About the Accelerating Reform Fund
The Accelerating Reform Fund is a new initiative which aims to transform social care services, making them more responsive to the needs of the community. It is a national programme, with 42 regions allocated to have their own projects. Greater Manchester is one of the ARF regions. The ARF has a particular focus on unpaid carers and aims to boost short-term breaks for carers and improve hospital discharge processes. This work is made possible by Department for Health and Social Care grants. As an organisation, Gaddum has supported and collaborated with unpaid carers for many years, so we’re excited to bring our experience to the table. This role will be based in the Manchester team working to improve hospital discharge processes.
Job Summary
To develop and ensure safe discharge planning and information for patients and unpaid carers. To embed procedures to raise awareness of the needs of carers in a secondary care setting, working in partnership with the discharge, virtual and enablement teams. To empower carers to be involved in the discharge process.
Job Purpose
The purpose of this role is to provide operational leadership and oversight of the Carers Hospital Discharge Pilot in Manchester. The role involves line-managing a team of three Hospital Discharge Workers across multiple hospital sites, ensuring effective collaboration with hospital staff and community services to support carers. The aim is to enhance the identification and referral of carers, ensuring timely and appropriate support to prevent carer breakdown and facilitate smoother, safer, and more efficient hospital discharges. We are seeking an experienced and enthusiastic Youth Engagement worker, to join our Somewhere For Us Service team. The role of the service is to ensure that children and young people feel supported on their grief journey. The service is co-designed with our Youth Advisory Group. The support is provided through a range of group and 1-1 supports, including Understanding Your Child’s Bereavement online sessions for parents, group support and activities for children and young people and 1-1 support, grief ally/buddy etc.
Workshops are delivered in the community to individuals and groups who work with children and young people, to help increase their skills and knowledge on how to effectively support children and young people going through a bereavement.
The Somewhere For Us Service supports bereaved children and young people aged 8 – 25. It is funded to June 2027, through the National Lottery Community Fund Empowering Young People funding stream.
There are 2 posts, each for 32 hours per week, one based in our Foyle office and one in Belfast office. You will be responsible for the delivery of a range of bereavement support to children, young people, families and communities across the Western and Northern Trust Areas (Foyle), Belfast, Southern and South Eastern Trust Areas (Belfast) and wider NI, if required. Ability to travel independently across NI to fulfil the role is essential.
You will receive training in bereavement support and have access to high quality CPD.

Aquarius Action Projects, is looking for a Floating Housing Support Practitioner to join their team. Solihull Integrated Addiction Service (SIAS) is a partnership between four organisations jointly responsible for the delivery of the drug, alcohol, homelessness and gambling services, in the Borough of Solihull. We offer services for adults who use or are affected by substance use, homelessness and gambling.
Your challenge? As a Floating Housing Support Worker, you will be working with clients who are vulnerably housed or homeless, working in clients’ homes and various locations to support clients in addressing their substance needs, finding accommodation and being able to live independently.The service covers support for clients who are homeless and at risk of becoming homeless, as well as a more structured longer-term floating support service to enable clients to maintain their tenancies. You will understand the barriers clients face to accessing services, and the impact substance use has on sustaining accommodation.
To succeed, you’ll:
- Have a qualification in health/social care, youth and community work (e.g. NVQ Level 3 or above, DipSW, Mental Health Nursing, Counselling, Addiction Studies). Alternatively, we’ll consider candidates with experience of working in the substance misuse field with a commitment to complete NVQ Level 3 Health and Social Care.

Citizens Advice Milton Keynes is a local charity that helps people to resolve their legal, money and personal problems. We have been providing free, confidential, and impartial advice since 1972 and helped over 8000 local people last year.
We are looking for an enthusiastic, reliable and confident person to join us in delivering a new project based in local GP Surgeries. The ideal candidate will provide a Social Prescribing Link Worker service, to patients of Ascent Primary Care Network (Ascent PCN) in Milton Keynes, working to support patients and staff at each of their 3 medical centres across the area.
The Link Worker will take referrals from the PCN’s practices, and from a wide range of external agencies, and through self-referrals. They’ll work with people to identify issues that affect their health and wellbeing and co-produce a simple personalised care and support plan that connects them to non-medical community-based activities, groups and services that meet practical, social, and emotional needs.
This is a challenging role, managing a demanding caseload, working to strict quality standards and playing a full part in multi-disciplinary team of health and social care professionals. Successful candidates will be able to ensure that all work is compliant with our contract funder – Ascent PCN, and Citizens Advice standards.
Applicants need to be –
- Computer-literate, although training on our case recording systems will be given.
- Able to travel between each of the 3 surgeries in Milton Keynes.
- Confident in working to performance targets.
- Able to show good communications skills, both written and verbal. These are essential in the role, as are excellent customer service skills with a non-judgemental approach.

