The Community Connector will support people with serious mental illness (SMI) to achieve better health and wellbeing, increase independence and reduce reliance on services.
Key to this role is person-centred support and building trusted relationships. The Community Connector will connect people to a broad range of services and interventions in their local area, supporting and empowering people to access opportunities and take positive steps to improve their mental health recovery goals.
The Community Connector will support people through:
• One-to-one-person centred support focussing on what matters to the individual
• Addressing the social determinants of health, such as education, employability and social welfare advice.
• Opportunities to build social connections within their local communities, fostering engagement in fun and meaningful activities.
• Emotional support to build resilience and confidence.
• Guidance on navigating and accessing wider support services.
The Community Connector will be embedded in the Bromley by Bow Centre’s Social Prescribing service, utilising their experience and support. It will work with a wider team of voluntary sector, primary care and secondary care staff in the Neighbourhood Mental Health Team (NMHT).
The role is funded by NHS East London Foundation Trust (ELFT) and managed by the Bromley by Bow Centre. There are a team of Community Connectors as part of a wider transformation programme in East London to redesign community mental health care for adults with serious mental illness in Tower Hamlets, Hackney and Newham.

If you are right for this role, you will be a committed, motivated, and outgoing individual who wants to support the sexual and reproductive health (SRH) of young people living in the Royal Borough of Greenwich, working in a partnership capacity as part of a holistic health and wellbeing programme. Your role will be multi-faceted, and you will therefore be flexible and able to offer a variety of SRH interventions in a broad range of settings, to a broad range of young people.
First, you will provide brief interventions in sexual health in an outreach capacity at schools, colleges, universities, leisure centres, youth organisations, and other venues. Here, your primary focus will be to provide basic information about SRH to young people, whilst also encouraging them to complete a Chlamydia screen and register to the free condom scheme. You will also promote online testing for those aged over 18, and connect young people with other local sexual health services through referral and signposting.
Second, you will deliver SRH and relationships training to groups of young people and professionals, in smaller workshop settings as well as to larger groups. You will be able to deliver accurate information on a variety of topics that are often sensitive and complex. You will have an existing knowledge base that you will be willing to expand and solidify through a combination of supported training and independent learning.
Third, you will facilitate offsite drop-in sexual health sessions and offer 1:1 sexual health support to young people who attend any of the four Young Greenwich youth groups. You and your team will each be responsible for attending the four Young Greenwich hubs once each week during evening hours, working closely with the youth group leader as well as the young people themselves to establish trusted relationships and an engaging programme of activity. You will be proactive and creative at integrating sexual health to other, ongoing activities, and ensure effective safeguarding.
Additionally, you will work closely with the wider SHO team, including supporting the sexual health phone lines processing screening results, and where necessary, signposting to relevant services. Across all aspects of your role, you promote local sexual health and contraception services to young people and adults alike, referring to in-person or online services as appropriate and handholding as much or as little as needed, particularly to those most vulnerable. You will keep accurate monitoring records for all interactions, referrals, Chlamydia screens, and C Card registrations / distributions. You will also contribute to quarterly service review reports.
This role requires flexibility, working unsociable hours in the evenings, and occasional weekends. Schedules may vary from week to week depending on programme need.

About the job
Due to a restructure, we are seeking a confident and experienced Modern Slavery Case Worker with a wide range of skills and importantly, a ‘can do’ approach.
As the Modern Slavery Case Worker, you will work as a member of a close and supportive team providing a safe environment for vulnerable female adults and their dependents up to the age of 1 from outside the UK who have experienced trauma and exploitation. You will provide high quality practical and emotional support to the residents within a risk assessed safe house setting. The role will involve assisting clients with increasing practical skills and self-confidence to empower and enable independent and safe living.
You will be expected to line manage Case Worker Assistants and bank staff. Additionally, you will be allocated a caseload of up to 5 service users to support.
This role is mainly Monday to Friday, with occasional evening and weekend shifts. You will be required to be part of an on-call rota and able to get to and from the project if it is necessary to attend the project whilst on-call.
You must have experience of working with vulnerable adults, the ability to work within a ‘risk associated’ environment and, ideally, experience of working within a residential or community setting with non-UK citizens.
Administration / IT skills and health and safety awareness are essential. Modern Day Slavery, Immigration and knowledge of the asylum process would be desirable. Please also note that there is a substantial amount of administrative work involved in this role.

How are staff supported to work remotely?

This includes assisting with the organisation and delivery of events.
You'll leverage your creativity and professionalism to manage and grow existing corporate partnerships, while proactively seeking new business opportunities by undertaking research, create compelling proposals for a range of audiences and pitching to companies to secure their support. It you have the transferable skills to succeed, no formal fundraising experience is necessary.

Purpose of the role
Are you a dynamic communications professional ready to take the lead on impactful sexual health campaigns? Brook’s award-winning Communications Team is looking for a Media and Communications Co-ordinator for Yuno, the new sexual health service for Bristol, North Somerset, and South Gloucestershire. This is an exciting opportunity to deliver bold campaigns at a time when STI rates are rising across England.
Essential criteria
- Experience in media, communications, or PR, ideally within the charity or public health sector
- A strong understanding of social media management, content creation, and digital marketing
- Outstanding written and verbal communication skills, with the ability to craft compelling narratives for different audiences
- Confidence in working with press and media outlets to secure coverage and respond to media inquiries
- Experience using Adobe Creative Cloud, Canva, or similar design software
- Excellent organisational skills and the ability to manage multiple projects simultaneously
- Based in Bristol, North Somerset or South Gloucestershire

A Dementia Adviser role requires you to work passionately to provide advice, support and guidance to people affected by dementia, carers and their families.
Sahara at Alzheimer’s Society is a Dementia Support Service dedicated to serving the South Asian Communities in Greater Manchester.

This role involves supporting South Asian communities across the Greater Manchester area. Therefore, we are seeking a candidate who is fluent in both English and Urdu. You would be working across all 10 boroughs of Greater Manchester so being able to travel independently is essential.
As a Sahara Dementia Adviser, each day brings new opportunities to make a meaningful impact. Our work involves managing referral/caseloads, collaborating with memory clinics, and supporting our memory cafés. We are committed to raising awareness by delivering educational sessions to social groups, places of worship, healthcare professionals, schools, and universities. Through close collaboration with other organisations, as well as corporate and charity partners, we strive to provide the best possible support for those affected by dementia.
About you
We are looking for someone who:
- Has experience of assessing people face to face with an ability to assess their needs
- Can deliver advice and guidance with a non-judgmental approach and have outstanding communication skills
- has strong organisational and time management skills to manage your caseload of clients effectively
- is proficient in MS Office and CRM systems
- can travel independently across Greater Manchester, with an expectancy to attend regular meetings across the area
- understands dementia and the needs of those living with dementia and their carers would be an advantage
- is fluent in English and Urdu but as advantage in proficient in Punjabi, Hindi, Bengali or Gujarati or any other South Asian languages.
- Confidence in public speaking and ability to deliver presentations in different languages if necessary.
- Has some experience of volunteer management.
