We are looking for a Family Support Worker to join our team and to make a difference in children's lives every single day. As a Family Support Worker, you will work in close partnership with the children, families and school staff to deliver a programme of casework to promote and improve educational outcomes for the children we support. 

You will also focus your support to help improve attendance, punctuality, engagement in learning, improved behaviour in school and the local community as well as improving social and emotional regulation and well-being.

You will have excellent relationship building and communications skills with a proven ability to communicate with a wide group of people.

• working and engaging with parents, carers or guardians, children or individuals through individual and/or group-based support

• working in an educational or outreach environment 

• working effectively with social and emotional factors affecting a child’s capacity to learn

Addiction Family Support is a UK-based charity dedicated to supporting adults affected or bereaved by a loved one's harmful use of alcohol, drugs, or gambling.

The Role: Family Support Worker (Bereavement)

As a Family Support Worker at Addiction Family Support, you will provide crucial emotional and practical assistance to individuals bereaved by the loss of a loved one due to addiction. You will be responsible for delivering a range of services to provide support to people bereaved (and affected) by someone else’s harmful use of alcohol, drugs or gambling. Through both individual and group support, you will offer a compassionate and understanding presence, helping people navigate grief and loss in a safe and supportive environment.

Our service delivery takes place via several methods, including taking calls on our telephone Helpline, responding to emails through our email service and delivering the 5-Step Method intervention to individuals, couples and groups and facilitating group support via an online platform.

Working patterns are flexible to allow our services to run from 9am and 9pm 7 days a week. You may be expected to work on a rota basis, covering shifts during this period.

We would hope to be able to offer more hours per week in time when funding allows it.

You will need to be confident in your ability to provide support via a variety of remote and digital platforms, being able to respond to immediate requests for support. You will be an active and involved member of our team and contribute to all aspects of our work to help us achieve our strategic goals and work towards achieving our vision.

Responsibilities:

• Offer empathetic and non-judgmental emotional support to those affected by the bereavement of a loved one due to addiction (via a Helpline and adapted 5-Step Method).
• Provide information, literature, and resources to help individuals and groups manage grief and cope with their situations.
• Facilitate or co-facilitate bereavement support group meetings, creating a safe space for participants to share their experiences and provide mutual support.
• Respond to inquiries and offer structured emotional support through helplines or support groups, following guidelines and safeguarding practices.
• Maintain accurate and confidential records in line with GDPR and the charity’s policies.
• Ensure safeguarding concerns are addressed promptly and signpost individuals to additional services where appropriate.

Recovery Navigator (Mental Health Support Worker)

About the Role

The Refugee and Migrant Centre is seeking a dedicated and compassionate Family Support Worker to join our Resettlement Department. As part of our mission to assist refugees and migrants in overcoming barriers to integration, this role will play a key part in helping newly arrived refugees settle into their new communities with confidence and independence.

RMC is a well-established charity with over 25 years of experience working with vulnerable refugees and migrants. We provide a range of services, including immigration advice, housing support, welfare assistance, and employment guidance. Our work ensures that refugees and migrants receive the necessary support to become equal citizens in the UK.

Purpose of the Role

The Family Support Worker will support newly arrived refugees and their families, enabling them to navigate the complex challenges of settling into the UK. Working closely with our multi-skilled resettlement team, you will deliver holistic support to refugees across a range of services, including welfare benefits, housing, health, education, and employment. You will also play a key role in empowering families to build their confidence, independence, and long-term integration into the community.

Main Duties and Responsibilities

Client Support & Case Management:

• Manage a caseload of newly arrived refugee families, providing tailored support to meet their specific needs.
• Conduct home visits and orientation sessions to welcome clients to their new community and complete holistic needs assessments.
• Develop Personal Integration Plans for each household on a quarterly basis, helping them set goals for long-term independence.
• Coordinate support with external agencies and specialists to address clients’ needs, ensuring seamless service delivery.
• Support families with housing, welfare benefits, education, healthcare, and employment.
• Assist with registration for healthcare services (GPs, dentists, opticians) and support with medical assessments.
• Facilitate access to education for children and English for Speakers of Other Languages (ESOL) classes for adults.
• Provide guidance on immigration matters and ensure the correct paperwork is in place, including registering for Biometric Residence Cards and supporting welfare benefit applications.
• Ensure clients are financially supported by applying for relevant welfare benefits and assisting with mandatory reconsiderations as required.

Integration & Independence:

• Help clients develop the skills and knowledge to achieve independence, focusing on self-sufficiency and integration into the local community.
• Support families in securing suitable accommodation, liaising with landlords, housing providers, and relevant stakeholders.
• Provide ongoing orientation on UK systems, including financial rights and responsibilities, social services, and community resources.
• Encourage social integration by organising outings and community events to foster connections with local services and other refugees.

Partnership Working & Community Engagement:

• Work closely with a range of internal and external stakeholders to ensure that refugees have access to the services and support they need.
• Act as the lead contact for other agencies involved in supporting your clients, ensuring a collaborative approach to case management.
• Promote the long-term integration of refugees by identifying and facilitating opportunities for engagement with mainstream services.

Administrative & Reporting Duties:

• Maintain accurate and up-to-date records of client interactions and case progress, ensuring compliance with organisational policies.
• Provide written reports and case updates as required by the Resettlement Coordinator and Manager.
• Contribute to the development and improvement of services based on monitoring and evaluation feedback.

General Responsibilities:

We’re looking for an innovative, passionate and professional individual with excellent communication and organisational skills to join the Stroke Recovery Services, based in the Morecambe Bay area. 

