The Role
We now seek a Monitoring, Evaluation, Learning and Research Manager to deliver a step change in organisational capacity in the MELR space, develop our approaches to data collection, and coordinate planning and reporting processes. This will require an experienced, self starting practitioner, capable of developing a long term strategic vision as well as getting deeply involved in ensuring its realisation.

Working across Calderdale with partners, we give people the practical support they need to rebuild their lives with dignity, and we offer a wide range of activities that help them feel happier and more connected.
We are a community of 22 staff (15 full-time equivalent), 150 volunteers, and over a thousand Centre Members from 86 countries. Our ‘Centre Members’ are people seeking sanctuary in Calderdale. 33% of our staff and trustees and 50% of our volunteers have lived experience of the asylum system; their insights and experiences shape what we do.

The role - your main responsibilities

Purpose of the role:
We are looking for 2 Mental Health Personal Independence Co-ordinator’s to join our team.
The Mental Health Personal Independence Co-ordinator (MHPIC) will work as part of the MHPIC team and will work alongside clinical and social care professionals in local multi-disciplinary teams assigned to specific GP surgeries. Each MHPIC will be assigned to a specific locality located within the Integrated Care Network Plus health and social care systems in either North, Central or South Croydon. The multi-disciplinary team learning and development during the initial two-year test period will inform the development and growth of the MHPIC team to ensure that it can support community-based mental health and wellbeing across the whole population and in all 6 Integrated Care Network Plus localities.
MHPICs will provide initial contact and assessment conversations with clients using Open Dialogue tools and techniques to enable people to develop their own Personal Recovery Plans and supporting them directly over a period of time to find and access the support and services available to them in their local community.
Each MHPIC will be the direct named key worker for a set number of people who are referred to the service by healthcare professionals, including GPs, Mental Health Practitioners & Health Care Assistants, Social Prescribers and other voluntary or community sector organisations. They will provide one-to-one support in the achievement of an individual’s recovery goals, ensuring that support is person- centred; that needs and wishes are identified by people themselves; that they have a stronger voice in relation to issues that affect their lives; and that they are supported and empowered to improve and/or maintain their mental health and wellbeing and to live their best independent lives.
Support in the community includes home visits. The aim of home visits is to assess clients' strengths and needs and link these to agency and community resources, and to ensure that individual clients’ needs can be met within a risk-assessed policy approach.

The Bridge Renewal Trust in partnership with Haringey GP Federation and North Central London Clinical Commissioning Group are delivering a new Pan-Haringey Diabetes Care Navigation Service.
As a Care Navigator, you will provide support to people whose diabetes is not well managed or who are not engaging with health professionals, in particular, deprived and vulnerable groups within this cohort. You will support people to access diabetes information and/or education in their own language, improve their ability to navigate the health and care system and improve their confidence to self-manage their condition. This is a new and exciting approach to working with people with diabetes in Haringey.

The Movement Collaboration Lead or Senior Movement Collaboration Lead (depending on experience) will work closely with the Programme Director and the Programme Team to lead the development and delivery of an exciting emerging programme of work building collaboration for economic system change between grassroots groups and new economy organisations.
We are looking for people who are: passionate about addressing economic injustice, are excited about the role of collaboration in creating systems change and capable of creating partnerships and strong relationships with diverse stakeholders.
The first twelve months of this programme will take an action inquiry approach to facilitate a mixed group of new economy organisations and grassroots groups through a collaborative process that will develop shared knowledge and practice about how we can effectively and equitably collaborate on economic systems change work through practical experiments, facilitated dialogue and collaborative research.
The role will involve responsibilities for project managing and delivering this work including programme development and delivery, developing partnerships, changemaker recruitment and support, conducting desk research and interviews, managing and running events, supporting project related fundraising, conducting project learning and evaluation and writing a learning report about the project.
Following this first year we plan to scale work on supporting collaboration between grassroots groups and new economy organisations, subject to funding we hope this could become a permanent role.

