About the Service: Family hubs are a place-based way of joining up locally in the planning and delivery of family services. They bring services together to improve access, improve the connections between families, professionals, services, and providers, and put relationships at the heart of family support. Family hubs offer support to families from conception to 19 or up to 25 for those with special educational needs and disabilities (SEND), with a great Start for Life offer at their core.

The service will provide a range of quality services for families, locally designed for the 6 town areas of the Borough, divided into 3 areas:

Cluster A - Oldbury, Smethwick

Cluster B - Tipton, Rowley Regis

Cluster C - Wednesbury and West Bromwich

Essential Requirements to evidence in your written application:

• NVQ Level 3 of Equivalent qualification in a relevant field
• Experience of working with and supporting children/young people and their families to achieve positive outcomes.
• Ability/experience of identifying and appropriately acting on safeguarding concerns.
• Ability to demonstrate a good understanding of child development.
• Ability to complete accurate and timely records, data and assessments.

As a Family Hub Worker, you will be integral to the relationship development with families who move through the Pregnancy Pathway into parenthood of their newborn baby up to the age of 3, as part of Sandwell's Start for Life offer.

Some key objectives/activities of the role include but are not limited to:

• Provide support to families as part of the Pregnancy Pathway.
• Provide Early Help support to families with children from birth up to the age of 3, ensuring the inclusion of fathers and co-parents.
• Act as a Key Worker as a point of contact for families.
• Provide support with Perinatal Mental Health, linking with other perinatal mental health agency support networks and receive training to deliver interventions.

Who will I be changing the lives of?

All of our learners are aged between 16-25 years, have some level of additional need and as a result have an Educational, Health and Care Plan (EHCP).  Some of the most common areas of need that our learners experience are Social, Emotional, Mental Health (SEMH), Autism (ASC), ADHD although we work with learners with a much broader range of needs.

Main Responsibilities

• To be responsible for a caseload of learners with EHCP's aged 16-25 years
• To act as the main point of contact for staff and services with SEND related queries
• To arrange and carry out Annual Reviews and PEP’s as well as completion and submission of all relevant paperwork, acting as chair for these meetings when required
• To assist the Change Leader with the transition process for learners starting at Really NEET as well as those learners leaving Really NEET
• Responsible for over-seeing the progression of learners on completion of the course
• Promote Inclusion best practice and contribute to the aims and ethos of the college

This role sits within our Youth Impact Domain, which works to provide immediate one-to-one and group support for children who urgently need it. Your role will be to help us maintain and develop innovative practice to support our work in our services around the country. 

Climb are an Prevention and Early Intervention Service for 10-17 year old's who may vulnerable to criminal exploitation and/or at risk of entering the criminal justice system. We offer 1-1 outreach support and access to positive diversionary activities in their local areas.

This is an exciting role within an established and expanding service across West Mercia. There are a number of varied, practitioner roles that are set to increase. We are currently looking for a Exploitation Practitioner to join our dynamic, ambitious team. 

This role will support young people at risk of criminal exploitation or entering the criminal justice system; at an early intervention level. This will be through 1-1, direct work and support in accessing positive diversionary activities, therefore reducing the risk. It will also require excellent mutli-agency work.

This role is for people with a passion in supporting vulnerable young people on a 1-1 basis, in raising their aspirations, building their confidence, working together with professionals to ensure the most appropriate, robust and holistic support is offered to meet each individual's needs. A key part of this role will be your ability to positively impact the lives of Children and Young people.

You will need to be able to engage with professional colleagues also, have presentation skills, and be able to bring people together around mutual goals.

Although the contracts are home based, there will be a lot of daily travel across Telford or Shropshire area. Although there is no specific office base, you will be part of a geographically based team and will be well supported by the management team and all of your colleagues.

• Referring members on to other services and ensuring that the offers of Sound Minds are joined up.

Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. The Support Worker role will be to manage a caseload of approx 7-10 young people .You will be expected to deliver positive outcomes for all our young people and support them to reach independence, keep safe and maintain their tenancy's. Aside from support workers delivering support there is a strong element of housing management associated wit this role, for example managing rents, raising/monitoring repairs and health and safety.

• Develop and implement a legacy strategy and operational plan for long term growth.

We are searching for a Partnerships Manager (Maternity Cover) to join Jamie’s Farm’s HQ team, working across an exciting and growing charity. This job will be based at our gorgeous Bath Farm, which also acts as the charity’s HQ. The successful candidate will need to be at the Bath Farm at least two days a week. This role will involve regular travel in order to build connections with potential partners and to develop relationships with all of our farms. It will also involve attendance at conferences and events, to help raise our profile. 

About Jamie’s Farm

Jamie’s Farm is a charity which helps young people to flourish at home, in education and in the community through an intensive weeklong immersion in rural life. During a busy residential stay, young people help us run large working livestock farms and tackle many challenging and unfamiliar jobs, designed to offer everyone an opportunity for achievement, thereby building their self-esteem and confidence. They live in a supportive family environment that encourages them to push themselves out of their comfort zone. Our therapeutic input makes use of individual and group sessions to allow children to reflect on their lives on the farm and back home, to draw out tangible strategies for improvements to be realised when they return home.

Our programme is shown to have long term positive impacts on the lives of the young people we support. At the end of their visit, 66% of all young people showed improved mental wellbeing. Six weeks after their visit, of those with concerning attendance, 67% were no longer of concern. Six months after their visit, of those previously at risk of permanent exclusion, 64% were no longer at risk. These dramatic changes are only possible through the combined efforts of the entire Jamie’s Farm team.

Partnerships Manager Role 

Jamie’s Farm is quickly growing to help meet the increasing needs of young people from around the UK. The legacy of the pandemic, increasing mental health challenges and the cost-of-living crisis are all causing more children than ever to need the support of programmes like ours. Our existing farms are all full to capacity and booked many months ahead. As part of our strategic plan, we wish to grow our capacity and expand our geographical presence so that we can help more children, from more communities, during the second decade of Jamie’s Farm’s history. 

The Partnerships Manager plays a crucial role in the next phase of Jamie’s Farm’s growth. Our new farm in Skipton opened its doors to young people for day visits in April 2023 and is currently undergoing development as another residential farm. We have also just purchased a second farm near Bath, which is currently being developed. Combined, these two sites will allow us to work with another 80 partners and support almost 1,000 more young people every year. 

The Partnerships Manager’s principal task will be finding and developing these new partnerships. This will involve building relationships with schools, local authorities, academy chains and other charities around the country, sharing the benefits of a Jamie’s Farm programme and supporting them to make a visit possible. The Partnerships Manager will also work closely with other members of the HQ team to develop and expand other earned income streams, all of which are used to directly subsidise our groups of visiting young people.
