Job Purpose

Provide a fully person-centred Support Worker/Case Management service, including assessment, recovery planning, co-ordination, and review, for veterans and their families. This role is an integral part of the day-to-day delivery of our Advice and Support Service, which has been re-shaped and went live in the summer of 2024. The service is targeted at older people in or facing financial hardship, and provides information, advice and follow up support relating to a range of issues, including Welfare Benefits, Social Care and Housing.

As a Senior Adviser you will be responsible for supporting Advisers with their welfare benefits and housing work, including responding to the advisers on duty, supporting with complex cases, carrying out independent file reviews in line with Advice Quality Standard and designing and delivering internal and external training. 

You should have in depth, specialist level knowledge of Welfare Benefits or Housing (ideally both) as they relate to older people, and experience of working within an advice setting and advice supervision framework such as the AQS framework. You will have a warm, supportive and motivating style to enable Advisers to grow and feel empowered in their work.

Breaking Barriers’ relationship with corporate partners is the USP upon which our growth and success as an organisation has been built. The Head of Corporate Partnerships has accountability for generating income, impact and influence from the private sector, and for leading a high-performing team to achieve ambitious growth and scale for refugees around the UK.

As Breaking Barriers has grown from a start-up organisation to a now 60-strong team raising c. £4m and supporting more than 1,200 refugees annually, our corporate partnerships have expanded beyond our original London base to be nationwide partnerships with a wide range of businesses and employers. You will lead a team to deliver outstanding partnerships management of a range of strategic and multi-faceted relationships with a portfolio of c.50 businesses alongside leading strategy for business development. Strategic development and implementation

Job Profile

The role of the Major Gifts and Philanthropy (MGP) Team Research and Information Manager is a unique role within the sector and CAFOD. Working across MGP (which is made up of Major Donors and Corporate teams) as well as others, such as the Trusts and Foundations Team, it has been developed to meet the growing needs of planning and reporting across MGP and to ensure income growth is focused on raising the funds needed to achieve short and long term organisation priorities.

A pivotal role, the post-holder will hold lead expertise in philanthropy communications at CAFOD, working with other stakeholders to identify funding priorities so MGP can achieve an income forecast of nearly £3.17m in 2024/25.

Managing the Research and Information Executive, the post-holder will make sure that concept notes, proposals and reports are written for the most strategic work, and that MGP relationship managers are informed of projects in need of funding. With oversight of all team donor proposals, reporting and communications, you will ensure they align with CAFOD’s values, especially promoting anti-racist messages in line with our commitment to becoming an anti-racist organisation. 

The post-holder will share key donor trends and work closely with the Philanthropy Manager and Corporate Partnerships Manager to understand their research needs and develop a research strategy. Alongside the Research and Information Executive, you will complete research into top-level prospects and donors.

As MGP’s information and prospect research expert, you will work with the Executive team, International Programmes, Integrated Fundraising and Business Innovation team, Data and Insight, Communications and Finance teams as appropriate.

Key Responsibilities 

Strategic leadership of MGP’s funding portfolio & engagement 

• Lead on the implementation of MGP’s funding strategy, maintaining a funding proposal pipeline and tracking where funds have been secured for projects, so it sits as a centre of excellence across CAFOD

• Oversee donor communications for the team, including appeals and cases for support for CAFOD’s most strategic work: unrestricted, and our humanitarian funding initiative, Every Second Counts; ensuring communications comply with anti-racist communication and fundraising principles

• Identify and influence change, especially relating to donor systems and processes

Lead prospect and donor research

• Identify key trends relevant to the MGP team - collating and analyse internal and external donor trends and reports and sharing them with the team

About the Service

This is a pilot service working as a multi-disciplinary team (MDT) with housing teams and Drug and Alcohol Services across District and Borough councils. These teams will be working with individuals who are presenting as homeless and have a number of Complex Needs. The Complex Needs Housing Team Leader plays a significant role in the co-ordination and running of the Complex Needs Housing Service.

The objectives of Hertfordshire Mind Complex Needs Housing Service are:

• To improve the mental wellbeing of people experiencing mental ill health.
• To increase early access to help for people experiencing mental ill health in, by providing a clear and effective pathway to services provided by Herts Mind Network and other third sector and statutory providers.
• To contribute to an improvement in individual mental wellbeing.
• To remain a source of independent support for all clients.
• To reduce the use of police, ASB, tenancy enforcement, housing needs and environmental health services by clients whose primary need is related to mental ill health.
• To reduce the use of crisis services by people experiencing mental ill health with positive outcomes for the individual.
• To provide an assertive outreach approach for clients who are in crisis and/ or those that may have complex or multiple needs.
• To work collaboratively within a multi-disciplinary team (MDT) consisting of Change Grow Live (CGL), District and Borough councils, housing teams and other professionals.

