This is an exciting opportunity to join our new service supporting Carers across City of London to improve their health and wellbeing, and access support that meets their needs.
The team work holistically with Carers to understand their individual circumstances and level of need. You’ll carry out home visits and community-based meetings, co-produce support plans, and develop effective relationships with Carers, families, and professionals. You will also support Carers to complete written forms and applications, draft letters, and advocate with third parties.
You’ll build on your local knowledge to signpost and facilitate access for Carers to support services, including financial and benefit advice, housing, health and safety home systems, and leisure and social activities. You will also support the development of escalation plans so that Carers are prepared for unforeseen circumstances that would impact negatively on ability to care.
Together with colleagues, you will plan and deliver the service’s activity package, including coffee mornings, drop-in sessions, activities, online and face to face groups, and peer support forums. There will also be opportunities to promote the service by attending meetings and forums, and to deliver training sessions to professionals and community organisations so that they have the skills to identify Carers.
Applicants should have relevant personal or professional experience of working with Carers and/or vulnerable adults or children in a social care, health, education or voluntary and community sector. You’ll need a practical understanding of barriers faced by Carers, and an awareness of cultural differences and access to services.

About the role
This role offers an exciting opportunity for you to play a key role in supporting the Executive Team at St Mungo’s by providing a range of services to ensure that their work runs smoothly and efficiently. Aquarius is a charity that helps people and communities overcome the harm caused by alcohol, drugs and gambling. Right now, we’re looking for someone like you to join our NGAGE Young People’s Service in Northampton. NGAGE is an early intervention drug and alcohol service for young people aged 10-18 and we provide information, education, advice and support to young people in relation to drug and alcohol use.
Your challenge? To provide advice, assessments, brief and structured interventions and group programmes. As a Practitioner, you will be the initial contact for referrals into the service, conducting assessments via telephone, face to face and offering relevant and appropriate advice, harm minimisation, family interventions risk assessment and management, actioning onward referrals to partner agencies. You will also be required to manage a caseload.
An energetic and confident self-starter, you have a qualification in health/social care, youth and community work (e.g. NVQ Level 3 or above, DipSW, Mental Health Nursing, Counselling, Addiction Studies). Alternatively, we’ll consider candidates with experience of working in the substance misuse field with a commitment to complete NVQ Level 3 Health and Social Care. As well as good knowledge of alcohol/drug and health related issues, you're used to liaising with voluntary and statutory agencies and health professionals and comfortable engaging with clients in a variety of service delivery settings. You will also have the ability to collaborate and work professionally with service users, families and external agencies to provide a service which maximises outcomes for service users. As our Community Engagement Executive at Nightingale Hammerson, you'll be at the heart of our efforts to connect with and support the Jewish community. Your role will involve spearheading community initiatives, organising impactful events, and managing key administrative tasks.
What will your typical day involve?
You will collaborate with Jewish communities and synagogues, orchestrating and overseeing community events, and managing related clerical tasks. You will also be responsible for liaising with external agencies and assisting in the production of departmental reports.
Your day-to-day responsibilities will include contributing to educational and research projects, delivering exceptional customer service, and enhancing departmental systems and processes. Upholding Nightingale Hammerson’s values, maintaining professionalism, and ensuring confidentiality are crucial aspects of your role.
In addition, you will manage telephone and email inquiries, foster positive relationships with colleagues and visitors, and occasionally work evenings and weekends to support events.
Who are we looking for?
We are seeking someone with A-Levels or equivalent qualifications and a genuine interest in improving older people’s lives. You should have excellent, caring communication skills and a good understanding of Jewish culture. Strong literacy and numeracy skills are required for creating written materials and handling data. Proficiency in office procedures and IT skills (Word, Excel, PowerPoint, and video calls) is essential. Experience with database management is a plus, and you should be able to work flexibly and independently.
Experience in healthcare or education settings is also desired. Prior experience in a similar role, along with project management skills, is advantageous, especially in supporting service developments from initial stages to full implementation.

