers to housing and support including community based and mainstream services needed to help them achieve their goals.
6. Compliance and Best Practices:
o Ensure all work adheres to legal, ethical, and organisational standards, including confidentiality and data protection.
o Stay informed about homelessness trends, policies, and practices to deliver effective and informed support including attending identified training.
o Ensure working for and volunteering at Hope4 is a positive experience.
7. Relationships and influence
o Develop and sustain positive operational partnerships that help ensure clients can access appropriate services in their communities.
o Build and maintain excellent relationships with colleagues within Hope4 and other organisations to ensure the delivery of a high-quality service as part of a wider service-offer that ends homelessness.
o Work reflectively, developing an awareness of your own and others’ relational styles and responses, and any judgements, biases or assumptions that may impact upon your work.
o Flexibility to cover other roles to maintain and adapt service delivery where required
8. Monitoring and quality
o Adhere to monitoring, case recording, outcome reviews and quality and audit systems. Ensure that accurate data is captured on the Case Management System and that there is good and appropriately responsive communication.
o Awareness of quality standards, including the compliance and good governance of service provision.
o Competent in the use of IT systems including Microsoft applications.
General:
o All information and data to be maintained in accordance with Hope4 policies and procedures, and within GDPR principles.
o Undertake identified training within specified timelines and maintain current operational knowledge where required.

Purpose of the Job

Key Responsibilities
- To provide practical mental health focused short-term case work to young people, through 1:1 face to face appointments
- To signpost students for further support as and when necessary
- To manage a busy case load

- To develop and deliver workshops, campaigns and training for young people, educational staff and youth workers about mental health (on occasion)

About the Role:
Depaul UK delivers a wide range of support and housing services for people experiencing or at risk of becoming homeless throughout the UK. Nightstop London, run by Depaul UK, is part of a national network of over 30 Nightstop services run through different organisations.
This role is busy and varied and involves working with young people from when they approach the service to when they find longer term accommodation. You will be the first point of contact for all young people, assessing each young person’s needs and risks. In addition you will be responsible for coordinating volunteers as well as organising the logistics of each Nightstop placement and making sure that each Nightstop placement is safe.
Key Responsibilities:
- Be the first point of contact for people experiencing homeless and referral partners, providing advice and signposting.
- Conduct needs and risk assessments with potential Nightstop guests.
- Arrange the logistics of a guest’s stay with a volunteer.

The Living Well Team:
The Bury Living Well Service is formed from an alliance of mental health services in the Bury area. The staff team is made up of staff from different backgrounds from Pennine Care Foundation Trust, Bury Involvement Group, the Creative Living Centre, and Early Break, and includes both clinical and non-clinical staff who employ a multi-disciplinary team approach to supporting individuals with significant mental health challenges in the community.
Job Summary:
You will form part of the Voluntary, Community, and Social Enterprise (VCSE) provision within the Bury Living Well Model as a member of the Creative Living Centre (CLC) Service Delivery Team. The Link Worker is responsible for case loading adults experiencing mental and emotional distress, offering person-centred and trauma informed support in the community both face to face and via telephone. Ensuring that service users are provided with both initial and ongoing support in either 1:1 or group sessions, as appropriate to their needs which are identified and reviewed on an ongoing basis. You will engage empathically with those you support, providing positive emotional and therapeutic support, through a collaborative approach which focuses on individual strengths rather than deficits, enabling and promoting autonomy and self-care. You will work alongside clinical staff and staff from other VCSE organisations to form multidisciplinary team approach to collaboratively support individuals in the community. King George’s is a 68 bed hostel for men with complex needs and a history of rough sleeping. It is part of Westminster City Council’s Rough Sleepers Pathway. Many of our clients are deemed high risk/high support due to chronic mental health and physical health issues, high levels of substance misuse, and offending histories. We accept that our clients have experienced complex trauma, usually from a very early age and often throughout their lives including currently. For this reason, we work to imbed the Psychologically Informed Environment (PIE) approach in a service.
Reporting to the Manager, your role will require you to develop, support and motivate the people who we support and accommodate. We run a service that is creative, responsive and that provides a foundation which empowers people to achieve their aspirations and make positive and sustainable life changes. You will be crucial to making that happen.
