Hospitals need to free up beds by discharging patients who do not need to remain in hospital. Despite the pressures we want discharges to be safe, successful and as painless as possible for patients. We also want to ensure a patient is not readmitted into hospital.
The Hospital Discharge Community Support Coordinator will match Age UK Lambeth (AUKL) volunteers and partner organisation volunteers to support a patient's discharge. Volunteers could provide support with transport, practical support such as food shopping or completing home safety checks. Upon discharge volunteers may continue supporting patients for up to 4 weeks, providing support in person (at the individual's home or in the community) and over the phone, linking them into support services and helping them access the community to prevent hospital readmission.
What you’ll be doing?

Oversee the development of a new, inclusive volunteer programme that offers a range of diverse and challenging volunteer roles to support patients leaving hospital and once settled home.

Work in partnership with the VCS to create 100 volunteering opportunities to support hospital discharge.

Using the volunteer platform, Care for Me, track allocated tasks to ensure they’re completed. Monitor volunteer reports to review the support and work delivered under the project.

Develop and maintain strong working relationships with King’s College Hospital and Guy’s and St Thomas’ Hospital to ensure appropriate referrals are made into the service.

Main Responsibilities:

· To lead and support safe, reliable, high-quality family support service across the district and integrate with existing provisions to ensure that services are not duplicated and multiagency working is upheld. To ensure communication is established with other services to establish a culture of mutual trust and respect.
· Prioritise referrals, manage work allocations and ensure effective deployment of the workers to ensure service users receive a timely and effective service.
· Ensure all appropriate staff development and core training requirements are fulfilled in the team and contribute to wider training plans for the locality. Foster a culture of continuous development including best practice sharing across teams and districts, involvement in Community of Practice etc.
- Respond appropriately to any safeguarding concerns, demonstrating an understanding of, and always complying with Family Action’s and Gloucestershire procedures for promoting and safeguarding the welfare of children and vulnerable adults. To ensure compliance with relevant local and national legislation. Act as a safeguarding Lead for the locality.
- To have an overview of the support needs of all families across the locality and Gloucestershire as a whole, and knowledge of how these are being met in order to influence the direction of service planning.

As Policy Manager for health and wellbeing, you will be working as part of the policy and public affairs England team, managing specified areas of policy and influencing work and helping to deliver our operational plan. The Clinical Service team in the Together Trust is at an exciting time of development. This post offers the unique opportunity to join a newly established Psychology Service and to work systemically in multi-disciplinary teams to contribute to trust wide and externally commissioned initiatives and support the needs of individual pupils.
In this role you will be working in Specialist Together Trust settings.
You will draw on knowledge and expertise in neurodiversity and work closely with settings that support learners with autism and social communication differences. You will work with pupils with significant social and emotional needs and the teams around them, using trauma-informed practice. You will support the needs of learners with complex learning and developmental needs, and you will have opportunities to work in other settings on externally commissioned projects across the region, including work in mainstream and community settings.
You will work in a variety of ways through training, problem-solving and collaborative consultation and advice and support to teachers, parents/carers, SENCOs, the wider community, as well as children and young people. You will be able to build relationships with those you support, take a reflective approach and work in cycles of ‘assess, plan, do review’ to achieve the best outcomes for children and young people. You will use psychological assessment and produce reports to contribute to an understanding of a child’s needs and work in collaboration with others to develop shared and person-centred plans of support.
You will be supported by and will support a small team of Educational Psychologists and Assistant Psychologists using formal and informal supervision, and you will be able to shape and contribute to the development of a service that is inclusive and celebratory of the experiences of the thoughts and ideas people bring. An exciting opportunity has arisen for two Community Care Coordinators to support some of our most vulnerable users of emergency services. These roles have been developed to further support our service users alongside our statutory partners at North Middlesex NHS Foundation Trust Emergency Department, to increase their wellbeing, confidence, social integration, and independence.
The successful candidates will be responsible for working with service users during periods of crisis and support them through their journey of recovery to ensure that they are receiving the support most beneficial to them. The Senior Programmes Manager: University Access will play a pivotal role in the University Access and Digital team, overseeing and managing the delivery of high-impact interventions, including UK Summer Schools, The Sutton Trust Fulbright US Programme and Teacher Champions, that support young people from underrepresented backgrounds in accessing highly selective universities. The role requires someone with a strong understanding of the barriers faced by young people from disadvantaged backgrounds, and a commitment to supporting them in reaching their potential.

About the role
The Creative Minds Programme funded by the National Lottery, is designed to enhance mental health and wellbeing for adults in West Kent through engaging in creative activities.
These 24-week programmes are held in various locations across West Kent and are conducted in collaboration with professional artists, local arts organisations, and supported by the West Kent Mind community wellbeing team.
Open to adults (over the age of 18) in West Kent, the programme offers inclusive, free sessions in a wide range of artforms.
Key Objectives include boosting confidence, fostering connections, enhancing life skills, and increasing participants' ability to manage their mental health and support others.
Now in its second year, the programme aims to support 864 adults over three years, with a focus on those with mental health challenges or feelings of loneliness. In addition to creative workshops, the programme offers leadership training to select participants, empowering them to become "Creative Agents." These agents lead "Creative Places" to engage the wider public in community-based creative activities.
The programme is set to conclude in March 2026.
Who you are
All previous experience may be paid or voluntary, full, or part-time, in the UK or overseas.

We have an exciting opportunity to join the South East Rivers Trust's Finance and Operations Team. We are a small dynamic team, sitting at the heart of the organisation. The role reports directly to the Finance Manager and will provide support in administering all financial controls and processing for the organisation. The successful applicant will come from a financial background with a strong understanding of financial principles, an accounting qualification and proficiency in Excel. They will have experience in bookkeeping, reconciling accounts as well as dealing with banking, grant applications and expenses.
The role would suit an individual, who is approachable and organised, with excellent attention to detail, able to work on their own or as part of a team. The role involves liaising and following up with all teams to ensure the data provided is accurate and timely, to support grant applications, payments and audit requirements.

Person Specification
Essential

Experience of encouraging charitable giving.

Experience managing a wide variety of professional relationships.

Educated to A Level or equivalent standard.

IT proficiency (MS Office suite).

Fluency in spoken English and ability to communicate clearly in English.

Understanding of the ethos and mission of the Church of England, and of the role of parish clergy.

Effective written and oral communication skills.

Numerate and financially astute – comfortable working with financial data.

Strong administrative skills, including use of Microsoft Office.

Self-motivated, as work needs to be undertaken proactively and with limited supervision.
