Outline of service:

Gaddum and Manchester Mind have come together to provide a new and innovative service which will support people to leave inpatient mental health settings and return to their community with the support they need to recover and remain well.

As a team, we will deliver psycho-social interventions, advocacy, housing and welfare rights advice and peer support. There will also be tailored support to young people (those under the age of 25). All we do is underpinned by listening and ensuring people feel heard – because we know that when people are listened to, they recover. The team will be working with colleagues within the mental health, primary care, inpatient and community sectors.

Job summary:

As a Peer Support Worker, you will have lived or living experience of receiving mental health care or treatment. As an ‘Expert by Experience’ you will support peers (people with similar experiences to your own) who are ready to leave hospital after being treated for their mental health needs. Receiving support from your colleagues, you’ll develop and manage your casework practice and hold a small case load.

Walking alongside people prior to discharge from mental health hospital, you’ll support people in the discharge process to get the care they need, and then into a community setting where they can continue to recover and begin to explore their aspirations. Ultimately, people will feel connected to their communities – to the right services, friends and family.

This role will work with a small but intensive caseload (max 12 cases for a full-time peer support worker) to focus on:

· Repatriate: when people are sent to an out of area placement (OAPs) advocate to get them back to Greater Manchester so they can recover in their community.

· Discharge: Supporting people when they’re ready to be discharged from inpatient mental health services to get back to the community and recover.

· Prevent re-admissions: Work with people, post-discharge, to ensure they continue to get the support they need to make informed choices, stay well and ultimately thrive.

Main Duties and Responsibilities

· To provide bespoke support to people that are ready to be discharged from inpatient psychiatric care to return to the community, building (with support) packages of care so they have a positive and sustained recovery.

· To work alongside people post-discharge, in their community setting (e.g. home or supported accommodation) to ensure they get what they need to stay well.

· To effectively manage a small but intensive caseload (max 12 cases for a full time worker), being flexible to people’s needs and short-term changes in schedule.

· To work in a person-centred, trauma informed, empowering and culturally appropriate way that enables people to feel listened to and heard.

· To help people engage with services and make sustainable links with their communities.

· To use and develop your knowledge of community care, policies, legislation and services to get people the support they need to recover and stay well.

· Identify when cases need escalation or onward referral and keep your line manager aware of case developments.

· Independently visit people in inpatient and community settings in Manchester and across Greater Manchester, adhering to lone working and risk management procedures.

· Occasionally visit people out of the Greater Manchester area who need to return to their community in Manchester (all travel expenses will be paid and non-commute travel time will be part of your working day).

· Build relationships with colleagues within services (e.g. inpatient staff, social care, CMHTs etc) to enable improved collaborative working

· Take part in multi-disciplinary meetings, representing the views of people, and their aspirations on returning to community settings.

· Support the development of this new service, drawing on your role as an ‘Expert by Experience’ and using what you hear from the people you support to contribute to the development of the services by reviewing and contributing to improvements.

The post holder will be required to undertake other tasks as reasonably directed by the Programme Management and Senior Management Team, which will usually be commensurate with the skills and experience of the post-holder.

All staff aree expected to work within all Gaddum / Manchester Mind policies and procedures. This role is subject to a standard DBS check.

This is an opportunity to join Refuge as a Floating Support Worker to provide high quality practical and emotional support to survivors of domestic abuse and their children living in our dispersed refuge accommodation in Warwickshire.

You will be working as part of the Warwickshire Dispersed Safe Accommodation service. This service provides emergency accommodation across Warwickshire with high-quality domestic abuse support, ‘by-and-for’ services for survivors of domestic abuse and their children.

The post holder will provide high quality support and safety planning to survivors and their children who are fleeing domestic abuse. This includes enabling survivors to access housing, welfare, benefits and legal advice. A key requirement is to provide personal welfare support and to ensure that survivors are provided with a safe, supportive and welcoming environment in accordance with Refuge’s philosophical principles.

The Floating Support Worker (FSW) will carry a caseload of survivors of domestic abuse to enable them to provide a more holistic and intensive support package. They will work closely with survivors from the point of crisis through to move on from the emergency accommodation. FSWs will work in partnership with both statutory and voluntary sector partners to ensure that the whole range of survivors’ needs are met.

As part of this role, you will be required to participate in an out-of-hours on call rota.

