We work with those affected by homelessness and those vulnerably housed in Tower Hamlets, offering an integrated service of housing advice & support, access to substance use, physical & mental health services, as well as, training & progression programmes. We ensure that people often excluded from mainstream services gain the support and opportunities they need to create a safe, healthy and sustainable life.
About the role
- Navigating the London Borough of Tower Hamlets, working with people who have a higher complexity in substance use and need additional support from multiple services.
- To work in a trauma informed and person centred way, empowering the client to make informed choices around their care and support
- Conducting street outreach with partner agencies to identify and locate people on your caseloads, who are sleeping rough.
- Attending local hostels with the aim of supporting those who are at risk of eviction and where substance and/or alcohol use maybe a contributing factor.
- Managing a caseload of clients
- Attending partner agency meetings relating to rough sleeping & hotspots in the borough.
- Providing targeted harm reduction advice and information to individuals around the risks associated with drug or alcohol use and supporting people to improve their health and wellbeing.
- Signposting individuals to other support services inclusive of substance use, based on individual need.
- Removing the stigma of homelessness and for those who use substances and/or alcohol
- Breaking the barriers of which people experiencing homelessness face, when trying to access needed services

Job Summary
This role is an expansion of our employment pathway and will be our first dedicated employment coach for our students.
PiP has developed a unique reputation for our experience and quality of employment support for adults with learning disabilities. This has led to an increased number of referrals with a focus on employment, an expansion of our in-house social enterprise capacity, and more employers keen to work alongside us to diversify their staff base.
PiP is keen to respond to and meet this demand. This role for us is the next step in doing so, allowing us to increase our job coaching capacity and providing more dedicated resource for students on their employment journey. As to date our coaching has been delivered across our existing staff team.

What We Offer:
· The salary for this role is circa £29,500 dependent on experience with incremental pay progression.
You'll get 25 days holiday + bank holidays ever year. We're closed for Christmas but the rest of the year you may take leave whenever you wish.

New Horizon Youth Centre has been commissioned by the Mayor’s Office for Policing and Crime (MOPAC) as part of an Alliance with Safer London, St Giles Trust and the Anna Freud Centre to deliver on their Children and Young People’s Violence and Exploitation and Reduction support service. The commission will run from June 2024 for three years. As part of this, an additional service will be set up within the NHYC Youth Justice team specifically to deliver on the Alliance contract. This will include a team of Housing Caseworkers who will carry out assessments and advocacy work with young people under 25 who are fleeing violence and exploitation, in order to move them on successfully into both emergency and long-term housing options. This is a new role offering an opportunity for someone who has an aspiration to support positive change for people and green spaces, through providing a bespoke service that touches on all of the NHS ‘Five steps to Wellbeing’ for the volunteers in its’ delivery.
Working with Roots means being part of a team that provides a gardening service that embodies Help & Care’s vision and values and the successful candidate would be expected to take an active role in that delivery, while also taking a lead on the ‘wellbeing and coaching’ aspects for the volunteers through supporting them, utilising appropriate wellbeing tools such as ‘Wellbeing Star’ or the ‘My Wellbeing Action Plan’ for example, to record baseline data, attendance, burgeoning skill sets and impact assessments.
For some of our longer-term volunteers this could be as simple as helping them to maintain their equilibrium and stay out of crisis or for those coming to us with vocational aspirations, then to provide a record of attendance, training and skills.
RESPONSIBILITIES:
· Provide one-to-one and group health coaching support for people, based on what is important to them.
· Supporting volunteers and maintain records.
· Support local health, social care, and voluntary sector professionals to make appropriate referrals of the service.
· Participate in regular support supervision and continual learning.
· Assist with practical day-to-day running of the service.
· To provide professional gardening service to our customers whilst working with our volunteers.

