Key Tasks & Responsibilities

1 To hold a caseload of families who need early intervention support, providing those families with more intensive support including regular telephone calls, home visits, signposting and attending meetings and appointments with them

2 Provide information, advice and support to parents/carers of disabled children individually and in group sessions

3 Signpost parents/carers to appropriate services for their whole family provided by the statutory sector, voluntary sector and Umbrella

4 Supporting parents/carers to access and engage with services as appropriate

5 To work closely with other members of the family support team, ensuring a smooth handover of families as they finish the intensive support phase of their journey with Umbrella

6 Develop and maintain strong and effective working relationships with staff working with disabled children and their families employed by other local agencies

7 Develop a high level of knowledge of local services

8 Facilitate support groups across the area for our early intervention service, taking account of geographical and community issues

9 Supporting and directing volunteers to provide peer support and help with group activities

10 Collating and reporting on relevant statistical and other performance information

11 Working with parents/carers to continually improve the service. 

General Responsibilities

1 Manage and organise your own time and activities to ensure tasks are prioritised, and agreed deadlines are met

2 Assist with the marketing and promotion of the service

3 Work closely with Parent /Carer Forums and Information, Advice Support Services.

4 Take an active part in continuing to develop own knowledge and skills relevant to the job, attending training as required

7 Support parents/carers and young people to express their views

8 Work with parents to identify and develop new ways of providing effective support to parents, including the use of ICT and social media

9 To contribute to Umbrella service reports and bulletins.

About the Role

As the Durham Service Manager you will oversee the successful operation of supported accommodation providing an assessment, support and move on service to young people at risk of homelessness – creating a safe and engaging environment that individuals feel proud to call home. 

You will work across the Bishop Auckland Supported Accommodation service and the Dispersed Accommodation service based in Tow Law managing a team of Progression Coaches, Night Progression Coaches and Outreach Workers. You will support and guide the team in providing targeted support for clients and take responsibility for the safety and wellbeing of all young people, volunteers, staff and the building in the service. Additionally, you will ensure the service meets all the relevant regulatory and quality standards, including compliance with Ofsted requirements to maintain the highest level of care and support.

Key responsibilities include:

• Build and maintain local relationships with commissioners, stakeholders, and delivery partners, ensuring effective collaboration.

• Oversee building safety through regular risk assessments, health and safety checks, and compliance with Depaul policies.

• Liaise with internal departments (Finance, HR, Fundraising, etc.) to ensure smooth service operations.

• Manage and monitor budgets in coordination with the finance team and Assistant Director.

• Deliver high standards of support for young people and ensure effective housing management practices, meeting KPI targets.

• Apply the Depaul Acceptance and Commitment Therapy model to help clients align with their values and long-term goals.

This is an excitign opportunity to lead our Youth Work programme and Day Centre provision at NHYC. This position is a six-month fixed-term contract to cover a staff member going on sabbatical. The contract is planned to run from February-August 2025. In this role, you will manage a caseload of clients, many of whom have experienced homelessness, addiction, or involvement in the criminal justice system. You will provide tailored employability advice, one-to-one support, and deliver workshops aimed at helping clients develop the skills and confidence they need to rebuild their lives.

You’ll support with our LEAP project, providing a crucial stepping stone for clients in the early stages of recovery. By collaborating with partner agencies and working alongside our Peer Navigators, you’ll ensure clients receive the comprehensive support they need.

Duties and Responsibilities

• Complete initial assessments, risk assessments and risk management plans ensuring a high standard of case management.
• Work with a caseload of people who have experience of homelessness, addiction, and/or the criminal justice system that want to improve their chances at moving into training, education and employment.
• Provide employability advice and a range of interventions with the aim of supporting them to improve their employability, access training, volunteering, and move towards employment.
• Lead on and develop the Good Shepherd structured volunteering programme for people in early stages of their recovery journey.
• Work with Peer Navigators from our LEAP team to provide additional support.
• Signpost and make referrals to external agencies and build positive working relationships with partner agencies across the city.

• Collect, collate, and interpret data that is required to report on the outcomes achieved through this work.

This role is an opportunity to work for Mind BLMK as part of the peer support service, you will have the opportunity to use your own personal experience of mental health challenges to coordinate the running of a Peer Support Service in Milton Keynes, provide information and guidance, and implement referral pathways to the wider Recovery Service.

Key Activities

• Maintain an effective workforce for the service in line with Mind BLMK’s HR policies, procedures and guidance (recruitment, line management, workload planning, support and development of Peer Support Workers and volunteers).
• Establish and maintain effective working relationships with the Mind BLMK team and relevant key post holders.
• Provide the infrastructure Peer Support Workers need to deliver the support safely and confidently.
• Hold responsibility for all Cash Handling, Health and Safety and Data Collection responsibilities as directed by the Team Leader in line with Mind BLMK’s policies, procedures, requirements and guidance.

