As a compassionate Point of Entry Mental Health Practitioner, you’ll help provide advice, guidance and potentially urgent mental health support to those in need in our local communities. Your role will help support people find the most appropriate mental health service in Swindon, whether internally to S&G Mind or externally to our many key stakeholder partners/providers across the county.

- Compassionate Caller Support: You'll handle a variety of incoming calls, providing a compassionate ear to individuals seeking advice, guidance or urgent mental health support. Your person-centered approach will offer immediate and practical help, aligning with the principles of positive mental health and evidence based-interventions.
- Connecting People to the Right Care: You’ll play a vital role in guiding callers to the most appropriate mental health services in Swindon. Whether referring internally to S&G Mind or externally to key stakeholder partners, you’ll ensure each person is connected to the right support.
- Holistic Assessment & Planning: Using collaborative framework across the healthcare system, such as ‘Safety Planning’ and ‘Your Team, Your Conversation, Your Plan’ frameworks, you’ll assess and understand each caller’s unique needs, ensuring timely and appropriate responses that prioritise their mental health.

Brent Synergy: Is a Housing Related Support (HRS) Service for a range of vulnerable people who have been or are at risk of homelessness. The age eligibility criteria for the service is adults between the ages of 18 to 60, who are identified to have low/medium support needs. Some of the needs we support are:
- Single homeless between ages of 18 to 60
- Socially excluded individuals
- Ex-Offenders
- People with substance misuse
- Mental Health (Low)
- Learning disability (Low/Medium)
- Women who have come from domestic abuse environments and situations who may need low level support as part of their resettlement following a move on from a refuge or other safe domestic abuse supported accommodation.

Senior Mental Health Support Worker

About the Role
The Senior Mental Health Support Workers are responsible for ensuring the effective and safe management of the project on a day to day basis including undertaking a case co-ordination role for individual young people as part of a multi-agency team, line managing progression coaches, and managing volunteers.

Background to the role
The voluntary, community and social enterprise (VCSE) sector is an integral part of local health and care systems. VCSE organisations deliver key services that support the health and wellbeing of the population. In addition, they contribute vital insight and intelligence on the needs of the people and communities they engage with and are frequently a route to engaging with communities themselves.
There is a drive through Buckinghamshire’s local mental health partnership forum to replicate and build upon the successes of the Buckinghamshire, Oxfordshire and Berkshire West (BOB) VCSE Health Alliance and the Buckinghamshire VCSE Mental Health Response Group that was established at the start of the Covid pandemic.
Community Impact Bucks has been commissioned by Oxford Health NHS Foundation Trust to:
- increase the awareness of VCSE organisations that deliver support to people who have or are at greater risk of poor mental health, with a focus on community Opportunity Bucks wards and health inequalities
- bring those organisations together through an alliance-based approach and
- help build relationships between the NHS and VCSE that ensure sustainable mental health services are provided to the people of Buckinghamshire.
About you

Key responsibilities
- Provide VCSE representation, attending and presenting to Integrated Care System/Integrated Care Partnership (ICS/ICP) Board meetings, events and other information sharing forums. Enable and co-ordinate other VCSE leaders to represent the sector at such meetings.
- Advocate for the VCSE sector, encouraging ICS partners to consider new and innovative approaches to working closely with the sector to support the mental health needs of people of all ages in Buckinghamshire.
- Undertake and maintain mapping to identify and engage VCSE organisations supporting people with mental ill health or improving mental health outcomes, including through addressing inequalities and the wider determinants of mental health.
- Develop and provide mechanisms for VCSE partners to collaborate on addressing shared priorities in relation to poor mental health, with alignment to the work of the BOB VCSE Alliance and with health and care strategies and plans.
- Build VCSE organisations’ capacity to take part in procurement and/or bid opportunities, including short-term opportunities and more sustained activities, working with partners and national bodies to identify additional opportunities and resources, and drafting bids if required.
- Lead the design, delivery and evaluation of the project, ensuring it meets contractual obligations, operates within budget and achieves the desired impact.
Key requirements
- Experienced in working with the VCSE sector, ideally in the context of improving mental health and wellbeing outcomes.
- A track record of successful partnership working and relationship building with key stakeholders within the health and care system and VCSE sector.
- Ability to assimilate complex information quickly and communicate (written and verbal) effectively with multiple stakeholders.
- Excellent persuasion, influencing and negotiation skills.
- Good analytical skills and a practical approach to problem-solving.
- Competent user of databases and the Microsoft suite of software.
- Either Full British driving licence and access to private means of transport or ability to demonstrate how you will meet this requirement.

