About the role:

The Gray's Inn Road supported accommodation project is a 16-bed short stay accommodation service which holds a critical position in the endeavour to address rough sleeping in the London Borough of Camden. The service, funded by the Department of Levelling Up Housing and Communities and the Greater London Authority, in partnership with One Housing Group and Camden council aligns with the strategic vision to make rough sleeping rare, brief, and non-recurrent in Camden. Although there is not a defined maximum stay, the service is designed to be a ‘short stay’ project and will aim to move people on into suitable housing options after 3-6 months.

As a resettlement worker you will provide flexible housing related support to vulnerable clients across the service. Clients within the service will often be experiencing multiple disadvantages, including substance misuse, mental and physical health issues and contact with the criminal justice system.

The resettlement worker will need to be resilient, innovative, flexible and creative, working alongside support workers to support clients in becoming move on ready. You will also need to be able to facilitate appropriate move-ons from the service by identifying tailored move on support for each client dependent on their support needs, which will require knowledge of housing pathways.

The resettlement worker will be expected to develop strong relationships with housing providers, including private rented sector landlords.

About you:

• A demonstrable understanding of the Recovery Model, and the ability to engage and motivate people to move towards independence and inclusion.
• A good understanding of the housing pathway to ensure effective and suitable move on options are offered to clients. This includes an understanding of the private rented sector market and ability to broker safe and affordable Move-on options.
• A proven ability to create and deliver SMART Resettlement action plans with clients through high quality risk and needs assessments.
• A working knowledge of Housing Management as applied to a residential setting, and / or client’s move on accommodation.
• A level of numeracy sufficient to carry out tasks such as budgeting with clients, calculating benefit entitlements and monitoring rent payments.
• A proven ability to work in partnership with external stakeholders, particularly community mental health services and/or substance use services.

Functional Responsibility: The post holder will work with existing partners in targeted support of successfully engaging with those on the SMI register in accessing and attending physical health checks

Job Purpose:

To offer outreach and engagement, to support people with SMI to access their annual comprehensive physical health check.

Based at: Outreach based role based in the community, GP practices, clinics in the borough of Queens Park, Paddington and North Kensington, with office base at GWS or Monck St.

Main duties and Responsibilities

Operational

Accountable for leading all activities within Community Programmes portfolio including, but is not limited to:

- Offer outreach and engagement, to support people with SMI to access their annual comprehensive physical health check.
- Conduct an outreach program to reach people with Severe Mental Illness (SMI) who are not currently accessing annual, comprehensive, physical health checks and
- Raise awareness about eligibility for people living with SMI (priority group 6)
- Provide support for people with SMI to access physical health checks and discussing their concerns
- Provide a tailored one-to-one engagement service for these identified patients to overcome barriers to them accessing, and, getting value from their SMI health checks.
- (BMI, BP & Pulse Check, Blood Lipids, Blood Glucose Tests, Alcohol Consumption, Smoking Status)
- Signpost – including smoking cessation is an indicator
- Working to engage and build relationships to better understand individual barriers and/or personal reasons that makes it difficult to take up the offer of one of more health check/s
- Agree a plan with the person to support their physical checks
- Building solid relationships with a wide range of statutory and voluntary agencies in North K&C in order that they can offer additional support and link the individuals in to other appropriate services, to resolve any non-health issues which may be acting as obstacles to accessing SMI health checks
- Working flexibly with primary and community nursing professionals inc. CNWL Physical Health Nurse (s), and Community Mental Health Hubs to identify the most appropriate route to access health-checks
- Giving people practical support (such as help with travel fares, reminders and accompaniment to appointments)
- Help prepare people for appointments and, where appropriate, with understanding the content of the appointment
- Explaining the roles of different professionals in the health service and enabling co-ordination of appointments
- Support people to secure access to other services
- Building the skills, confidence and knowledge that will enable patients to access health services independently and appropriately
- The Provider will support the recording of health checks on relevant systems. Subject to access to system 1
- Undertake training in the use of the relevant clinical record system in primary care
- Carry out an audit of patients who have missed one or more of the physical health-checks, cross referencing between practice and secondary mental health care records (where such access arrangements are in place) · Input completed health-checks into the relevant database · Ensure that data is managed in compliance with General Data Protection Regulations
- Ensure completion of patient experience questionnaires and collation of them to facilitate reporting against KPI’s for service.
- Dispel misinformation (by, for example, distributing flyers, brochures, or other informational or educational documents)
- Educate people about the importance and availability of healthcare services, such as cancer screenings
- Help individuals to identify barriers / reasonable adjustments that the practice / vaccinating team can make to enable the individual to access their health check / Covid-19 vaccine
- Where appropriate, engage supporters (e.g. family, carers, providers) to offer support in accessing the health check
- If required, liaise with the practice, vaccination team or individual to support access to the health check
- Ensure an inclusive approach for those who are most vulnerable, including BAME populations, older people, pregnant women and those with missed reviews
- Gain insight into the reasons for vaccine hesitancy and the barriers to access the vaccine, to shape wider CCG communications
- Work with existing service users / group attendees where they meet the category of SMI and with people with SMI who are referred by the funder / other identified health stakeholders
- Assist in the collection of information and data in order to monitor and evaluate the service