This is an exciting opportunity to work with stroke survivors and their families to support them following a stroke.

Position: S11246 Stroke Support Coordinator

Location: Home-based, Morecambe Bay. However, frequent travel will be required as part of this role (will include team meetings, other work related meetings and home visits)

Hours: Part-time, 28 hours per week

Salary: Circa £21,391 per annum

Contract: This is a fixed-term contract for 12 months. Our services are contracted, we currently have funding for this contract until 31 March 2028.

We are looking for a collaborative, driven and problem-solving candidate to design and lead our core support projects and improve the way we make life better for people affected by aplastic anaemia.

This is a fantastic opportunity to join a high performing team within the AAT. Our team is small, but mighty. Our current S&O team is made up of three part-time support and outreach workers some of whom have lived experience of aplastic anaemia. Lived experience is hugely important to us but is not an essential criterion for this role.

As part of the Senior Management Team, you will bring the process, structure and programme-oriented approach to our successful range of support services. You will be helping to give people living with aplastic anaemia access to better information, better networks and better ways to advocate for themselves. 

This opportunity is for a permanent contract, starting immediately. You will work 35 hours a week, and we encourage flexible working. However, the team does generally operate during core office hours (between 9am to 5pm) and there are regular team meetings on Thursdays.  

Today, 12 children and young people will be diagnosed with cancer. We’ll stop at nothing to make sure they get the right care and support at the right time.

About the role

We’re looking for an experienced social worker to join our Central Support & Social Care Team. This is a fully home-based team providing tailored support to children and young people with cancer across the UK, over the phone and via digital channels. The team works alongside our social work teams in hospitals across the UK.

A key remit of the team is to provide support to our hospital-based social work teams during periods of reduced capacity or a spike in referrals. So as well as 17.5 core hours each week, this postholder will be expected to undertake additional hours as and when required. Support carried out for hospital-based teams will also be over the phone or via digital channels - you won't be expected to travel to the hospital site.

About this key role
As a proactive member of the multi-disciplinary team, you will develop support plans and promote programmes of social education, activities, support, advice, and information, using a range of multi-sensory methods.  You will be expected to build positive relationships and promote the value of lifestyle changes to the client group and their families and work directly with them to promote positive change, build resilience, reduce risks, and prevent young people from experiencing significant harm.

About the opportunity 

A vital area of our charity’s work is our Musicians Visiting service, which offers musicians who receive long-term support from the charity (due to long-term illness, terminal illness, age, or often a combination of factors) regular telephone check-ins and in-person visits.

The purpose of the call or visit is to provide practical and pastoral support, and to make ongoing assessments of musicians’ needs, ensuring we continue to offer the most appropriate and timely support.

In-person visits take place at the musician’s home or at another agreed meeting place close to where they live, and therefore our team of Regional Visitors operate within agreed geographical areas across the U.K. We currently have opportunities for Regional Visitors to join our team in the following regions:

Our Regional Visitors are allocated to musicians within their assigned geographical location and undertake an agreed number of calls and visits across the year. Approximately 35 in-person visits are required per year but can fluctuate depending on the geographical location need and applications we receive from musicians. 

As self-employed/freelance workers, our visiting team are paid an agreed rate for each call or visit. 

About you 

If you are based in Scotland or the West Midlands and are interested in using some of your time across the year to support musicians through our vital work, this could be the opportunity for you. 

With a passion for music, if you have experience of working in the health and social care sector, ideally with previous experience of conducting home visits to individuals, and you can offer your services for an agreed amount of time across the year, then we would love to hear from you.

It’s essential that you have the skills to manage your own workload, actively listen, and communicate with empathy. If you have experience of working within the music industry, that would be an advantage. 

Other requirements

• Our calls and visits are made to musicians between 9.30am-4pm Monday-Friday so you must be available to undertake work between those times (i.e. this is not evening / weekend work)

• The role will involve lone working, including in-person visits to the homes of musicians, so you must be comfortable to lone work and travel – reasonable travel expenses are reimbursed in line with our expenses policy. Suitable lone working training will be given.

• Due to the geographical location of most in-person visits, a full driving license and willingness to drive is essential (reasonable adjustments will be considered for applicants with a disability).

The Health and Wellbeing Coach will be integrated into the wider multi-disciplinary team of a PCN and Primary Care practice working alongside Social Prescribing Link Workers and practice staff. 

The Role

As Health and Wellbeing Coach for Mental Health, you will provide one-to-one and group support (where appropriate) to people who are referred to you by GPs and the wider PCN team, providing low intensity interventions to support patients to manager their own recovery and self-management.  Sessions will focus on motivational interviewing, goal setting and action planning with support spanning a short time frame.  Patients who are unsuitable for the service should be referred to higher level mental health support through approved pathways. 

The Health and Wellbeing Coach will manage a caseload of patients and maintain accurate record keeping to support outcome monitoring and understand trends and emerging needs. 

The role will build trusting relationships with people, devising shared plans and agree type of intervention with patients which may include online sessions, group and one to one support in practice or community venues, or telephone support. 

The post holder will create strong relationships with primary care staff and wider organisations including Social Prescribers, VCSE Organisations and specialist support agencies as well as Mental Health services in the area. 

About You

Required skills and experience:

• Have experience of supporting people in a mental health setting, be this through clinical practice or support work
• Be a good listener who is able to motivate patients to change using recognised techniques
• Have the ability to manage own time and workload to provide an efficient, effective service to patients and the PCN
• Make decisions on suitability of the service for patients and flag to colleagues where a referral is inappropriate or a patient requires a higher intensity intervention or service
