The Role
In this newly created role of Programme Lead, you will be responsible for working closely with pregnant mums and Maternity Mates volunteers to provide tailored support to ensure each pregnancy is a positive experience. You will be responsible for the assessments to determine the support needs of pregnant mums, matching each with her volunteer Maternity Mate; whilst mentoring each volunteer through her journey with mum to provide the very best support.
You will develop good working relationships with volunteers and pregnant mums and take responsibility for continuous improvement so that mums receive the very best possible support and are able to navigate and make choices for themselves. You will proactive and enjoy working collaboratively with your Project Manager and other members of the Sister Circle team.

Job description:
General Management:
· Deputise in the Project Director’s absence or as required.
· Support the Project Director (PD) and Project Co-ordinator to manage and organise daily operations with trainees to help support a trainee’s journey at LandWorks.
· Monitor and take the ‘temperature’ of the site as it operates.
· Occasional out-of-hours trainee phone calls.
· Lead morning team meetings and facilitate afternoon de-brief meetings two days a week, and in the PD's absence.
· Act as a core part of the Senior Leadership Team, to facilitate the smooth running of daily tea breaks and communal lunches, to ensure that all trainees, staff and visitors take part and feel included.
Staff Management:
· Direct line management of the LandWorks enterprise team (that includes the wood workshop, market garden and pottery)

· Provide team motivation and promote professional development, including identifying training needs and setting team goals, across the whole LandWorks team.

· Support and assist with the mandatory review of organisational polices in consultation with the Charity Manager and PD.

We are looking for an Apprenticeships Service Administrator to support the apprenticeship provision within Harris Federation Central Head Office.
This is an exciting opportunity to join the apprenticeship team and support by managing administration and providing essential support to apprentices across the Harris Federation. Key responsibilities include the organisation of Apprenticeship events, maintaining individual learner records (ILR), data analysis, and managing the virtual learning platform and MIS. The Role

Job Purpose
To manage and develop the home from hospital settle service in the hospitals that you are responsible for, to support timely and successful patient discharges from hospital and prevent re-admissions. You will work in collaboration with hospital teams and community teams to lead and manage your team to support the smooth transition from hospital to home for service users. The service works with people for time limited support (usually up to 6 weeks) to adjust and settle back in their homes, and to prevent readmission into hospital.
Key Tasks
- Work in partnership with the hospitals and other health and social care professionals to coordinate safe and timely discharges for referred clients. This involves attending meetings with hospital personnel including Discharge Co-ordinators and Ward Managers to discuss discharges and plan who and how individuals will be supported as appropriate.
- Manage a team working across two hospitals and ensuring the service is available between the contracted hours.
- Work collaboratively with other agencies providing support services.
- Conduct initial assessments of people referred to the services as necessary.
- Plan and develop person centred interventions to provide short term support for people after hospital discharge.
- Work with GPs as appropriate in identifying and supporting patients to reduce unnecessary hospital admissions.
- Work with people with a range of needs, dealing with issues ranging from social isolation and keeping people engaged in their community, to preventing unnecessary admission to hospital.
- Support the hospitals with delayed transfer of care (DTOC).
- Coordinate and monitor the caseload of referrals allocated to team members to ensure a fair and manageable distribution of work.
- Rota planning to ensure staff flexibility and availability, enabling planned rosters that best suit both hospital and patient needs.