Policy and Service Development:

- Develop and support service innovation and effective service model
- Ensure awareness of relevant policy and procedures and that communication is developed and deployed appropriately.
- Maintain a good knowledge of emerging policies from government departments, the organisation in defining the strategy. To support delivery of the NHS Long Term Plan.
- Develop and maintain appropriate links with agencies concerned with the health, welfare and development of people with mental health problems
- Work across multiple agencies
- Develop and support on policy development with impact across the organisation

Financial and Physical Resources

- Support the Team Leader with budget setting across a range of areas/services of community Programmes, managing and monitoring related activity, liaising with Finance colleagues to ensure appropriate costings, and ensure compliance with standing financial instructions
- Responsible for ensuring adherence to the Community Programmes operational budget, ensuring appropriate documentation is available for scrutiny. Constantly striving for value for money and greater efficiency
- Support and provide guidance, management, and assurance on the procurement of identified products, equipment, services and facilities for assigned Community Programmes
- Support and develop, recommendations, providing advice and able to prepare strategic reports/briefings as required
- Constantly strive for value for money and greater efficiency in the use of these budgets and to ensure that they operate in recurrent financial balance year on year.

People Management

- Support and develop the Adult Services Directorate i.e., community Programmes, building a collaborative working environment and an innovative culture
- Support, develop and inspire staff throughout the strategy to role model leadership and innovation
- Support and work across the wider organisation to agree prioritisation of blocks of work and related resource allocation

About the role

• Delivering support to single homeless people, who have been admitted into hospital due to their physical and/or mental health needs and to prevent them from being discharged back into a cycle of homelessness or risk at the end of their treatment.
• Liaise with the Hospital Pathways Team to accept and assess new client referrals into the service based on the agreed criteria.
• Conducting person-centred assessments with those who are referred to the service.

You will work with young people between the age of 17-25 with needs related to substance misuse, including those that are affected by their parent’s substance misuse. The purpose of the role is to engage young people in a variety of interventions, using a young person centred approach, empowering them to reduce risk, increase resilience and achieve positive outcomes.

Based in the Transitions team, you will support young people with multiple risk factors and substance use. Our Complex Needs Practitioners work flexibly across the community and within our multi-agency partnership settings. About the role:

Single Homeless Project's in house psychotherapy has secured further funding to provide psychotherapy/counselling and psycho-educational groups for young people (16-25 years old) who are using SHP's accommodation and support provision. The post provides a psychologically informed service to young people and the care teams who support them. 

We are currently looking to recruit a young person's psychotherapist for 6 months maternity cover, who will work as part of a multi-disciplinary team offering in person interventions which are preventions focused rather than crisis management. This will include both individual psychotherapy and psychoeducation group work, which will be co-produced, informative and enlightening on bio-social-physiological responses to trauma; for example abusive relationships, racism and poverty. 

It is highly likely this contract will be extended to 12 months after internal funding decisions have been made, this cannot be confirmed at this time but will be discussed further in interview with any shortlisted candidates. • Minimum one year's experience of providing psychotherapy/counselling young people.
• Experience of facilitating groups and understanding group dynamics.
• Understanding and experience of the difficulties faced by young people and especially of young people who have been in care.

• Understanding and some experience of client co-production.

Role Overview:

Are you an experienced fundraiser, enjoy fostering funding partnerships, and have worked in finance? If yes, an integral role has come up to lead The Resilience Project's Finance & Funding Office. We are looking for a dynamic self-starter to join our team as our Finance & Funding Lead. In this role, you will be responsible for developing and leading our fundraising strategy, expanding our grant application pool, and supporting the financial systems in our organisation. You will work closely alongside our founder and CEO to oversee, manage and drive the relationships and grants that fund our work. This role requires a flexible and experienced finance manager with a history of managing fundraising work and skill in networking.

