Taking referrals from GP practices, you’ll work with adult mental health patients to identify what is affecting their health and wellbeing. You’ll then work with them to create a personalised support plan to improve resilience, health and wellbeing. You’ll find services and activities that offer practical, social and emotional support, working as a team with other healthcare professionals who may provide more traditional medical interventions.
This might mean connecting people with art classes, walking groups or gardening clubs, or it could be signposting to more practical support, such as services providing advice on debt or housing issues. It’s essential that you can listen carefully, build relationships, and motivate people to make choices and changes about the things that matter to them.
The position is based in Sheppey and applicants must have experience of working with adults in a medical or social care setting, and of providing mental health support. Additional experience of providing mental health support to children and young people would be an advantage as a small number of referrals are from this group.
You should be organised, self-motivated and able to work as part of a team and on your own. Car driver essential.

Job Purpose
This role sits within our crisis alternative service, Safe Space, which is a core and out of hours service providing a safe and welcoming space for people who are feeling distressed and experiencing crisis. Our aim of the service is to keep individuals well in the community and prevent readmission into hospital. The service operates 365 days a year from 14:00-22:00pm across all of our boroughs.
Staff within the service will provide person-centred, practical and emotional support, face to face or via telephone or virtual on a one-to-one basis to individuals experiencing mental health crisis or preventing crisis.
The objectives of the service include:
- To improve the mental wellbeing of people experiencing mental health and social crisis in HFEH.

Purpose of the Job
An exciting opportunity has arisen for a clinical lead working within our Mental Health Support Teams and wider Children and Young People’s Services. As an integral part of our services, you will ensure the high-quality delivery of mental health interventions for children and young people. You will provide clinical supervision and support to a multi-disciplinary team, ensuring adherence to evidence-based practice and national guidelines. This role involves overseeing the delivery of low-intensity and early intervention mental health support within schools and the community for children and young people between the ages of 5-18 (for young people with SEND under 25). In addition, you will ensure professional development and clinical governance.

The role includes:
- Providing case management and clinical supervision (1:1 and group) and some line management
- Supporting the team to write sensitive and appropriate reports concerning the assessment, formulation and treatment plans of children and young people

Job Title: Mental Health Rehabilitation Worker
Location: In service based in Catford across two sites (SE6 2QB & SE6 3BE)
Salary: £27,000
Shift Pattern: 37.5 hours a week over a rota which can include 12.5 hour shifts (including breaks) Monday to Sunday (Including weekends, and bank holidays) 08:00 - 20:30. We also have a night rota between 20:00 - 08:30 which is not a part of this role but desirable if there is flexibility around working patterns
About the role
We are looking for a Mental Health Rehabilitation Worker to work within our Integrated Community Rehabilitation Service which provides rehabilitation support to residents who face challenges with their mental health, and have been discharged from acute mental health wards to start structured rehabilitation support for 9 months in the community. You will work directly with our residents to support them to overcome their personal barriers and challenges to achieve positive outcomes. You will do this through one to one support sessions, group sessions and activities, and general support with their daily living. This is not a personal care role, but is a role to rehabilitate and empower our residents towards independent living.
The role further includes:
- Supporting residents with taking their correct medication
- Carrying out various health and safety tasks, including reporting repairs and maintenance
- Keeping support plans up to date using our online portal
- Plan and deliver various activities which includes within the service and outside in the local community. This can include days out to museums and other areas of interest by our residents
- Carrying out housing management support including welfare benefits support
The service works closely with a dedicated clinical team provided by South London and Maudsley NHS Foundation Trust (SLAM). The clinical team includes an Occupational Therapist, Psychologist, and Psychiatrist, complimented by a Substance Misuse/Dual Diagnosis worker in our internal team. The Rehabilitation service works with adults aged between 18 and 65, from Croydon, Lewisham, and Southwark. You will be part of a service which offers 24 hour support across 2 buildings in Catford.

How Will You Make a Difference?
Amna is seeking a compassionate and experienced Arabic speaking facilitator to join Amna’s team of trainers, with therapeutic/clinical background. As a Trainer & Facilitator your responsibilities will involve delivering training sessions and group facilitation to support and create learning spaces for training participants on themes centred around trauma and identity informed psychosocial healing care with displaced communities. The role holder is expected to work on multiple responses, and will lead Amna’s Gaza and Syria responses, while working closely with Arabic speaking team members.
Amna’s identity and trauma informed approach is centred around community healing practices with communities that aim to restore participants sense of safety, belonging, agency and joy. Amna services are co-created with the communities being worked with and we prioritise refugee participation and leadership in service design and delivery wherever possible.
The role holders need to have experience working with communities affected by adversity, including displacement, war and conflict, poverty, abuse or marginalisation. Given the nature of Amna’s work in emergency settings with displaced communities, applicants need to be comfortable working in environments where there is uncertainty, change and exposure to distress.

About the Service
Hertfordshire Mind Network’s Nightlight Service is proud to provide urgent support, crisis intervention and emotional support to people experiencing Mental health Crisis in Hertfordshire as an alternative to statutory pathways. We provide support to adults across Hertfordshire 7 days a week, 365 days a year through our Helpline, Crisis Cafes and a 24/7 Crisis House. We aim to provide a safe space for people in crisis to talk and feel supported, be listened to without judgement, access relevant support with problem solving and facilitate access to external services when necessary.