You will work with young people between the age of 17-25 with needs related to substance misuse, including those that are affected by their parent’s substance misuse. The purpose of the role is to engage young people in a variety of interventions, using a young person centred approach, empowering them to reduce risk, increase resilience and achieve positive outcomes.
Based in the Transitions team you will support young people with multiple risk factors and substance use. Our Complex Needs Practitioners work flexibly across the community and within our multi-agency partnership settings. We have a vacancy for a Flourish (Refugee and Asylum Seekers) Community Outreach Worker to join our team, supporting refugees and asylum seekers in the community with multiple needs who have mental health as a primary need, through providing advice, information and holistic support.
About the Service
‘Flourish’ is a highly flexible and responsive outreach service developed to meet the needs of refugees and asylum seekers in Hertfordshire. The aim of the service will be to support refugees and asylum seekers in the community with multiple needs who have mental health support as a primary need.
Purpose of Post
- To provide advice, information, onward referral and holistic support to clients with mental ill health.
- Support to build social networks and reduce isolation whilst settling into the Hertfordshire community this includes building a sustainable and empowering peer support network and building confidence and self-esteem.
- To ensure that the safety and wellbeing of the client is monitored and reviewed regularly.
- To remain a source of independent support for all clients.
- Embed a person centred and solution focused approach in all aspects of the role.
- To work in a trauma informed way including awareness of the 6 principles of trauma informed practice, putting the clients emotional and practical needs at the forefront of this work.
Key Responsibilities:
- Working closely with all key delivery partners across the system to ensure the service is responsive, inclusive and easy to access.
- To take allocated referrals from any agencies including the Community Navigators, Borough and District Councils, Herts Help, Housing Providers, Herts Welcomes Refugees, the Refugee Council and self-referrals including walk in contacts at any of the Mind wellbeing centres across the county.
- Hold a caseload of clients, supporting for approx. 10 weeks
- To create a safe space for the individual to be able to explore their feelings and improve their understanding around their mental health.
- To complete a holistic needs assessment.
- Ensure all person centred risk assessments and support plans are completed.
- Proactively recognise the indicators of deteriorating mental health and facilitate appropriate action, whilst liaising with relevant agencies.
- Support clients to build their emotional resilience, promoting self-management through building an understanding of their own triggers/coping strategies.
- To provide safeguarding intervention support. This will include responding to problems, which pose an immediate risk.
- To work alongside translators when required to provide a robust package of support for clients.
- To accompany clients to initial visits and appointments as necessary in order to ensure the sustained use of existing support services in the Borough.
- To provide advocacy services to clients experiencing mental ill health.
- To assist clients to access appropriate mental health services, housing/tenancy services and other services according to assessed needs.
- To establish effective pathways across a variety of agencies to maximise and provide effective partnership working, working with other third sector and VCSE organisations to provide signposting and continuity of care.

About Us
The Upper Room (TUR) works with a wide range of financially and socially disadvantaged groups including homeless people, people with long-term mental health conditions, asylum-seekers, refugees and ex-offenders – helping them to improve their lives and become productive members of society. Central to our offering is our UR4Meals service, providing free, home-cooked food to people who cannot afford to provide for themselves. We are also very proud of our UR4Driving programme, which supports people recently out of prison with free driving lessons in exchange for community volunteering. Gaining a driving licence greatly enhances their future employability prospects and helps break the cycle of re-offending. Additionally, our UR4Jobs project focusses on both client wellbeing and breaking down the many barriers to employment faced by disadvantaged people every day.
The role
Is leading the charity through its next phases of development as it prepares itself for year on year increases in demand with a focus on delivering the charity’s future resilience plan (Project 12/25). The CEO is accountable for overall charity performance (financial, operational, governance, compliance), team (paid and volunteer) support and development. Additionally, there are a wide range of community and external partner relationships to manage and develop. At FearFree we run various trauma-informed groups for victims such as Empowering You (an educational domestic abuse online support group) and Rediscovering You, a face-to-face emotional wellbeing recovery group.
We are looking for experienced and motivated staff to join to help deliver the group work programmes we run around locations in Wiltshire. Groups run in the morning, afternoon and evening. Training for the groups will be provided.
In addition to facilitating groups, staff will be asked to attend training events and meetings, including regular supervision.
Main Duties and Responsibilities
- To facilitate group work provisions across the Wiltshire Service on weekdays, including afternoons and evenings.
- Completing pre and post group assessments and required paperwork, following the procedures in place.
- Write comprehensive case notes following groups about service user involvement, any concerns raised and any action required.
- Identify and take the appropriate action on any disclosures or safeguarding concerns raised within the group.
- To work cohesively with a second facilitator both virtually and in person in locations around Wiltshire.
- A willingness and ability to travel around Wiltshire and access to a vehicle with business insurance.
Skills and Qualities
- Experience of working with victims of domestic abuse or sexual violence or the willingness to increase knowledge and skills in this area.
- Experience of working with vulnerable people.
- Excellent communication and interpersonal skills.
- Experience running groups, including listening skills, a good sense of timing, sensitivity to overall group dynamics, maintaining group safety and practical elements such as ensuring course material is delivered.
- A willingness to participate in in-house training, including group observation.
General
- Attend supervision as required.
- Attend and contribute to team meetings.
- Ensure service user records are kept up to date.
- Contribute to effective team working with a flexible and pro-active approach, including cover for other team members’ holidays and sickness.
- Undertake agreed training and keep updated on changes in legislation, policy and best practice.

Young Care leavers are some of the most vulnerable young people in our society; many leave school with few qualifications and a significant number are unemployed, living in inadequate housing, and experience poor health and levels of poverty. Nationally 25% of female care leavers become parents between 16 -18 years of age and 50% by the age of 24.
Adjusting to parenthood must happen simultaneously whilst they are still in the process of transitioning out of care or adapting to independent living. Coping with the stressful demands of learning to take care of themselves and a baby at such a young age, whilst often still struggling to make sense of unstable, often chaotic childhoods, can leave little space for engaging with services in the wider community. Instead, many care leavers struggle with poor self-esteem. Isolated and lonely, they may lack the confidence, social skills and social network to join new parenting groups in their community.
Additionally, time spent in the care system can make care leavers reluctant to ask for and accept support. Becoming a parent can be the most rewarding experience in the world, but all parents need support networks to thrive as a family.