This post is restricted to women due to the nature of the role. The Occupational Requirement under Schedule 9 (part 1) of the Equality Act 2010 applies.

• Work closely with the Supporter Engagement Lead to deliver the Supporter Engagement Transformation programme which is our key programme for growing retention

This is an exciting opportunity for an experienced project manager and effective communicator with an understanding of Health and Social Care legislation.  As Carers Hospital Support Coordinator, you will be responsible for coordinating an effective support service for family carers who support patients in hospital with the aim of reducing and avoiding hospital readmissions and supporting carers to sustain the caring role.  You will be working in hospitals settings across Redbridge, to provide comprehensive support and information to families and carers. To raise awareness of carers’ needs with professionals in the hospital environment and contribute to carer friendly policies. Your role will be key to the early identification and referral of carers to RCSS and other support services. 

The post holder will need to demonstrate a good understanding of health and social care legislation, be confident in liaising with professionals at all levels and ideally have experience of working with carers or having undertaken a caring role.  The role requires delivery of advice, information and informal advocacy and peer group sessions to carers. The post holder must have at least 2 years’ experience of successful project management and be able to set, record and evidence outcomes.

This new work is funded by the Department of Health and Social Care (DHSC) as part of the Accelerating Reform Fund. Redbridge is one of 7 North East London (NEL) boroughs funded to deliver Carers Hospital Support Coordinators.  The post holder will liaise with fellow Hospital Support Coordinators across NEL to develop effective peer networks and contribute to learning and impact reporting.  This is 12 month fixed term post with the possibility of extension subject to funding.

The shift pattern for this role will be on a rota basis. Some weekend work will be required.

Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation
Participate in the support planning and risk management, as facilitated by the lead Support Worker
Carry out security duties to ensure the safety of the customers and premises
Monitor the CCTV throughout the shift
Enable customers to make full use of community facilities by providing support as directed
Carry out support duties to enable customers to integrate into the community including e.g. accompanying the customer to attend benefit agency appointments and meetings, go shopping, to enquire at colleges about possible courses, to visit various public amenities
Report any observations relating to customers welfare

Kiran Support Services is a specialist South Asian charity providing refuge accommodation and outreach support to Asian women fleeing all forms of VAWG and harmful practices. Our services include refuge accommodation, resettlement and outreach support, advice and advocacy services, counselling and immigration advice and casework support.

We are a small team dedicated to providing intersectional culturally specific support to vulnerable women and their children through high quality services delivered in South Asian languages tailored to meet individual needs.

Job Title: Outreach & Advocacy Support Worker/ IDVA

Hours: 35 hours per week, 28 days annual leave, plus bank holidays

Annual Salary: £30,000 - £32,000 plus pension contribution.

Start date: 1st April 2025, 1 year fixed term contract. Extension for 1 further year subject to funding (provisionally agreed).

The main purpose of this role is support women and children living in the community in Waltham Forest and surrounding boroughs through the provision of one to one advocacy, advice and casework support. You must have a sound knowledge of housing legislation and welfare benefits. The role will require you to run advice surgeries and support groups to a wide range of Asian women across 3 boroughs.

An understanding of the intersectional complexities and dynamics of domestic abuse and its impact on women from South Asian communities is essential.

You will have demonstrable experience of providing advice and support to Asian women who have experienced domestic abuse, harmful practises and all forms of VAWG (violence against women and girls). You will be experienced at completing risk assessments and support planning. You will have a working knowledge of welfare benefits and housing legislation. An understanding of the issues faced by migrant women with no recourse to public funds is desirable.

It is essential that you are able to speak fluently in at least one South Asian language (Urdu / Punjabi preferred).

This peer support project is delivered in partnership with South West London and St George’s Mental Health NHS Trust to support Carers of people with mental health conditions.  This project will work to support Carers through one-to-one support, outreach, training and peer support groups.

About The Role: 

Using your lived experience of caring for someone with a mental health condition, and a strengths-based approach, you will support mental health Carers in their caring role.  You will provide person centred one-to-one- support, information, signposting, and advocacy, develop, and facilitate peer support groups and deliver training and outreach sessions at different locations across the borough. 

Key Requirements Include: 

• Lived experience of Caring for someone with a mental health condition 

• Knowledge and understanding of mental health and the associated challenges and support needs. 

• The ability to work within the principles and values of peer support.