About the role
As a Crisis House Support Worker, you will work closely with other staff as part of a team in supporting the development of quality Crisis Support and instilling Hertfordshire Mind Network’s values throughout the provision.
Purpose of Post
To be a key member of staff supporting the delivery of the Nightlight mental health Crisis Service. Drawing upon your own lived and/or professional experience (where applicable,) in order to provide a mix of face-to-face, therapeutic and holistic support for those accessing overnight provision.
Key Responsibilities
- To provide emotional support and work towards developing, flexible and realistic crisis/person-centred plans with Service Users.
- To work within agreed guidelines with and for Service Users, whilst being guided and supported by Senior Staff (as well as the Leadership team) in regard to documents/plans such as: Assessments of Need, Risk, Crisis support plans and Review forms.
- To draw upon one’s own lived experience (If applicable) as appropriate, as to promote the delivery of person-centred support.
- To provide immediate and appropriate advice and information to clients as to support with crisis prevention.
- To liaise with Nightlight management and external services such as: police, ambulance service, Crisis Resolution Home Treatment Team, Single Point of Access, Safeguarding Out of Hours Service, Mental Health Liaison Teams, Adult Community Mental health Services, etc. as needed in terms of accepting referrals and proactively recognising the indicators of deteriorating mental health and facilitating appropriate support.
- Complete appropriate signposting/onward referrals to both internal and external services in accordance with identified needs and risks.
- Arranging transport for service users to facilitate access to services as needed.
- Support clients with activities of daily living as required.
- Ensuring record keeping is maintained to the required standards at all times and contributing to service monitoring requirements.

About the role:
This is a great chance to be a part of a service working alongside the Healthcare & Education Department within HMP Bronzefield to identify and support women who are identified as being neurodiverse and will be returning to the community. Of that cohort, the Advocate will focus support on women with a mental health need and improve their transition into the community, with ongoing community support; including collaborating with other healthcare professionals to their develop support plans.
The Senior MH Advocate will work in a multi-disciplinary way, including attending the weekly complex case meeting and/or the Safety Intervention Meeting (SIM) as appropriate, they will act as a specialist member of the wider Minerva Criminal Justice Service - London team, to facilitate a pathway for women with complex needs including mental health and neurodiversity needs. The Senior Advocate will create a link between prison and the community, helping women to navigate support services and to positively re-integrate into their community upon release. They will co-design a person-centred support and action plan with women accessing support, enabling to support them to address their needs and any risks. The role will combine a casework- based approach, along with a signposting and advice service for the women.
The Senior Mental Health Advocate will be based in the community and will provide a drop-in service (1-2 day per week) in HMP Bronzefield to support women who are close to release. The role will also include line management of other advocates such as; Specialist Prison & Probation Advocates who will be based in the prison and the Loss & Bereavement Worker who will be providing support in the prison as well as in the community.
A car may be desirable for this role, though not essential
About You:
To be successful as the Senior Mental Health Advocate you will need the below experience and skills:
An excellent understanding of mental health, neurodiversity needs violence against women and girls and its links to women in the criminal justice system
Experience of managing/supervising a team of advocates/caseworkers – and leading a team to achieve targets & outcomes as well as appropriately managing and leading the team on any safeguarding concerns/incidents.
You will have the ability to complete trauma informed, support and action plans in collaboration with the woman; to support in addressing their multiple and individual needs and enable them to engage with services, which will result in timely and prescribed outcomes being achieved.
You will possess excellent organisational skills, excellent communication skills and be able to work in a prison environment whilst remaining calm.

We are seeking a motivated and experienced individual to take responsibility for the day-to-day work of our innovative Hearing Voices Projects. To be successful in this role, you need to have an in-depth understanding of the Hearing Voices Movement and the role of peer support groups within this. You will ideally have experience of supervising or mentoring volunteers and some project management experience.

As such, this role is best suited to someone who is flexible enough to modify their approach to suit the situation.
We are looking for someone with strong facilitation skills who is able to deliver and design training that communicates the values of the Hearing Voices Network clearly and accessibly.
Among other tasks, you will be required to:

South East London Mind has been delivering a young person’s counselling service to students at Southwark College for several years.
As a Young People’s Counsellor, you’ll provide an age-appropriate counselling service that allows the students to explore their mental health and wellbeing, along with the socioeconomic factors that impact this. You’ll manage a caseload of students, triaging new clients to the service, coordinating the waiting list and capturing data and outcome measures on a CRM database.
We are looking for someone with a professionally-recognised diploma or equivalent in Counselling or Psychotherapy who has experience of providing counselling to young people aged 14 upwards. As you’ll be working closely with pastoral staff on-site, an ability to work independently but collaboratively, to be proactive and show initiative is essential for this role.
The role will involve:
- Offering age-appropriate counselling sessions, including assessments and risk assessments
- Working with teachers and pastoral staff to ensure that students are triaged and the relevant level of care and support is offered
- Record keeping: maintaining up-to-date records of work undertaken, including session notes, service records and Views CRM database records
- Contributing to quarterly monitoring reports, including reflections on key themes and issues experienced by students, case studies, and outcome measurement tools