Purpose of the post:
We are looking for a Community Development Worker with experience supporting with people with their mental health, with significant experience in statutory and/or voluntary sector services, knowledge of the City of London, Hackney and Waltham Forest (or a specific local area or demographic), and strong interpersonal skills.
You will work closely with individuals from Black, African and Caribbean Communities, with serious mental illness (SMI) developing an understanding of complexity in order to provide the most appropriate support.
Working alongside the Director and Clinical Operations Manager you will, be responsible for linking in with Secondary Services in organising and delivering tasks which facilitate an informed and safer hospital discharge and smooth transition into the Community to avoid re-admission.
As a Community Development Worker, you will become a local expert, gathering and sharing information about local opportunities, activities, and support, bringing people together and supporting them to remain confident and independent in their everyday lives.
Main duties and responsibilities:
1. You will be responsible for linking with Groups and Services to understand the mental health needs across the Community.
2. To work closely with the Hospital Discharge Teams as part of the Discharge process
3. Collectively develop a robust Hospital Discharge plan that will help embed the Clients into the Community.
4. Explore with Secondary Services and other relevant agencies the current provision and identify the gaps and assist IRIE Mind and its partners to develop programmes to meet the identified need.
5. To raise awareness of the importance of talking about Mental health and seeking support early, especially in the Black, African and Caribbean Communities.
6. To provide information on where to go for additional support upon Discharge
7. Collaboratively develop material with colleagues to distribute to in patients and Clients, families and friends.
8. Regularly organise and support relevant Outreach events to bring people together/make connections.
9. To play a lead role in identifying and initiating Outreach to new client Groups (Young Black Men and Complex needs) and develop new projects.
10. Develop and establish local networks of support that will meet the religious, cultural, occupational, therapeutic and recreational needs of the Clients.
11. To identify partnership opportunities in working with local community colleges, employers, carers, care co-ordinators, and Clients.
12. Consult and liaise with the Mental Health Recovery Teams, Hospital staff, Psychiatric nurses, social workers and other providers in order to promote the service and consult regarding Client progress, achievements and concerns.
13. To encourage and develop active participation of Clients in the running of IRIE Mind, and in the planning and development of activities,
14. To develop effective liaison arrangements with statutory and voluntary agencies in the community to enable Clients make full use of community resources in the borough
15. To manage your own small caseload of complex clients within the Community and in rare cases, their homes.
16. Record all referrals, actions, progress and data via a CRM/Database.

The Asylum Support Appeals Project (ASAP) is a small national charity which was set up in 2004 to reduce destitution by protecting people's legal rights to food and shelter. We have a team of 12 permanent staff members and 45 volunteer solicitors and barristers.

- High-quality legal representation at the Asylum Support Tribunal, for people appealing against Home Office decisions to refuse or withdraw their accommodation and financial support.
- Training, an advice line and online forum to support hundreds of frontline organisations, advice agencies and legal practitioners across the UK who are helping people apply for financial support or accommodation.
- We use our strong evidence base to improve government policies and procedures on asylum support through policy work and strategic litigation.
The Women’s Legal Advisor works across the three strands of ASAP’s work to promote and protect the rights of women within the asylum support system. It is a varied and interesting role spanning legal representation at the Asylum Support Tribunal (AST), the provision of advice on ASAP’s Advice Line, the delivery of training, and influencing work with the Home Office. The post is currently focused on access to support for those experiencing domestic abuse however, the future focus of this project will be one of the first priorities for the new post holder, after induction.
A strong interest and commitment to upholding the rights of women is essential, as is an understanding of the barriers and difficulties that women seeking asylum face. You will work closely and collaboratively with colleagues internally, providing advice and support on issues impacting women in the asylum system. You will work with partner organisations externally, pursuing asylum support policy improvements for women and supporting advice workers to assist women to access housing and financial support. The role is part of ASAP’s policy team as well as our legal team (staff members providing legal representation at the AST and via our advice line). The Head of Legal provides legal supervision to the role, and assistance with legal accuracy of training materials.

