This is a very exciting time to be involved with Social Prescribing. It has been identified as a key component of NHS England’s Universal Personalised Care objective contained within their Long Term Plan, as well as London’s Health Inequalities Strategy for healthy communities published by The Mayor’s Office in 2018. Both strategies recognise the influence of social, emotional, economic and cultural factors on health and well-being and the importance of community connections to improving health and wellbeing, self-care and self-management of long term conditions.

You will work as part of a Primary Care Network Multi-Disciplinary Healthcare Team, providing 1:1 personalised support to Children and young people,ndividuals, and families utilising a holistic and strength-based approach, to help identify the wider issues that impact on their health and wellbeing and work together to co-produce a wellbeing plan to address these needs. You will have excellent holistic assessment skills, as well as interpersonal and communication skills that enable you to build trusting relationships which support individuals feel empowered with their wellbeing.
You will forge strong links with local VCSE organisations, community and neighbourhood networks, building on what is already available, to create a menu of community groups and assets to connect individuals to as part of their wellbeing plan.
You must be a good listener and have experience of working positively with individuals facing complex, social and emotional challenges that impacts on their wellbeing. Being creative as well as having a solution focussed attitude to removing barriers to health and wellbeing is essential.
You will also have strong leadership qualities , experience of supervising staff and report writing to support strategic and organisational requirements .

Key Responsibilities (a full list of responsibilities is listed in the Job Description):
- Work in conjunction with Oakleaf's Client Services team and Guildford Action to help identify clients who are ready to access employment.
- Prepare clients for employment by supporting the development of a CV, compiling Job Applications, building career development plans, undertaking interview practice, arranging, and assisting at interviews as and when required. Plus, all other elements as may be undertaken through our “Job Club.”
- Enhance employer engagement by identifying and building relationships with local employers, other VCFS agencies, Jobcentre Plus and commercial employment agencies.

Age UK Mid Mersey are offering a great opportunity to join our professional, innovative and dedicated team to help launch a new lottery funded Maintenance Cognitive Stimulation Therapy (MCST) programme in Mid Mersey’s four boroughs: Halton, Knowsley, St Helens and Warrington.
Maintenance Cognitive Stimulation Therapy (MCST) is a weekly group programme for people living with mild to moderate dementia or cognitive impairment.
Group members take part in meaningful and stimulating activities, proven to help maintain memory and mental functioning. The groups provide a fun, supportive environment where people can build new friendships.
Activities include:
- discussions
- word games
- quizzes
- physical activities
- creative and musical activities.
Cognitive Stimulation is the only non-drug treatment recommended to improve cognition, independence and well-being by the National Institute for Health and Care Excellence (NICE).
Alongside the MCST group sessions the programme will also provide advice, information and facilitated peer support for the carers of groups members.
The MCST Team Leader will lead a small team of MCST group facilitators, Carer Support workers and volunteers.
The Team Leader will plan programmes, provide support and supervision to staff, and ensure compliance with relevant guidelines and standards. They will be responsible for undertaking initial assessment visits with clients in their own homes to ensure the MCST programme is appropriate for them.
Travel across Mid-Mersey is expected so the post holder must have access to their own vehicle and a full clean driving licence.
Full training in MCST will be provided alongside management training and mentoring.

The Housing Advice Practitioner will by providing tailored advice and guidance through mainly remote online or phone support, as well as in person housing advice at a weekly housing advice surgery in the community. About the role:

Role Overview -
The Multibank Manager will focus on driving the growth and impact of the West Midlands Multibank by building strong relationships with referral and donation partners, overseeing daily operations including inventory management and distribution, and ensuring the project's financial sustainability through effective communications and fundraising. It also involves evaluating the Multibank’s impact to identify unmet needs and emerging trends, while providing direct line management to the Logistics Co-Ordinator and coordinating all staff and volunteer teams to deliver efficient and equitable support to communities in need.
Key Reponsibilities
· Develop and grow the impact and scale of the West Midlands Multibank including building relationships with potential referral and donation partners.
- Engage with organisations to establish and maintain an appropriate network of referral partners to support those in need to access the support of the Multibank
- Build and manage relationships with local businesses and corporations in order to secure additional donations for distribution and to secure Corporate support, with support of BVSC leadership
- Work collaboratively with partners to ensure the Multibank is meeting the needs of communities effectively and supporting those in poverty to access resource and wider help as needed
· Manage the day to day running of the West Midlands Multibank, including the referral and distribution processes and inventory management

The Fund invests in sustainable farming practices, training and capacity building, data collection and research to benefit farmers, farm workers and the environment.
About this role
As Grants Manager you will work closely with a portfolio of implementation partners that the Better Cotton GIF funds. You will assess applications for funding, review project reports, manage the Fund’s relationship with these organisations and, together with the Programmes team, play a key role in recommending what funding and support should be committed to them. The ideal candidate will have lived in South Asia or Sub-Saharan Africa or visited them extensively. If you have a particular expertise in gender, climate change, soil health, pest control, or smallholder livelihoods, that would be an enormous advantage.

Responsibilities
Collaborate with Better Cotton GIF team to manage GIF grant-making workflow and calendar, including:
- Coordinating application processes, including managing incoming grant proposals.
- Assessing grant applications to the Better Cotton GIF– analysing the quality and potential impact of proposals and organisational viability; obtaining appropriate advice and references; reaching a judgement or recommendation for submission to the Field Impact and Investment Committee and handling relationships throughout the process until the application is either successful or declined.
- Supporting regular meetings of the Field Impact and Investment Committee which is responsible for approving the investment portfolio.
- Preparing and sending funding agreement letters to grantee partners
- Managing and monitoring grants, through the review of reports and project visits, to ensure their effectiveness and delivery against project design and targets.
- Support programme partners to innovate, introduce new solutions to challenges and deepen the impact of their projects
- Ensure project budgets are developed and spent appropriately and in-line with project plans
- Maintain the GIF grant management database: Ensuring that all grant and grantee information is entered consistently and accurately. Generating reports and dashboards for the programme and finance teams. Regularly proposing ways to better track grants, budgets, and managing the database.
- Collaboratively evaluate, develop, implement new and refresh ways of managing the Fund’s workflows, processes, policies and procedures to improve efficiency and accuracy.
- Capturing and analysing information on the delivery of programmes and reporting on progress to the relevant stakeholders.
- Work closely with the Better Cotton country team Programmes teams to support their relationships with GIF funded partners, through coordination around grant management and programme review processes.
- Support the Monitoring and Evaluation team to ensure that MEL processes and frameworks are effectively used with partners, and learnings integrated into programme implementation. Support the delivery of project baseline and endline evaluations.
- Keep abreast with developments in the cotton/environment sectors to inform the Fund’s future grant programmes.
Each Grant Manager also takes responsibility for one of the smaller sub funds and one or more other areas of work according to interest and/or need, for example, livelihoods or gender, or workers
The role is expected to evolve in line with Better Cotton and GIF priorities.

Working arrangements
The position is full-time (40 hours per week) and will be based in the London, Lahore, Delhi, or Abidjan offices. Better Cotton offers flexible working, with core hours being 10 am – 4 pm and the option to work from home one day per week.

We are looking for a Youth Services Manager to manage our youth team and project, empowering young people to feel connected to their community and live to their full potential. Our youth project has been established in the community since 2007, with summer provision, evening drop-ins, after school activities, mentoring and a youth leadership programme. As a small community organisation, working as a manager at WSF provides the opportunity to be involved in all aspects of the project, from fundraising to staff management, budgeting to delivery.
