We’re looking for an innovative, passionate, and professional individual with excellent communication and organisational skills to join the Stroke Recovery Service based in Haringey. This is an exciting opportunity to work with stroke survivors and their families to provide stroke recovery support following a stroke.

Position: S11275 Stroke Support Coordinator

Location: Home-based, Haringey, however, regular travel will be required as part of this role within the service area (may include other travel in London and nationally to include team meetings or other work-related meetings).

Salary: Circa £27,400 per annum (inner London weighting £3,950 per annum or outer London weighting £2,100 per annum may be applied in accordance with where you live)

Hours: 35 hours per week

Contract: Services are contracted and there is currently funding for this contract until 31 March 2026.

The Role

Reporting to the London Service Delivery Coach, you will have the ability to support stroke survivors to identify their goals. Using strong assessment skills and a person-centred approach enabling them to maximise their recovery and improve communication. 

Key responsibilities will include:

• Supporting new stroke survivors and their carers from hospital discharge into the community

• Provide personalised information, advice and support

• Supporting stroke survivors to make informed lifestyle changes which will help them to prevent further strokes

• Work collaboratively with NHS colleagues and other areas of the community to make a difference in the lives of people affected by stroke

About You

You will have:

• Experience / background in a caring profession, ideally supporting people with disabilities

• Excellent IT skills and the ability to maintain accurate records

This role requires regular travel across Haringey and to visit people at home and in community settings. This is a new role in our Participation Team to help us deliver work that amplifies the voices of people with lived experience to find solutions to homelessness and health inequalities. The post holder will lead a few projects and partnerships, with responsibility for planning and delivering activities and monitoring their progress.

Some examples of current projects are:

• Coordinating and supporting a group of six people with lived experience to co-design an immersive audio story and campaign to improve the health of people living in temporary accommodation in London. 

• Working with the City of London Corporation to embed meaningful co-production practices in their homelessness services; bringing people with experience of homelessness, frontline staff, and commissioners together to improve service design and delivery. 

Your main task will be to coordinate groups of people with lived experience of homelessness and health inequalities, alongside Groundswell peers and colleagues, services, local authorities, funders and policymakers.

Location: Hybrid – NASP have an office space at London's Southbank Centre which can be used by staff at any time. Purpose of This Role: 

This is a crucial role to support NASP’s strategic objectives to promote and spread social prescribing at a local, national, and international level.

This pivotal role involves supporting a number of programmes that aim to develop social prescribing within healthcare systems. The role will work closely with our Healthcare Integration Lead to meet our workforce, clinical engagement, and education priorities across the organisation, providing programme coordination as well as supporting a variety of dynamic projects with the aim of supporting the expansion of delivery of social prescribing in a variety of different settings. Support Coordinator

We’re looking for an innovative, passionate, and professional individual with excellent communication and organisational skills to join the Stroke Recovery Service based in Lincolnshire. This is an exciting opportunity to work with stroke survivors and their families to provide stroke recovery support following a stroke.

There are two positions available:

1 x 28 hours per week – This is a fixed term contract until 31 March 2026
