The Barnardos Achieving Change Together Plus Service will provide support for disabled young people (18-25) and their families, who live in Blaenau Gwent. This service will extend the support that is currently provided by the Achieving Change Together Service (0-18)

- To support and enable disabled young people and their families to access mainstream, specialist and community resources that meets their needs.
- To promote and increase inclusion and accessibility within communities.
What does the Job Role involve?
As a project worker (PW1), your duties will include:
- Utilising effective communication skills to engage young people and families to build collaborative relationships.
- Supporting young people and families to identify strengths and prioritise support needs.
- Planning, facilitating and evaluating groupwork programmes and one to one support.
- Maintaining accurate recordings.
- Celebrating achievements / successes.
- Establishing, developing and maintaining productive working relationships with colleagues and other agencies.
You will have:
- Experience of working with young people and their families.
- Experience of undertaking assessments with young people and families to co-produce holistic support plans.
- Experience of planning, facilitating and evaluating group programmes.
- Knowledge and experience of safeguarding.
- Ability to prioritise, organise, self-motivate and work on own initiative.
- Experience of partnership working with a range of voluntary and statutory agencies.

A lot of people know NCT for our antenatal classes, and we want to continue to support as many parents and families as we can to access evidence-based information. But we are also much more than this. We are a charity that campaigns on the issues that matter to parents. We run breastfeeding and infant feeding support, provided by specialist counsellors. We run thousands of free community activities and events across the UK, led by our fantastic volunteers and peers. And we support women and families facing specific challenges, such as social isolation, feeding difficulties or poor mental health.
Our Lancashire Infant Feeding Support project is commissioned by Lancashire Council, to offer infant feeding support across the region in both community and hospital settings. The support is delivered by staff and volunteer peer supporters.
We are seeking a capable Peer Supporter who will support parents at Preston Hospital and Ormskirk Hospital and at various community settings in Lancashire and offer support in service user homes. This role will mainly be based at out in the community, with some homeworking (expenses will be reimbursed).
About the Role
You will be responsible for offering breastfeeding support for parents within Preston Hospital and Ormskirk Hospital and the community.
Some of the key responsibilities of the role include:
· Providing breastfeeding support to families on the wards at the hospitals, in service user homes and community settings within Lancashire.
· Support in delivering inductions for volunteer peer supporters.
· To report any concerns or additional support needs of women to the NCT management team and/or Infant Feeding Lead for referral onto partner agencies as appropriate.
· To attend regular supervision/support sessions.
· Having a good working knowledge and understanding of the local demographics in the Lancashire area and ensuring all support is accessible and inclusive.
· Ensure that data collection and feedback is collected in a timely manner in an inclusive way.
You must have good communication skills and be able to build relationships and create a rapport with a wide range of people easily. This is a large project and covers multiple locations so you must be able to confidently prioritise your own work, be highly organised and able to work independently.
Being able to make decisions and respond appropriately to our staff, peer supporters and parents and other external stakeholders is essential, alongside good IT skills– including Outlook, Word and Excel – and you must have good attention to detail.
It is essential for this post that you undertake the training to become an NCT Breastfeeding Peer Supporter. This requires breastfeeding experience of at least 3 months. The training can be undertaken within your working hours.
The role is 17.5 hours per week which will be worked Monday Tuesday, Thursday 09:00 – 15:00. This will be split across the two hospitals, in service user homes and at community settings across Lancashire, however there is also some home-based working required.

About the Role
We are seeking a dedicated Lead Worker to join our client's floating support team, responsible for providing expert casework and resource provision for service users. This role involves supporting Assured RSI Tenants who may need assistance after graduating from support services, contributing to the implementation of team aims, and maintaining high standards in service delivery within a complex and changing environment.
Key Responsibilities:

Role Overview:
Housing and Service Delivery
- Provide strong and effective support to residents in Acorn House that focuses on their strengths and goals, and is delivered in a personalised way and within a framework of active engagement and co-production
- Developing and reviewing support plans that reflect the interests and aspirations of residents
- Induct new residents in line with the service’s policies and procedures and support them to
continue in their journey towards recovery from addiction - Carry out assessments of those referred to Acorn House, and induct new residents
- Ensure the fundamental needs of residents are met including good quality accommodation and facilities, healthy food, advice and guidance around benefits, and alcohol and drug testing (to help maintain an abstinent environment)
- Deal with the immediate support needs of residents as appropriate, providing crisis intervention where necessary

About the team
We specialise in emergency homelessness work, intensive support to children and families and people experiencing domestic abuse, and/or multiple disadvantage and have been providing housing advice and support to the people of Sheffield for over 20 years. We take a holistic approach to help people to deal with interrelated issues that can impact their ability to get and keep a home, so they can thrive within the community. These include high intensity support, legal representation, practical DIY assistance and specialist housing, employment training and welfare benefits advice. The clients we help have a range of issues include physical and mental health conditions, drug and alcohol use, income and money management and domestic abuse.
About the role
You will help clients to maintain independence and to thrive, by offering practical assistance, support, advice, advocacy, coaching and encouragement to enable households to resettle and to prevent homelessness. All our Support Workers hold a ‘caseload’ of households and are responsible for developing outcome focused support plans, managing any risks and continually assessing need. Shelter staff are highly knowledgeable and capable and we provide our staff with a range of training opportunities, in subjects such as: housing and homeless legislation; benefits and welfare; safeguarding and risk management.

About you
You will need a relevant background that includes experience of housing, homelessness and welfare benefits law and supporting people who are vulnerable and/or have multiple and complex needs. Our trauma informed staff specialise in providing nurture and care to children and young people
who are looked after. We work really hard to create a home environment that exceeds the needs of

The children and young people that we care for can demonstrate a wide range of behaviours and
can be very demanding of time, attention, and emotions. A senior residential childcare worker

Aquarius is a charity that helps people and communities overcome the harm caused by alcohol, drugs and gambling. We are part of ‘One Recovery Buckinghamshire’ (ORB), a partnership between Inclusion (MPFT NHS Trust), Aquarius, and Build on Belief providing support for alcohol and drug issues across Buckinghamshire.
We are looking for two Support Workers (Community Connectors – Engagement Team) to join our team. We have two positions available one position in Aylesbury and one position in High Wycombe. Please state on the supporting statement which position (or both) you are applying for.
Your challenge? To provide advice, support, advocacy and key work for recovery plans and undertake elements of assessment and case management as directed. And, all the while, we’ll expect you to operate in a way that recognises and values diversity and understands the functional model of substance misuse, the Recovery Agenda and the problems caused by alcohol and drug misuse.
Preferably with a NVQ 2/3 in Social Care, or willing to attain this, you are used to working to policy and procedural requirements that include child protection, vulnerable adults, confidentiality and health & safety. A flexible approach is important too, as is a commitment to taking a client centred approach to providing services and working in a way that recognises and values diversity.