Project Development
- Working closely with the Programme Director and the Programme Team, lead the translation of existing strategic thinking, planning and relationship building into a project plan.
- Working closely with the Programme Director and the Programme Team, contribute to business planning and fundraising for future development of the action inquiry beyond the first year.
- Working closely with Programme Director lead outreach and relationship building with new economy organisations, funders and other relevant stakeholders.
- Working closely with the Head of Community, lead outreach and relationship building with changemakers and grassroots groups for the action inquiry.
- Working closely with the Programme Director, develop a plan for learning and evaluation of the project.
Project Delivery

We are looking for people who are: passionate about addressing economic injustice, experienced trainers and facilitators and are excited about the prospect of leading work to build infrastructure that will significantly scale our training capacity.
The Head of Training will lead People’s Economy’s overall approach to training providing strong oversight and ownership over: turning economic analysis content and our community economic education framework into engaging and accessible training; individual workshop and learning journey design; our pedagogical approach to learning; and our unique facilitation approach. The role will have a focus on building the infrastructure to consolidate and scale our training through supporting, training and coaching staff, freelancers and collaborators in implementing our training approach. Leading People’s Economy’s approach to training
- With support from the Programme Director, lead the development of People’s Economy’s approach to training and workshops to create an approach and set of standards to be applied across the organisation.
- Stay abreast of developments in training best practice across similar organisations and use this knowledge to further develop People’s Economy’s training approach.
- Develop resources (e.g. types of training exercises) that can be used across People’s Economy’s training work
- Support colleagues and freelancers to implement People’s Economy’s training approach, including through delivering ‘train the trainer’ training and creating learning resources
- Work with the Programme Director to create processes to oversee the quality and delivery of all of People’s Economy’s training

We are looking for someone with a heart for addiction recovery work to join the friendly staff team at Yeldall Manor, a Christian drug and alcohol treatment centre for men, near Reading. We are a healing community with a holistic approach, supporting men with addiction problems to heal, transform and thrive.
The role of Recovery Support Keyworker involves one-to-one and group support with our residents; encouraging and supporting them in all practical aspects of their recovery including objectives and goals, leave and visit planning, behaviour and health, housing and employment matters. You will be a committed Christian, with excellent inter-personal and the ability to implement firm but caring boundaries, plus good administrative abilities, able to complete care planning and risk assessments. About Wicksteed Park
Wicksteed Park, in Kettering, Northamptonshire, is one of the UK's oldest leisure parks, founded in 1921 by Charles Wicksteed. Role Outline
The Head of Fundraising will be instrumental in the instigation, implementation and completion of the Trust’s long term funding strategy. This important role leads on the Trust’s approach to individual giving, legacies, campaigns and community fundraising. The role plays an important part in raising vital unrestricted income and will act as the connection point with our loyal supporters. The role will lead and develop a fundraising team, working closely with the Community and Heritage team and volunteers.

