Our belief is that rough sleeping is an intrinsically harmful and dangerous activity both for the individual and for the wider community. Therefore, we take a proactive and assertive approach to supporting people away from this activity. Our aim is to support people to recover from the effects of rough sleep and move away from a street lifestyle permanently.
We are looking for a dynamic, self-driven person with relevant language skills to support the Roma Rough Sleeper Team as an Outreach Worker (known internally as Roma Mediator), working across central London boroughs. In this vital role you will:
- Undertake Street outreach shifts where you will be involved in making contact with people who are sleeping rough, assessing their needs and delivering personalised case management; to support individuals to make the move away from the streets.
- Support teams of Street Outreach Workers to deliver accessible services and ensure that contact is made and sustained with Roma people sleeping rough.
- Ensure the assessments made and the support offered are appropriate and relevant for the client group and that Roma people are supported to engage with the available services.
- Work in partnership with multi-disciplinary teams and agencies and advocate, where appropriate, on behalf of Roma people with external agencies regarding their welfare rights and other support needs.

An exciting opportunity has arisen for 2 experienced Crisis Outreach Workers to support Mind in Enfield and Barnet's Family Hubs Service.This role is to support individuals during the perinatal period (expecting and up to 2 years after birth) presenting at the Family Hubs experiencing mental health crisis and problems gain immediate support
The purpose of the Outreach Worker is to work with adults during the perinatal period (pre birth up to 2 years after birth), including both parents and carers. You will work as part of MiEB perinatal family hub team to identify those at risk of crisis and ensure referral pathways are in place to facilitate rapid engagement
The successful candidates should have a minimum 2 years experience of working with people with mild, moderate and severe mental health issues. Experience of utilising de-escalation techniques, and supporting clients presenting in emotional distress

About the Service
This is a pilot working as a Multi-Disciplinary Team (MDT) with housing teams and Drug and Alcohol Services across District and Borough councils. These teams will be working with individuals who are presenting as homeless and have a number of Complex Needs.
Purpose of Post
Providing advice, information, onward referral and holistic support to clients who are experiencing mental ill health or need support with their mental wellbeing. These will be clients who are presenting to District and Borough councils as homeless and need intervention and support with their mental health and remain a source of independent support for all clients. Ensure that the safety and wellbeing of the client is monitored and reviewed regularly and that a person centred recovery orientated and trauma informed approach is embedded approach in all aspects of the roles.
Service Objectives
- To improve the mental wellbeing of people experiencing mental ill health.
- To increase early access to help for people experiencing mental ill health in, by providing a clear and effective pathway to services provided by Herts Mind Network and other third sector and statutory providers.
- To contribute to an improvement in individual mental wellbeing.
- To remain a source of independent support for all clients.
- To reduce the use of police, ASB, tenancy enforcement, housing needs and environmental health services by clients whose primary need is related to mental ill health.
- To reduce the use of crisis services by people experiencing mental ill health without positive outcomes for the individual.
- To provide an assertive outreach approach for clients who are in crisis and/ or those that may have complex or multiple needs.
- To work collaboratively within an MDT consisting of CGL, District and Borough councils, housing teams and other professionals.

We have a vacancy for a Flourish (Refugee and Asylum Seekers) Community Outreach Worker to join our team, supporting refugees and asylum seekers in the community with multiple needs who have mental health as a primary need, through providing advice, information and holistic support.
About the Service
This is an exciting and reasonably new project, ‘Flourish’ – Mental Health for Refugees in Hertfordshire, which is a highly flexible and responsive outreach service developed to meet the needs of refugees fleeing the war in Ukraine & other refugees in Hertfordshire. The aim of the service will be to support refugees in the community with multiple needs who have mental health support as a primary need.
The countywide holistic service also provides emotional wellbeing support for families who are hosting refugees.
Purpose of Post
- To provide advice, information, onward referral and holistic support to clients with mental ill health.
- Support to build social networks and reduce isolation whilst settling into the Hertfordshire community this includes building a sustainable and empowering peer support network and building confidence and self-esteem.
- To ensure that the safety and wellbeing of the client is monitored and reviewed regularly.
- To remain a source of independent support for all clients.
- Embed a person centred and solution focused approach in all aspects of the role.
- To work in a trauma informed way including awareness of the 6 principles of trauma informed practice, putting the clients emotional and practical needs at the forefront of this work.
Key Responsibilities:
- Working closely with all key delivery partners across the system to ensure the service is responsive, inclusive and easy to access.
- To take allocated referrals from any agencies including the Community Navigators, Borough and District Councils, Herts Help, Housing Providers, Herts Welcomes Refugees, the Refugee Council and self-referrals including walk in contacts at any of the Mind wellbeing centres across the county.
- Hold a caseload of clients, supporting for approx. 10 weeks
- To create a safe space for the individual to be able to explore their feelings and improve their understanding around their mental health.
- To complete a holistic needs assessment.
- Ensure all person centred risk assessments and support plans are completed.
- Proactively recognise the indicators of deteriorating mental health and facilitate appropriate action, whilst liaising with relevant agencies.
- Support clients to build their emotional resilience, promoting self-management through building an understanding of their own triggers/coping strategies.
- To provide safeguarding intervention support. This will include responding to problems, which pose an immediate risk.
- To work alongside translators when required to provide a robust package of support for clients.
- To accompany clients to initial visits and appointments as necessary in order to ensure the sustained use of existing support services in the Borough.
- To provide advocacy services to clients experiencing mental ill health.
- To assist clients to access appropriate mental health services, housing/tenancy services and other services according to assessed needs.
- To establish effective pathways across a variety of agencies to maximise and provide effective partnership working, working with other third sector and VCSE organisations to provide signposting and continuity of care.