The Resident Support Workers/Asset Coaches make up the Resident Support Team (RST). The purpose of the RST is to support people facing the most challenging of circumstances to make positive and lasting changes in their lives. The individuals will be in either temporary accommodation or supported housing (2-year tenancies) which KCAH oversees.
About the role
The Asset Coach will be someone who is passionate about driving change and has a keenness to apply a strength-based approach amongst our service. The Asset Coach will be willing to unlearn the traditional ways of offering support in order to build a positive one-to-one professional relationship that helps individuals to identify and develop their strengths, passions, interests and links to their wider community. The Asset Coach will support, empower and encourage resilience and resourcefulness from within each individual during to achieve strengthened confidence, identity, self-worth, sense of belonging and overall positive outcomes, which in turn can allow for a successful move on to permanent accommodation.
Main Responsibilities
Build an honest and real-world relationship based on mutual respect and observe the dignity of the person at all times. Undertake regular face to face coaching sessions which are casual, led by the individual and result in the creation of a support plan based on their support needs, strengths and interests in the wider community.
Participate in the referrals and interview process for potential tenants.
Provide one-to-one support for a caseload of individuals often facing multiple and complex challenges which includes the recording of relevant data on in-house databases such as Lamplight and other tools.
Participate in case reflections/management, reflective practice and team meetings. Attend training identified in supervision that will enable you to deliver the best service possible.
Identify opportunities and activities for individuals based on their interests, strengths, passions using the support plan including facilitating access and possibly accompanying individuals to these. Coaching sessions, opportunities and activities ideally take place in the wider community and not the ‘homelessness sector’ in order to promote independence, lessen the power imbalance between people and professionals and to normalise experiences.
Encourage the individual to access positive peer support networks and to develop healthy relationships, also ideally outside of the ‘homelessness sector’ in order to promote independent living once such services are no longer accessible to them.
Work with a Person-led and Strength-Based approach; opportunities and activities are identified based on what the individual wants with a focus on strengths and assets as well as development of personal identity, not prescribing solutions based on ‘problems’ or ‘issues`.
Liaise and learn from other Strength-Based projects, partner agencies and external organisations where appropriate.
Be involved in the delivery, organisation and risk assessment of monthly group activities identified by residents.
Maintain and promote the values and principles of the RST both in work with individuals, across KCAH and when building relationships with new organisations.
Maintain in depth and up-to-date knowledge of all KCAH policies and procedures.
Present a professional image of KCAH at all times.
Comply with all legal and health and safety requirements including safeguarding procedures.
Ensure that all safeguarding incidents and/or concerns are dealt with sensitively and professionally in line with KCAH policy and procedures, including the recording of all relevant information.

The Crisis Centre is one of the 5 crisis services under our Nightlight brand we provide to residents in Hertfordshire:
- Crisis Centre – 24/7, 365 days a year
- Crisis Helpline – 365 days a year, 7pm to 1am
- Crisis Cafes providing drop in support – 365 days a year, 7pm to 1am
- Daylight – short term, outreach support
- A&E Crisis Support in Watford and Lister Hospitals (in partnership with Mid Herts Mind)
This role is leading a team of coordinators and support workers to provide excellent support within a small 4 bed crisis centre in Hemel Hempstead, which is a short stay provision, usually 3 or 4 nights, to provide;
- emotional support
- support to develop coping strategies, support plans and safety plans
- ensuring robust discharge plans in place for on-going support
Key Accountabilities
- On occasion, provide emotional support, advice, signposting, assessing risk, support planning for clients in mental health crisis often where risks are significant.
- Overseeing and ensuring the delivery of appropriate needs and risk assessments and support planning for clients.
- Liaise with statutory agencies to ensure appropriate intervention is obtained.
- Organising a rota to ensure appropriate service cover is in place at all times across the Nightlight Crisis Centre provision.
- Contribute to an on-call service, on rotational basis, usually every 8 weeks, taking calls outside of 9am to 5pm Monday to Friday providing staff support.
- Representing Nightlight and wider HMN at external meetings, and network locally to develop contacts, services and raise Herts Mind Network’s profile.
- Manage budgets, expenses, payroll as well as facilities needed for the centre
- Supporting the Crisis manager with developing and ensuring ongoing positive relationship with external NHS, local authority and emergency services including Crisis Resolution Home Treatment Teams, Single Point of Access, Safeguarding Out of Hours Service, Mental Health Liaison Teams, Adult Community Mental Health Services etc.
- Provide management support to Crisis Centre Coordinators and support the Crisis Manager in ensuring practice within the service is safe, effective and person centred, as well as supporting the implementation of strategic service plans.