You will be a Mental Health and Wellbeing Lead who is passionate about supporting MAC-UK’s objective of working collaboratively with young people, innovating services and systems and their approach to mental health, to address health and social inequalities. About the role:

The Diversion programme began as a two-year pilot programme launched in July 2019 and funded by MOPAC (Mayor’s Office for Policing & Crime). The aim was to drive multi-agency working between the police, Voluntary Community Sector (VCS) service providers and other statutory partners and was part of a wider programme of work to develop a whole system approach to identifying and addressing the needs of women involved in the criminal justice system in London. The Diversion programme integrates a short-term, holistic package of support for women with an out of court disposal, to improve outcomes for the woman and offer Police officers a robust alternative pathway other than prosecution.

We have recently been awarded funding from Portsmouth City Council to deliver a 12-month pilot of the Diversion Programme in Portsmouth, to support those women of the PO1-PO6 area postcodes, who encounter the Police and as a result are then to be referred to the Diversion service rather than be prosecuted.

The successful post holder is required to pass Police Vetting

About You:

To be successful as Diversion Keyworker you will have an in-depth knowledge of issues facing women offenders and those at risk of offending; both in and out of the criminal justice system and the ability to see how violence against women and girls interlinks to women in the criminal justice system.

In short, you will have: -

You will bring proven practice of working with women experiencing issues related to offending (e.g. homelessness, substance misuse, domestic abuse, mental health or other relevant work area) preferably within criminal justice setting. You will be able to demonstrate your experience of conducting needs and risk assessments and of risk management crisis management skills and an ability to cope in stressful situations. You will show forth your ability to manage a caseload and a top-quality skill set with prioritising and organising your own workload.

You will be a self-serving uniquely able to undertake working independently and at times be managed remotely. That said, you will have a proven record of collaborative and working partnership with relevant statutory and voluntary groups, demonstrating the ability to develop and maintain strong constructive working relationships.

Right now, one of our partners, Aquarius is looking for a Drug and Alcohol Practitioner to join their Bedfordshire Young People’s Team. Aquarius helps young people aged 5-18 overcome the harm caused by drugs and alcohol by working closely with the individual, family members and other professionals . But it’s only possible with the help of people like you.

Your challenge? – To provide information and advice, brief interventions and long term structured interventions to support young people who are using drugs and alcohol or being impacted by someone else’s use.  You will be conducting assessments, risk assessments, care-planning and working directly with other services such as schools, social care, mental health services and youth offending.The successful candidate must have exceptional organisational skills and the ability to work with other agencies both face to face, email and via telephone.

An energetic and confident self-starter, you will be required to hold a qualification in health/social care, youth and community work (e.g. NVQ Level 3 or above, DipSW, Mental Health Nursing, Counselling, Addiction Studies). Alternatively, we’ll consider candidates with experience of working in the substance misuse field . As well as good knowledge of alcohol/drug and health related issues, you will be used to liaising with voluntary and statutory agencies and health professionals and comfortable engaging with clients in a variety of service delivery settings. You will have a flexible approach, a commitment to the principles of confidentiality, excellent record keeping and  a working knowledge of Microsoft Office.

As this role requires travelling around Bedford, you will also need to have a clean driving licence and access to a vehicle.

This is a permanent full-time role requiring the post holder to work 37 hours per week.

Outreach Practitioner

An Outreach Practitioner is required to work on an initial 3-month contract for a drug and alcohol charity based in North London.

The role of an Outreach Practitioner is to provide high-quality street-based drug and alcohol interventions to people who are homeless or at danger of becoming so in order to promote positive outcomes.

They will oversee, evidence-based interventions, recovery planning, and aftercare planning while managing a small, complex caseload.

Responsibilities:

• Engage and reengage homeless individuals misusing substances by conducting street outreach, building relationships along the way.
• As part of a well-integrated care package, provide evidence-based psychosocial one-on-one therapies and adopt a proactive stance when dealing with service user disengagement.
• This includes naloxone programs, recovery interventions, opiate replacement therapy, blood-borne virus intervention, assertive outreach, needle and syringe program delivery, and harm reduction advice, guidance, and education.
• Participate in internal and external multidisciplinary review meetings and collaborate with housing teams, local authority officers, and other treatment agencies to encourage individuals in seeking treatment in order to reduce the occurrence of antisocial behaviour and street drinking.

Experience Required:

• Proven ability to provide individuals with complex needs with high-quality services. This includes having the capacity to work one-on-one or lead group sessions, complete assessments, care and support plans, risk assessments, and case recording in a timely manner and to a high degree.
• Enthusiasm for working with disadvantaged or socially excluded people, with a track record of meeting deadlines, properly and promptly recording information, writing reports, and remaining composed under pressure.
• Strong IT abilities, such as the ability to use digital meeting platforms, Microsoft Office applications, and case management systems like Theseus, System One, or Nebula.
