About the Peer Support Services

Our Peer Support Service started 10 years ago, with 2 Wellbeing Centres offering this new service. Since then, we have seen our service grow and develop in ways we could not have imagined. This service is the key service most requested by people and we now offer Peer Support across all of our Wellbeing Centres across Hertfordshire.

Working in co-production with clients, staff, volunteers and stakeholders, we were able to create our own Peer Support Charter Values (this can be found on our website). These values form the ethos we work towards within our peer support team. These values also complement HMN’s values of Hope, Courage, Togetherness and Responsive, which form the basis of all the work we do as an organisation. 

About the Role

Working with the team to offer 1-1 peer support to anyone accessing HMN. The Peer Support service provide tailored support for individuals to support their needs. This includes using different tools and techniques to help an individual improve areas of their life they may be struggling with. Having a lived experience of mental health issues or supporting someone who has is a key element to what makes this service so unique.

The Peer Support approach includes empathy, warmth, acceptance, authenticity and compassion. Peer Support is based on the recognition that there is no better person to support the path towards recovery, than someone who has walked a similar path as that person.

To meet client need, and work to this personalised approach, we do support people out in the community, within their home, as well as in one of our Wellbeing Centres. Being able to drive and having access to your own vehicle (or equivalent) is essential for this role.

The successful candidate will need a personal understanding of mental ill health, either from your own lived experience or by caring for someone with a mental health condition and have awareness of issues in mental health service provision.

Peer Support Workers will also provide support within our Peer Support Groups, Peer Support Learning Sets and Peer Support Academy when needed.

• Annual leave entitlement of 25 days per year pro rata, rising in increments to a max. of 28 days after 5 years employment (plus 8 days Bank Holidays)

They are both 24-hour supported housing projects offering high and medium levels of housing-related support for young people aged of 16- 25. We provide support to our residents to help them build essential life skills, identify their goals and work towards aspirations so that they can move on to enjoy fully independent futures. Each Support Worker holds a caseload of residents and meets with them weekly to build a support plan and help them achieve their goals. 

We are looking for Supported Housing Bank Support Workers to join the team - you will bring your own individual and unique qualities to the team for the betterment of the team dynamic. You will contribute to the smooth and safe running of the services, along with supporting young people to develop living skills such as cooking, cleaning, budgeting, managing behaviour, seeking and securing employment and making wise decisions. Support workers are vital members of our team as they provide support to tenants, helping them to develop the life-skills they require to move on to independent living. About the role

We are looking to recruit a female Project Worker for our diverse team at the Chrysalis Project in South London. Reporting to the Deputy Manager you will be responsible for providing person-centred support to clients to manage their tenancies and access independence.

As the post-holder, you will be working in our Women only Palace Road location, to maintain an environment of support for women with complex needs.

In this role you will:

• Support clients to liaise with external services like GP, substance misuse teams, mental health teams, social services.
• Support with all necessary skills to enable an individual to move on to more independent accommodation,
• Work shifts which are normally 8am - 8:30pm. You will also be expected to work some weekends and Bank Holidays.
• Carry a caseload of clients and be their lead worker.

About you

You will be able to liaise effectively with external agencies, have clear communication skills, a non-judgemental approach, and a good working knowledge of homeless clients and the challenges they face.

The successful candidate does not have to have direct experience in a similar environment. We are looking for a part time Information & Advocacy Community Support Worker in Scotland.  You will be home based living in the North of Scotland.

You will ensure that service users (children and adults) and their families, receive high quality support, from the point of diagnosis and throughout their journey, and will assist them to access funding, equipment and care and support that they are entitled to. You will join a team that makes a real difference to the lives of the people living with muscle-wasting conditions, and their families, in Scotland. The role will be primarily home based, but there will also be a requirement to attend outpatient clinics within the region, attend home visits where appropriate and to support responding to queries through the helpline and information inbox. Some travel will be required to attend meetings and events outside of Scotland. Change Mental Health is a leading national mental health charity providing unique support to people with severe and enduring mental ill health. With over 50 years’ experience across Scotland, we believe people affected by poor mental health and illness deserve the highest quality of support in the community and that every person has the right to be valued and to share in the opportunities, challenges, and joys of everyday life. 

The Community Link Worker service collaborates with selected GP surgeries, supporting adults over the age of 18 through social prescribing. Referrals are received from the GP to our Community Link Workers when there might be a concern caused by social factors, rather than a medical problem. The service focuses on improving health outcomes for people experiencing stressors in their lives due to social problems, which could lead to poor mental health.

As a Community Link Worker, you will work directly with people referred to the Community Link Worker Service from their GP Practice who require immediate or on-going emotional, practical or vocational support in order to deliver the best possible health outcomes. 