We have a vacancy for a Flourish (Refugee and Asylum Seekers) Community Outreach Worker to join our team, supporting refugees and asylum seekers in the community with multiple needs who have mental health as a primary need, through providing advice, information and holistic support.
About the Service
This is an exciting and reasonably new project, ‘Flourish’ – Mental Health for Refugees in Hertfordshire, which is a highly flexible and responsive outreach service developed to meet the needs of refugees fleeing the war in Ukraine & other refugees in Hertfordshire. The aim of the service will be to support refugees in the community with multiple needs who have mental health support as a primary need.
The countywide holistic service also provides emotional wellbeing support for families who are hosting refugees.
Purpose of Post
- To provide advice, information, onward referral and holistic support to clients with mental ill health.
- Support to build social networks and reduce isolation whilst settling into the Hertfordshire community this includes building a sustainable and empowering peer support network and building confidence and self-esteem.
- To ensure that the safety and wellbeing of the client is monitored and reviewed regularly.
- To remain a source of independent support for all clients.
- Embed a person centred and solution focused approach in all aspects of the role.
- To work in a trauma informed way including awareness of the 6 principles of trauma informed practice, putting the clients emotional and practical needs at the forefront of this work.
Key Responsibilities:
- Working closely with all key delivery partners across the system to ensure the service is responsive, inclusive and easy to access.
- To take allocated referrals from any agencies including the Community Navigators, Borough and District Councils, Herts Help, Housing Providers, Herts Welcomes Refugees, the Refugee Council and self-referrals including walk in contacts at any of the Mind wellbeing centres across the county.
- Hold a caseload of clients, supporting for approx. 10 weeks
- To create a safe space for the individual to be able to explore their feelings and improve their understanding around their mental health.
- To complete a holistic needs assessment.
- Ensure all person centred risk assessments and support plans are completed.
- Proactively recognise the indicators of deteriorating mental health and facilitate appropriate action, whilst liaising with relevant agencies.
- Support clients to build their emotional resilience, promoting self-management through building an understanding of their own triggers/coping strategies.
- To provide safeguarding intervention support. This will include responding to problems, which pose an immediate risk.
- To work alongside translators when required to provide a robust package of support for clients.
- To accompany clients to initial visits and appointments as necessary in order to ensure the sustained use of existing support services in the Borough.
- To provide advocacy services to clients experiencing mental ill health.
- To assist clients to access appropriate mental health services, housing/tenancy services and other services according to assessed needs.
- To establish effective pathways across a variety of agencies to maximise and provide effective partnership working, working with other third sector and VCSE organisations to provide signposting and continuity of care.

Senior Director of Development and Strategic Partnerships
The Senior Director of Development and Strategic Partnerships will lead fundraising, outreach, and engagement for the WRC at the level of both strategy and implementation. The WRC’s current annual budget is US$3.3 million, and we seek to grow this in line with our program strategy to roughly $5–6 million over the next two to three years.
Areas of responsibility include:
- Designing and implementing a comprehensive fundraising strategy to progressively increase annual revenue, including identifying, building relationships with, and securing new funding from foundations, other institutional funders, and individuals, as well as pursuing opportunities to expand relationships with current funders;
- Creating and implementing a framework for strategic external partnership communications, including working with the WRC’s global field team to convey the real-world impact of the WRC’s work and overseeing relevant enhancements to the WRC’s website and other communications pathways;
- Leading on grant writing and framing of external communications;
- Supporting the Executive Director and other colleagues to engage with, and ensure coherent messaging to, donors and other partners—including ensuring that the WRC participates in relevant conferences, convenings, and other discussions related to our organizational mission and objectives;
- Supervising the work of a second development professional who has day-to-day responsibility for grant management, grant reporting, implementation of sub-grantee/grantor arrangements, management of the organization’s small individual donor program, and who also assists with preparation of grant proposals;
- Monitoring progress on grant-funded projects through regular communication with program staff;
- Participating in the development of overall organizational strategy and aligning fundraising strategies accordingly; and
- Contributing to financial reporting and oversight.
This is a full-time position, available immediately. The position is hybrid for a successful candidate who resides in the Washington, DC-area or remote in the case of a candidate who resides elsewhere.
The WRC will consider candidates located within six hours of the US Eastern time zone. For international candidates and US candidates based outside of Washington, DC, the position requires periodic travel to Washington, DC, and possibly other countries in which the WRC works. The Senior Director of Development and Strategic Partnerships will report to the WRC’s Executive Director.
Qualifications
Candidates must have:
- A minimum of seven years’ experience in fundraising for nonprofit organizations and/or seven years’ combined experience with fundraising and strategic communications;
- A substantial track record of successfully cultivating institutional donors and proven experience in establishing new fundraising partnerships;
- Significant experience in communications and storytelling, with an ability to distill complex analysis and program work into communications that inspire and motivate our audiences;
- Exceptional written and oral communications skills;
- Excellent interpersonal skills, including the ability to initiate, build, and maintain working relationships with existing and potential grantors and a global team across numerous time zones;
- Strong budget management skills and experience delivering against financial targets;
- Excellent organizational and project management skills and rigorous attention to detail;
- Energy, enthusiasm, a strong work ethic, and a sense of humor; and
- A commitment to workers’ rights and corporate accountability.
The following are desirable but not required:
- Experience with labor rights work; and
- Experience working in an international context.
Compensation
Compensation for this position is $115,000–$135,000 per year, depending on relevant experience (adjusted to the local currency, where applicable). The WRC provides its US staff, who are covered by a union contract, with an excellent benefits package, including 100% employer-paid family health insurance, including dental and vision care, with an additional flexible spending plan with employer contribution; 401(k) retirement plan with employer contribution; and five weeks paid vacation.
