About the Role
As a Health & Wellbeing Facilitator, you will play a pivotal role in delivering our Long-Term Health Conditions Programme. This includes leading self-management workshops, organising peer support groups, and empowering clients to make positive lifestyle changes. You’ll be a key player in helping individuals build confidence, enhance their health knowledge, and take control of their well-being. Working closely with colleagues and volunteers, you’ll connect clients to valuable services, create support networks, and make a tangible difference in their lives.

What You’ll Do:

• Work with the Lifestyle Coordinator to plan and deliver engaging health and well-being workshops for individuals with ongoing health conditions.
• Facilitate support groups, training and supervising volunteer peer mentors to provide targeted, condition-specific guidance.
• Collaborate with partner organisations to ensure a smooth referral process for additional services like Befriending, Carer Support, and Handyperson services.
• Provide personalised support through various channels, including face-to-face meetings, phone calls, and online communication.
• Help clients develop essential skills, such as managing medication, reducing stress, and communicating with healthcare providers.
• Monitor programme outcomes, maintain accurate client records, and contribute to continuous service improvement.

About You
We’re looking for someone with excellent communication skills, a proactive attitude, and a genuine passion for helping others. You should have experience working with individuals who face health challenges and a commitment to promoting independence and well-being. Knowledge of local resources and services is a plus, as is experience in organising health-related workshops or programmes.

WorkWell is an exciting new government/DWP initiative to get people back to work. As well as providing work placement support, it will also address and support any physical or mental health needs via a Mental Health Provider and Physical Health Advisor who form part of the team. The pilot in Surrey will be going live in December and aims to empower people to take control of their health, wellbeing and any impacts this is having on their employment via referral to a WorkWell Coach. That’s where you come in.

Working as a key part of the primary care network (PCN) and other community hubs, your challenge will be to provide a supportive environment that enables each individual to identify their health and wellbeing concerns and the impact these are having on their employment. That will involve taking a strengths-based approach, focusing on ‘what matters to me’ and, together with the individual, producing a personalised and holistic support plan that aims to help the individual thrive at work. We’ll also rely on you to work alongside a multi-disciplinary team of physical activity, mental health and skills and employment advisors to provide direct interventions and connect individuals to diverse community-based support, thus ensuring they are able to maintain their health, wellbeing and employment in the longer term.

To succeed, you’ll need a good understanding of motivational coaching and interview skills, the ability to organise, plan and prioritise and a real commitment to develop. Familiar with office IT systems, you have good word processing skills and know how to use emails and the internet to create simple plans and reports. Just as important is a proactive and flexible approach plus the ability to listen to, and empathise with, people from all backgrounds and provide person centred support in a non-judgemental way. Put simply, you’re committed to reducing health inequalities and able to support people in a way that inspires trust and confidence, thus motivating others to reach their potential.

The role will be working across the Kingston borough on our Active Wellbeing Kingston BME pilot service the option to work across our Richmond Active Wellbeing programme.

• The post-holder will be responsible for attending initial and final assessment meetings either side of the 10 week programme for each client assigned to them as part of the Active Wellbeing programme.
• Ensures that the service is provided to the standards required by the organisation, Government Legislation and governing body rulings.
• Deliver a one hour exercise session each week over a duration of 10 sessions per assigned programme client.
• Actively involved in programme assessments either side of the 10 session programme.
• Deliver an individualised physical activity programme to each client assigned.

Main Duties/Responsibilities:  

• Design and deliver a weekly physical activity session for each client considering safety and progression with each of the 10 sessions.
• Engage in all client assessments prior to and beyond their 10-week programme
• Active engagement and design of goals and individualised exercise plans for each client
• Monitor clients exercise intensity, type, and ability to ensure clients are exercising safely.
• Monitoring clients mental wellbeing during sessions to ensure safety and communicating any concerns to the line manager.
• Responsible for arranging weekly sessions with clients.
• High level of communication between instructor and client to confirm session times and locations.
• High level of communication between instructor and line manager to report any concerns with a clients mental and physical wellbeing during sessions.

• Level 3 qualified personal training certification (essential)
• At least 2 years’ experience in a role that involves delivering face-to-face personal training physical activity support
• Experiencing working in a mental health setting and/or long term health conditions (essential)
• Experience working with vulnerable groups (desirable)
• Mental health first aid, suicide prevention or mental health awareness qualification (desirable) or willingness to undertake training (essential)
• Understanding of the principles of confidentiality and how these apply when handling service-user information.
• Understanding of the challenge’s vulnerable adults diagnosed with an SMI face in relation to independent activity and accessing health programmes
• Good ability to use Microsoft Office and Outlook email

We are recruiting a Data and IT Assistant apprentice to capture and measure the impact of PAPYRUS’ services and provide IT support to our team. 

What you will do: 
• Support the Data Manager to capture, cleanse and present data on PAPYRUS’ activities. 
• Provide first line IT support to colleagues and liaise with our third party IT provider where necessary. 
• Assist with the setup of equipment for new starters and keeping a log of PAPYRUS issued equipment. 
• Manage PAPYRUS CRM systems to maintain data accuracy and ensure good data capture and management practices are followed.