ABOUT THE ROLE
We are seeking a dynamic and self-motivated Enhanced Hostel Intensive Needs Support Coach in Medway to join us on a fixed term contract until 31st March 25 as part of the Rough Sleeper Initiative Project - RSI (this is likely to be extended but is funding dependent) to provide effective practical and personal support and guidance to Pathways’ clients during the period of their stay in our accommodation to enable them to become more resilient and to live independent lives.
Pathways to Independence is committed to becoming a psychologically informed environment; one which considers and responds to, the potential impact of trauma in every area of its service delivery and design
The primary responsibility of the RSI Enhanced Hostel Intensive Support Coach will be to provide personalised support and guidance to individuals referred to Pathways to Independence through the Medway Rough Sleepers Initiative. This will include assessing client needs and providing support to meet these needs, completing safety plans, ensuring the client can identify and access relevant services, liaising with other statutory and voluntary agencies to ensure the appropriate support for clients.
You will be allocated a caseload of between 5 and 7 clients who have a history of non-engagement, rough sleeping and complex needs. You will be expected to be proactive, in engaging clients demonstrating an approachable, flexible and caring style. The Intensive Needs Support Coach will be required to participate in activities to encourage the development of skills and the building of positive relationships within the local community.
Shift Pattern: 37.5 hours per week, Monday to Friday 9am - 5pm. You may need to work flexibly outside these hours as per the requirements for the service, residents and participants. You will be based across multiple projects in the Medway area.

What are we looking for?
- Experience of working directly with people who have experienced homelessness, with issues around substance/alcohol use and/or mental health
- Experience and ability to build rapport and develop a good working relationship with clients who are using substances, some of whom may display challenging behaviour
- Experience of communicating, liaising and negotiating effectively with a wide range of people including clients, other staff, and representatives of other agencies such as local authorities and statutory services and the general public to build strong partnerships
- The ability to work in fast paced and pressured environment whilst managing multiple priorities and demands to complete delegated tasks or meet deadlines
- The knowledge, skills and ability to assess the needs of people with substance misuse/mental health and health problems and to formulate support plans and risk assessments
- A clear knowledge and understanding of harm minimisation and how it can be used as an intervention; a working knowledge of other interventions in the substance use field
- Proven ability to demonstrate compassionate and creative work practices with residents to address support needs and/or risks

OVERVIEW OF KEY RESPONSIBILITIES
- Supporting a small caseload of individuals with complex needs referred to Pathways through the Medway Rough Sleeper Initiative project.
- Work proactively, innovatively with clients who are hard to engage
- Working as part of a team responsible for creating a safe environment in which clients who have experienced multiple disadvantage are able to build their confidence and discover opportunities for positive change
- Advocating on behalf of clients who frequently experience multiple exclusion from main stream services ensuring that clients can access relevant services
- Liaising with other statutory and voluntary agencies to ensure the appropriate support for hard to reach clients and/or those experiencing multiple disadvantage
- Utilise knowledge, skills and experience to recognise difficult or challenging situations, and use appropriate communication skills to influence and bring about positive change to behaviour
- To offer advice and support to staff working with residents with complex needs, helping to create a safe and trauma informed environment
- Cover for other members of the team as necessary.
- Assess clients with multiple needs to identify support needs, identify risks to complete safety/risk plans and identifying personal goals/desires in conjunction with the client

The Role

- Leading and developing a team of complaints handlers, fostering a culture of empowerment where team members are trusted to make decisions that deliver the right outcomes for tenants.
- Overseeing the investigation, response, and resolution of complaints in line with Regulator for Housing and Housing Ombudsman requirements.
- Collaborating with internal and external stakeholders to proactively prevent recurring complaints and reduce avoidable contact.
- Ensuring accurate, reliable and timely reporting of business performance data gathered from complaints and other feedback mechanisms, communicating this effectively across the business.
- Working closely with customers and colleagues to provide internal advocacy and achieve timely resolutions.

You’ll have experience working with regulatory bodies or an Ombudsman in complaints management and will demonstrate resilience and focus when handling high-pressure situations. With excellent communication skills, you’ll articulate concepts clearly, earning buy-in from direct reports and influencing senior leaders across our organisation to support key initiatives. You’ll also bring a strong approach to compliance and performance management, ensuring we continuously improve how we handle complaints and meet regulatory standards.