Key Responsibilities:

• Fundraising Strategy: Lead the design, development and implementation of a multi-year fundraising strategy that considers current and potential partners, sector-specific funding opportunities, and relies upon multiple income streams. 

• Network Engagement: Co-lead in the furthering of The Resilience Project's partnership network with TRP's founder and CEO.

• Grant Application Management: Oversee the discovery and submission of grant applications 

• Strategic Planning: Collaborate with the C-Suite to contribute to the strategic financial and fundraising planning process, ensuring that financial and fundraising objectives align with the organisation's overall mission and vision.

• Finance Management: Oversee the collection and management of cash flow records of the organisation, ensuring clear storage of records. Support in document delivery for quarterly and annual taxes. 

• Budget Management: Lead on the annual creation of budgets, working with the CEO to allocate Office budgets. 

• Monitoring and Evaluation: Lead in the assessment of the impact of the organisation's fundraising efforts, making informed suggestions for continuous improvement in fundraising strategy, in collaboration with the Chief Operations Officer.

• Community Outreach and Ambassador work: Represent The Resilience Project, our mission and values in community events, national and international meetings and summits, actively engaging with the target audience and stakeholders.

• Team Collaboration: Work closely with cross-functional teams, providing leadership to ensure effective collaboration in programme development and delivery.

Essential 

• Proven experience in fundraising strategy development and management, ideally within a nonprofit or community development setting.

Kentown Family Support Worker

About the role:

We are looking to appoint a Kentown Family Support Worker to deliver a high-quality family support service as part of the pilot Kentown Children’s Palliative Care Programme, in collaboration with Kentown Wizard and Together for Short Lives.

Reporting to the Kentown Project Lead out of our Lancaster team base and working in partnership with health, education and social care professionals, you will take responsibility for providing needs-led emotional, social and practical support to families where a child/young person has a life threatening or terminal illness.

What we’re looking for:

· An experienced child health, education or social care professional - applications will be particularly welcome from those who have worked in a community environment and those with a recognised qualification in education, health or social care.

· A warm, inclusive approach to achieving goals quickly and correctly.

· Practiced in child protection, information sharing and the rules around data protection - you lead by example, drawing on your own professional experience and working within established guidelines.

The Role

As GamCare continues to expand, we are looking to recruit new advisers for our Helpline. Helpline advisers play a key role at the heart of GamCare and are the first port of call for those seeking help, advice, and support for gambling related harm. 

This is a great time to join us and be an integral part of an award winning (Helpline of the Year) team, providing vital support to those affected by their own, or someone else’s gambling. As our team of Advisers grows, we will be able to help more people who reach out to us.

Support is offered via our Freephone Helpline, Live Chat (Netline) and digital platforms, including WhatsApp. The Helpline is operational 24 hours a day, 365 days a year.

This role will be worked across a 2-week rota. In each 2-week period you will work 7 nights. Shifts begin at 22:30 and finish at 07:30 the following morning. Full training and supervision are provided, and a full CPD-programme is offered.

Key Responsibilities

• Recognising issues of risk and ensuring they are dealt with effectively, in line with our safeguarding policies.
• Collaborating and engaging professionally with other remote-working colleagues as part of a Helpline team.
• Operating various IT systems including our call handling system, CRM and internal communication platforms.
• Capturing client data in order to improve understanding and to help us tailor our service to those in need of support.
• Moderating our online Forum and Chatrooms on a rota basis.
• Providing advice, support, and guidance to callers, who are affected by problems with gambling.

About you

You will have a professional qualification in Health, Social Care, Psychology, Youth or Community Care, e.g. NVQ Level 3 or above or a Diploma in Mental Health Nursing, Counselling, Addiction Studies, Advice and Guidance. Alternatively, you will have demonstratable experience of working with vulnerable people experiencing addiction harm and/or other complex needs.

We also welcome applications from those who have lived experience of gambling related harm and experience of working in a professional supportive environment, with an understanding of relevant safeguarding legislation.

You will be able to engage with callers getting in touch with GamCare effectively, sensitively and with empathy, and be able to deliver non-judgemental, confidential support. You will be willing to maintain a sufficient level of understanding around problem gambling, and you will have a genuine interest in working within the field of Gambling Addiction, treatment, early intervention, and prevention.

You will also be competent in the use of various IT systems including our call handling system, CRM and internal communication platforms.

You will also be subjected to a Disclosure and Barring Service (DBS) check.