About the Service

We know that poor mental health can make earning and managing money harder, and that worrying about money can make our mental health worse. Could you help and empower people to navigate and improve this?

Mind in Salford are looking to recruit a Money & Me Navigator to proactively identify people with mental health issues whose lives are being impacted due to financial concerns and provide support those who are less likely to come forward for support or reluctant to access mental health care due to stigma or discrimination.

You will provide one to one and/or group support, using trauma informed approaches, to empower people with mental health issues whose lives are being impacted due to financial concerns. By doing this, you will support people to become more resilient and better able to deal with problems they may have in the future.

About You and the Role

Your main duties and responsibilities will include but are not limited to:

• Manage a caseload of clients accessing one to one support, providing goal orientated support, for up to 8 weeks to help people build their skills and resilience to take control
• To set up, develop, and facilitate group sessions for people with mental health and financial problems and deliver a predetermined course
• To work with clients within Money & Me in relapse prevention and to provide waitlist initiative/holding services to clients awaiting higher interventions when required
• To follow a set programme including referring people to higher levels of intervention mental health or financial when required

The charity envisions a world where no young person is defined by their mental illness and where they have access to the right support at the right time.

The organisation invests in services and research that prevent and treat mental illness in young people, with a specific focus on the prevention and early intervention in anxiety and depression for 11–25-year-olds. They are interested in non-clinical approaches and building a stronger evidence base for what is effective. This front-line management role comes with responsibility for the day-to-day management of staff and supporting the Service Lead with the running of the service to ensure we provide recovery focused delivery. Seeing that operational KPIs, outcomes, utilisation and voids, etc., are met will be vital too, as will ensuring data entry to MI & reporting systems is accurate, timely and in line with quality assurance requirements. Preparing information and reports, leading on positive risk management practice within service/location(s), ensuring timely reporting of incidents and accidents and generally acting as an ambassador in representing Waythrough – all are part and parcel of this attractive new role.

To succeed, you’ll need a relevant professional qualification, e.g. QCF/NVQ minimum level 3 (or be working towards/willing to obtain one), substantial experience of working with a housing and social care background and of managing a caseload of individuals with complex needs. About the role:

When a person or their family member sustains a spinal cord injury it is a life changing experience. They can feel very isolated and that no one understands what they’re going through.

Back Up’s mentoring service is there to help. We know that peer support can have a life changing impact and our Mentoring Team are the ones who can facilitate this through matching them with a suitable mentor who will be able to support them over a series of phone calls to reach their goals. The Mentoring Team Leader leads the team through providing excellent line management and driving the delivery of the team’s business plans to empower the team to ultimately support more people affected by spinal cord injury

In addition to leading the team, a Mentoring Team Lead will assist in creating mentoring relationships and support mentors and mentees as appropriate, assess mentee suitability and provide support in the process of identifying, recruiting and training new mentors.

A Mentoring Team Lead will be motivated to achieve the aims, targets and milestones that have been set in terms of making quality matches and will be comfortable and efficient with data management and GDPR compliance.

The Engagement and Activity Practitioner (EAP) will work closely with the children and young people in the care of TACT foster carers across the North West. The Engagement and Activity Practitioner will facilitate opportunities for participation by organising, and, attending, events and trips that are in line with the children’s interests, developmental stages, and abilities, inclusive of children with additional needs. The Engagement and Activity Practitioner will continue the development of the children’s buddying club, for children to meet on a regular basis and share their views, ideas, and interests for TACT as an organisation to meet their needs.

Overall Duties of the Engagement Activity Practitioner will include:

• Being an accessible point of contact for the children living with our carers and to our young people, keeping them up to date with the facilities and opportunities available with TACT
• Supporting interventions and activities to ensure stable placement arrangements
• Being able to transport children and young people to events, appointments and meetings (mileage reimbursed)
• Organising and participating in virtual and face-to-face events and occasional residential meet-ups
• Managing all paperwork associated with events and activities
• Willingness to work flexibly, according to deadlines and needs of our families
• Undertaking trauma-informed direct work with children and young people 

The main role requirements for this role include:

• A formal qualification in working with children and young people
• Experience in a paid or voluntary capacity of participation, advocacy or direct work with young people
• A good working knowledge of relevant legislation, regulation and statutory guidance 
• Good understanding of child development
• Understanding the care experience for Children & Young People
• Can establish constructive and effective working relationships with other colleagues, agencies and partners
• Understands inter and multi-agency working
• Can relate well to young people

The successful candidate will be required to attend regular face-to-face meetings in the Liverpool area and attend other face-to-face meetings such as training and team wellbeing events as well as meeting with children and young people in person.  The EAP is required to be available to transport and attend at least two residentials per year within the UK. 

The candidate is expected to have access to their own vehicle for transportation and have the flexibility to work during busy times, which include, after school, school holidays and occasional weekends.