The Resident Support Workers/Asset Coaches make up the Resident Support Team (RST). The purpose of the RST is to support people facing the most challenging of circumstances to make positive and lasting changes in their lives. The individuals will be in either temporary accommodation or supported housing (2-year tenancies) which KCAH oversees.
About the role
The Asset Coach will be someone who is passionate about driving change and has a keenness to apply a strength-based approach amongst our service. The Asset Coach will be willing to unlearn the traditional ways of offering support in order to build a positive one-to-one professional relationship that helps individuals to identify and develop their strengths, passions, interests and links to their wider community. The Asset Coach will support, empower and encourage resilience and resourcefulness from within each individual during to achieve strengthened confidence, identity, self-worth, sense of belonging and overall positive outcomes, which in turn can allow for a successful move on to permanent accommodation.
Main Responsibilities
Build an honest and real-world relationship based on mutual respect and observe the dignity of the person at all times. Undertake regular face to face coaching sessions which are casual, led by the individual and result in the creation of a support plan based on their support needs, strengths and interests in the wider community.
Participate in the referrals and interview process for potential tenants.
Provide one-to-one support for a caseload of individuals often facing multiple and complex challenges which includes the recording of relevant data on in-house databases such as Lamplight and other tools.
Participate in case reflections/management, reflective practice and team meetings. Attend training identified in supervision that will enable you to deliver the best service possible.
Identify opportunities and activities for individuals based on their interests, strengths, passions using the support plan including facilitating access and possibly accompanying individuals to these. Coaching sessions, opportunities and activities ideally take place in the wider community and not the ‘homelessness sector’ in order to promote independence, lessen the power imbalance between people and professionals and to normalise experiences.
Encourage the individual to access positive peer support networks and to develop healthy relationships, also ideally outside of the ‘homelessness sector’ in order to promote independent living once such services are no longer accessible to them.
Work with a Person-led and Strength-Based approach; opportunities and activities are identified based on what the individual wants with a focus on strengths and assets as well as development of personal identity, not prescribing solutions based on ‘problems’ or ‘issues`.
Liaise and learn from other Strength-Based projects, partner agencies and external organisations where appropriate.
Be involved in the delivery, organisation and risk assessment of monthly group activities identified by residents.
Maintain and promote the values and principles of the RST both in work with individuals, across KCAH and when building relationships with new organisations.
Maintain in depth and up-to-date knowledge of all KCAH policies and procedures.
Present a professional image of KCAH at all times.
Comply with all legal and health and safety requirements including safeguarding procedures.
Ensure that all safeguarding incidents and/or concerns are dealt with sensitively and professionally in line with KCAH policy and procedures, including the recording of all relevant information.
Desirable: Have a thorough understanding of a strength-based, person-led approach, and an overall belief that all people have skills, gifts and strengths.

We are looking for experienced practitioners who are qualified to level 3 in childcare or a related field and have a passion for working with families to help them achieve their best outcomes.
Job Purpose: To provide focused 1-1 and group family support for families with children and young people aged 0-19 years old (up to 25 years for young adults with additional needs). This will be delivered either at the Family Centre, out in the community or in service users’ own homes. The post holder will be required to work with families as a lone worker. All services provided will support the delivery of the Family Support Specification, which aims is to enable children and families to achieve and improve outcomes.
Hours of work: Monday to Friday 9am-5pm. Flexible options available.
Location: Horley or Epsom Down Family Centre, with travel around RBBC
Annual leave:Five weeks plus bank holidays (pro-rata for part time workers) Holidays increase after two years’ service to a maximum of six weeks after six years’ service. (pro-rata for part time workers)

Main Responsibilities:
· Manage a case load working in partnership with agencies to provide integrated and planned interventions, giving advice, information, and practical and emotional support to families.
· Undertake direct work with children and their families who are assessed as requiring help and support, both at the Centre, community and in the home environment for emerging need and targeted services, offering timely support and interventions to address identified needs.
· Provide support for the family, using a variety of interventions including motivational interviewing, solution focused approaches and accredited parenting strategies.
· Deliver 1:1 evidence-based parenting programmes of support to achieve best outcomes for families and priority groups and record observations on electronic systems to the agreed standard and within agreed timescales.
· Plan, deliver and evaluate targeted services within the Family Centre and community venues.

· Convene, chair, and minute-take for multi-agency meetings as appropriate.
· Maintain accurate records including use of the Early Help Management System (EHM) of all interventions and communications, including Early Help assessments, plans, chronologies, reviews, and outcomes electronically; share information with other professionals as appropriate (with consent).
· Coordinate and facilitate TAF/TAC meetings and provide precise information.
· Develop community knowledge and links for the benefit of children, young people and their families and the wider family support service.
· Use the various IT systems and programmes to log families progress which includes the Early Help Management system and Outcome Star records.
· Work at various locations throughout Reigate and Banstead or Mole Valley.
· Work alongside other YMCA East Surrey Children and Young People managers and staff to share good practice.

Abbots Bank is a mental health, supported housing and community based service, working to a recovery model. We provide a range of support services for people with mental ill health to access in their local community, including outreach support, peer support and supported housing. We have been operating in the area for a number of years and offer a staffed service to our residents aged 18 and upwards.
We know that recovery can look and feel very different to each and every one of us. But we also know that if we provide the right support, at the right time, we can inspire recovery nationwide and that recovery is possible for everyone. That’s where you come in.