This role is key to the development and support of the team and the wider organisation. 

The post holder will need to:

• Support the Services Manager with effective running of the Complex Needs Housing Service.
• Provide advice, information, onward referral and holistic support to clients who are experiencing mental ill health or need support with their mental wellbeing. These will be clients who are presenting to District and Borough councils as homeless and need intervention and support with their mental health.
• Line manage staff and where necessary volunteers.
• Lead a team of Mental Health Outreach Workers. Ensuring the team are using databases and outcome tools as required to report for the project.

Key Responsibilities

• To receive and process referrals from the local district and borough Councils for those who have presented as homeless with a mental health need, as well as other appropriate referrals pathways across Hertfordshire and make direct contact within 72 hours of referral.
• To establish effective pathways across a variety of agencies to maximise and provide effective partnership working.
• To manage a caseload of up to 5 clients.
• Embed a holistic, trauma informed and person-centred service
• Proactively recognise the indicators of deteriorating mental health and facilitate appropriate action, whilst liaising with relevant agencies.
• Ensure a direct community service is in place which encourages, motivates and builds patients self-esteem and confidence. This will include goal setting and support with social and practical skills development.
• Ensure all person-centred risk assessments and support plans are completed as appropriate.
• To assist clients to access appropriate mental health services, drug and alcohol services, housing/tenancy services and other services according to assessed needs.
• To work with partners both internal and external to ensure that vulnerable person’s cases are monitored and risks evaluated in order to promote community safety.
• Support clients to build their emotional resilience and crisis planning tools, promoting self-management through building an understanding of their own triggers/coping strategies.
• To work with HMN staff in other services in order to make onward referrals to other projects.
• Liaise with appropriate HPFT staff to ensure that individuals are assessed for personal budgets to support with any longer-term needs.
• Support individuals to develop sustainable local social networks, both through our Herts Mind Network Wellbeing Centres and other community services and resources.

The successful candidate will play a significant role in the co-ordination and running of the Complex Needs Housing Service.

• Ensure that the safety and wellbeing of the client is monitored and reviewed regularly.
• Be an independent source of support for all clients.
• Embed a person-centered and recovery orientated approach in all aspects of the roles. The Recovery approach includes empathy, warmth, acceptance, authenticity, compassion and humanity.

Key Accountabilities

• To receive referrals a wide range of referral pathways.
• To undertake initial contact with clients and complete a holistic needs assessment, making use of assertive outreach work to achieve the initial client contact, if required.
• To provide holistic support both outreach and in our centres and advocacy services to clients experiencing mental ill health.
• Ensure all person-centered risk assessments and support plans are completed as appropriate.
• To accompany clients to initial visits and appointments as necessary in order to ensure the sustained use of existing support services.
• To liaise with other agencies to ensure that clients receive appropriate support and continue to remain engaged with the service.
• To assist clients to access appropriate mental health services, drug and alcohol services, housing/tenancy services and other services according to assessed needs.
• To establish effective pathways across a variety of agencies to maximise and provide effective partnership working.

About the Service User Network (SUN)

The SUN is an open access model of community-based facilitated group peer support for people experiencing complex emotional needs associated with complex trauma and/or a diagnosis of ‘personality disorder’. The SUN aims to help people develop effective ways of coping, reduce emergencies and improve access to appropriate services.

Those accessing the SUN may or may not have a formal diagnosis of personality disorder. Group members will have experienced longstanding emotional difficulties, isolation and may demonstrate patterns of behaviour that may not be helpful in the longer term.

About the Role

Fundamental to creating empowerment and engagement in the SUN Project is the therapeutic community principle, that the effectiveness of the service depends upon engaging group members in the task of running, delivery, development and evaluation of the service. The utilisation of SUN members’ own resources and expertise is integral to the SUN model of peer support.

SUN Facilitators will have their own lived experience of complex emotional needs associated with a diagnosis of ‘personality disorder’ and will be able to demonstrate their recovery journey. They will work closely with mental health practitioners from ELFT (East London NHS Foundation Trust) to deliver regular Service User Network (SUN) groups in different community-based locations in Bedford, Central Bedfordshire or Luton. Full training in the SUN model, and weekly supervision by a Psychotherapist / Psychologist, will be provided for all team members.

Facilitators will work to develop and support the therapeutic community principle of the SUN model and the effectiveness of the service through engaging group members in the task of running, delivery, development, and evaluation of the service. The utilisation of SUN members’ own resources and expertise is integral to the model of peer support upon which the SUN groups are based.

SUN facilitators will have regular supervision and support in this role along with appropriate training.