Migrant Democracy Project (MDP) envisions a society where all migrants at home in the UK hold collective, political power to shape a society rooted in justice, freedom, and solidarity.
We are looking for a full-time Community Organiser to organise first-generation migrants in the UK towards migrant justice by supporting community members to organise events and participate in campaign activities. The work will connect across 3 streams: voter registration; advocacy & campaigns; and political empowerment.
Whether it’s engaging migrants at a voter registration stall, supporting migrant leaders to develop activities in their own communities, or sharing stories for the campaign for votes for all - the lived experience of first generation migrants is at the heart of all our activities.
We are looking for a proactive Community Organiser who is passionate about migrants’ rights and democracy to start working as soon as possible.
Who funds the role?
The role is funded by a grant from the Esmee Fairbairn Foundation.
Contract
This is a fixed term contract, 2 years (with the possibility of extension depending on future grants). Benefits include:

Location
Migrant Democracy Project does not have a physical office, although the team does meet in person on regular co-working days. The Community Organiser will work flexibly from home on administrative tasks and be expected to work in-person when necessary to deliver voter registration, training, informative, advocacy and campaign activities, as well as representing MDP at relevant community events.
This role will suit someone based in Greater London.
Responsibilities

Register migrant communities to vote.

Engage migrant communities in key locations in the UK towards the Our Home Our Vote campaign.

Plan and deliver outreach events with first-generation migrant communities where there is scope for building power in their local areas.

Connect community leaders with their elected representatives on the Our Home Our Vote campaign.

Train and empower migrant communities on UK politics and how to best achieve their campaign aims.

Identify new local and national migrants’ rights groups that can support MDP’s campaigns and keep MDP’s contact database up to date.

Organise the logistics behind Parliament tours where migrants without the right to vote visit their MPs to ask for their right to vote.

Share physical and digital materials on democratic rights and participation at relevant community events.

Assist MDP’s Digital Organiser in collecting digital material from community events, which can be shared on MDP’s platforms.

Keep track of community engagement and assist with report writing for our projects’ evaluation.
Qualifications
We are looking for a proactive and confident Community Organiser with the following essential skills:

Demonstrable experience working with local communities at the grassroots level on campaigns.

Event administration skills, including booking venues, inviting speakers and communicating event programmes, as well as dealing with last-minute event changes.

Outreach Practitioner
An Outreach Practitioner is required to work on an initial 3-month contract for a drug and alcohol charity based in North London.
The role of an Outreach Practitioner is to provide high-quality street-based drug and alcohol interventions to people who are homeless or at danger of becoming so in order to promote positive outcomes.
They will oversee, evidence-based interventions, recovery planning, and aftercare planning while managing a small, complex caseload.
Responsibilities:
- Engage and reengage homeless individuals misusing substances by conducting street outreach, building relationships along the way.
- As part of a well-integrated care package, provide evidence-based psychosocial one-on-one therapies and adopt a proactive stance when dealing with service user disengagement.
- This includes naloxone programs, recovery interventions, opiate replacement therapy, blood-borne virus intervention, assertive outreach, needle and syringe program delivery, and harm reduction advice, guidance, and education.
- Participate in internal and external multidisciplinary review meetings and collaborate with housing teams, local authority officers, and other treatment agencies to encourage individuals in seeking treatment in order to reduce the occurrence of antisocial behaviour and street drinking.
Experience Required:
- Proven ability to provide individuals with complex needs with high-quality services. This includes having the capacity to work one-on-one or lead group sessions, complete assessments, care and support plans, risk assessments, and case recording in a timely manner and to a high degree.
- Enthusiasm for working with disadvantaged or socially excluded people, with a track record of meeting deadlines, properly and promptly recording information, writing reports, and remaining composed under pressure.
- Strong IT abilities, such as the ability to use digital meeting platforms, Microsoft Office applications, and case management systems like Theseus, System One, or Nebula.

The role is working 21-28 hours per week, providing support to clients who are unable to access the service.

About the role

About You

- You will be an excellent communicator liaising effectively with key stakeholders including the multi-disciplinary and health professionals, family carers and people we support. (E)
- You will have experience of staff management and the ability to lead, motivate and develop a large team, ensuring staff are appropriately trained. You will be visible in the service and strive to model best practice and inspire support workers. (E)
- You will be able to think creatively and be adaptable in approaches, with experience of implementing emergency management guidance to support staff. (D)