Who are we looking for?

About the role
Mental health social work is a highly varied role that offers a wide range of opportunities to support the lives of people with mental health needs.

The Project
GPIMHS is an emotional and wellbeing service that supports adults aged over 18 in primary care who are experiencing a range of mental health difficulties. Patients are offered extended consultation times, quick and easy access to practical advice and tailored support for their mental health needs from a multidisciplinary team.
Through sharing wisdom from their own experiences, the Lived Experience Practitioner will inspire hope and belief that recovery is possible in others. As an integral and highly valued member of the team, the Lived Experience Practitioner will provide formalised peer support and practical assistance to people using the service in order for them to regain control over their lives and their own unique recovery journey.
In particular, the Lived Experience Practitioner will support the smooth transition of people using the service to other Mental Health services, by helping them to complete recovery and crisis plans for example. Within a relationship of mutuality and information sharing, they will promote choice, self-determination and opportunities for the fulfilment of socially valued roles and connection to local communities.
The Role
You will make lives better by utilising your lived experience of mental illness or of caring for somebody who has mental health problems. In this Peer Support role, you will contribute to the care and recovery of people using GPIMHS (General Practice Integrated Mental Health Service) and MHICS (Mental Health Integrated Community Services), and help to ensure an accessible, non-stigmatising localised service.
About You
Skills, knowledge, experience and qualifications required:
- Lived experience of mental illness or of caring for somebody who has mental health problems.
- Experience accessing primary and / or secondary mental health services.
- Paid or unpaid experience of working in a lived experience role.
- Computer literate in Microsoft Office, and willingness to use the Organisation’s IT systems.
- Experience of recovering a meaningful life.
- Experience of being in a supportive and enabling role.

Purpose of the Job
Hammersmith and Fulham, Ealing and Hounslow Mind have secured funding from the National Lottery Community Fund to deliver a new outreach programme, designed to increase awareness and access to mental health support for children, young people (CYP) and families from underserved communities, children who may not be in school, or who may not be able to travel to use our services. Utilising your skills in engaging young people where they are, these roles are a key part of ensuring our work supports any young people who need our services.
Job Summary
The role includes:
- Delivering direct outreach with young people to raise awareness of mental health support and pathways.
- Providing detached mental health services within the community to address challenges in a non-clinical environment.
- Using your skills to develop engaging resources that increase awareness and understanding of youth mental health issues, and adapt them to different languages and local communities.
- Work alongside our Parental Engagement Practitioner to deliver with community groups and partner agencies, raising awareness throughout diverse communities across our boroughs.
- Create and manage partnerships with local community groups for the purposes of raising awareness and increasing access.
Key Responsibilities
- Work with your team to develop children and young people’s outreach provision offered by HFEH Mind, using creative approaches to engage CYP.
- Help CYP and parents to overcome logistical challenges to accessing support.
- Work in partnership with community organisations to work with underserved groups including boys and young people from minority ethnic backgrounds.
- Offer direct outreach support to young people who have been excluded from school or with emotionally based school avoidance.
- Participate in a mapping exercise of local communities, languages and knowledge of mental health.
- Use trauma informed approaches (e.g., ARC, Tree of Life) to support children, young people and their parents and carers.
- Signpost anyone accessing the provision to appropriate and local on-going support; and build up your knowledge and networks of such support.
- Deliver interventions in community settings and at our Circle site.
- Meet the KPIs and outcomes of our Lottery funding.
- Work alongside our young advisors and parental support group to ensure new ideas for engagement are co-produced

We have designed a multi-year anti-racist programme of work contained in our Race Equality Action Plan which demonstrates our intention to hold ourselves accountable and be judged on our progress on becoming a truly anti-racist organisation.