This post works directly with people using the service to develop and review personal support plans. You will work closely with other members of the Community Link Worker team, colleagues in Primary Care and representatives of community-based groups and organisations, to achieve outcomes as defined by your line manager. 

Within the Community Link Worker team there is a clear line management structure which supports and shapes the practice. The post will contribute to an environment where people can improve their health outcomes, establish improved connections in their communities, and enhance their quality of life.

Key Responsibilities:

·        Work as part of a team building positive working relationships

·        Work closely with colleagues in Primary Care

·        Coordinate and refer service users to resources in the local community and to statutory and non-statutory services as appropriate

·        In partnership with service users, design, implement, and review outcome focussed development plans to enable service users to achieve their identified goals

·        Employ a number of appropriate intervention strategies including positive communication skills

·        Ensure ongoing risk assessment is integral to my practice to ensure the wellbeing of service users

·        Utilise group work skills which bring people together in a safe environment 

·        Awareness of appropriate professional boundaries including confidentiality 

·        Understanding of the requirements of Health and Safety in the workplace and follow Change Mental Health policies 

·        Ensure that careful and ongoing review systems are in place and maintained

The St Mungo’s Pan London Rough Sleeping Teams (No Second Night Out and Somewhere Safe to Stay) provide a rapid response service, intervening to ensure no one needs to sleep rough. We are currently looking for people to join our teams as Night Assessment Workers in our No Second Night Out Assessment Hubs. In these roles you will: 

• Work as part of a busy team environment working closely with those affected by rough sleeping. 

• Provide a safe and supportive environment at night to clients; carrying out assessment when clients first arrive out the service.  

• Work on a 7 day weekly night rota pattern with an average of 3 waking night shifts in a row (for example 8pm–8.30am or 9pm–8am) 

About you 

These crucial roles will suit people with an interest in in developing key skills to build a career working in a support role, as well as:

• Some experience of dealing directly with the public and/or customers in a busy service environment. 

• Good communication skills and the ability to work with others effectively to get things done.

In this role, you will be part of a dynamic team providing tailored support to clients with multiple and complex needs, including mental health challenges, substance use, offending behaviour, and physical health issues. Using a Biopsychosocial model and working within a Psychologically Informed Environment (PIE), you will deliver high-quality, holistic support to help clients achieve stability and move forward in their lives.

Role responsibilities will include;

• Providing personalised support to clients with multiple disadvantage, including poly-substance misuse, mental health concerns, and histories of trauma.
• Conducting needs and risk assessments, and work collaboratively to develop and implement support plans.
• Promoting harm minimisation and recovery, enabling clients to set and work towards their own goals.
• Supporting clients to develop life skills, manage relationships, build social networks, and access external services and community resources.
• Facilitating group activities and structured interventions as part of our in-house recovery programme.
• Working in a multi-disciplinary setting, coordinating with other professionals to create integrated care plans.

About you:

• Experience of working with multiple disadvantage individuals, including those with substance use, mental health issues, or histories of offending and trauma.
• Proven ability to provide personalised assessments, support planning, and casework.
• Strong understanding of the challenges that disrupt clients' journeys towards independence, such as substance misuse, mental health issues, and domestic violence.

Full-time (37.5 hours per week) / Permanent / Working on four nights on, four nights off rota including working over weekends and bank holidays. Although these are long shifts, you will get four days off after, which can provide a great work life balance.

Our mission is to help children and young people have a fair chance to be who they want to be. We do this by providing a safe home, increasing life skills and self-confidence, and improving emotional wellbeing and mental health.

We are looking for a Housing Mobile Night Worker to provide floating support to our teams in high support 24-hour projects as well as medium support and transitional housing units to ensure that our properties are safe, secure, and welcoming through the night. You will be based at the Horsham Y Centre and will support the static night worker there during the night as well as undertaking your mobile duties.

Working on site you will be responsible for ensuring the service is safe by keeping clear records of night time activity within the building. You will regularly complete building checks and monitor CCTV upkeeping health and safety regulations in accordance with YMCA DLG's policies and procedures.  You will work to prevent any anti-social behaviour from impacting our neighbours and other residents. You will work alongside other support staff to maximise the wellbeing of residents by completing one to one interventions with young people who may be experiencing mental health crisis and/or who may need medical assistance.

This is a mobile role, requiring all applicants to have a full valid, UK Driving Licence. You will also be lone working as part of the role, so you will need to be comfortable with decision making and following procedures.

This is an exciting home based opportunity working with stroke survivors and their families to support them following a stroke.

Position: S11212 Stroke Association Support Coordinator (two posts)

Location: Home-based, Bury. However, frequent travel will be required as part of this role (may include team meetings or other work-related meetings).
