Position: S11267 Stroke Support Coordinator

Location: Home-based South Warwickshire, including Rugby. However, extensive travel will be required as part of this role (will include team meetings and other work-related meetings)

Hours: Full-time, 35 hours per week

Salary: Circa £27,435 per annum 

Contract: This is a fixed-term contract until 31 March 2026. Services are contracted and there is currently funding for this contract until 31 March 2026.

The Role

Reporting to the Stroke Service Delivery Coach, the coordinators will work with the local Health and Social Care services to receive referrals and support stroke survivors and their carers. As a coordinator you will provide personalised information, advice and support with regular reviews throughout.

Key responsibilities will include:

• Working with the local Health and Social services to receive referrals to support stroke survivors and their careers.

• Providing personalised information, advice and support to address any needs identified.

• Completing Needs Assessments with stroke survivors and supporting them to develop a stroke recovery plan.

About You

You will:

• Have a background in a caring and/or charity profession supporting people with disabilities.

• Be able to demonstrate previous experience of working collaboratively with other organisations to achieve joint ambitions. 

• Have experience of working with people who may have additional communication support needs.

• Have the ability to use basic Microsoft system.

Direct Support to Carers: 

• Deliver comprehensive carer assessments in line with the Care Act 2014, and co-produce tailored support plans

About the role

As an Experienced Practitioner, you will provide direct support and care for our young people who have complex emotional and mental health difficulties (such as trauma and loss) and can struggle to regulate their emotions. Working within a therapeutic framework, you will develop a direct programme of work aimed at developing authentic relationships with our young people (aged 16 – 24 years old), you will also have responsibility for making sure our residents are safe, providing support, liaison with other professionals and completion of support documents to record information.

Throughout the Waking Night shift, you will have the additional support of an on-call manager should there be any emergencies. Further to this, you will be involved in monthly team meetings and receive monthly supervision.

Job Purpose:

Reporting to the Partnerships Manager and working closely with colleagues in the Partnerships Team, the Partnerships Engagement Coordinator is responsible for working closely with volunteers and school staff to support successful delivery of interactive activities aimed at schools and young people under key funded projects for the charity’s Inspiring the Future and Primary Futures programmes. Sutton Community Works (SCW) is a Christian charity in South London. Sutton Foodbank is one of our faith in action projects and we are looking for a dedicated manager to oversee the smooth delivery of this amazing project supported by almost 100 volunteers!

Our vision is for our faith in action projects to contribute to a safe and flourishing community in Sutton.

Our work focuses on areas including food poverty, young people, rough sleepers and homelessness. In all our work, our ethos is to love God and others in all we do and say, ensuring people feel seen, heard, understood, and helped.

Sutton Foodbank has been helping people in food crisis since 2009, and supported nearly 4000 adults and children in 2024. We are open 5 times per week across 4 different venues and people are referred to by referral partners across the borough. Details of our sessions can be found on Sutton Foobank website.

Sessions are run by our dedicated volunteers who come from local churches; and our dependable warehouse, restocking and transport volunteers always ensure food gets safely to where it is needed.

People visiting the foodbank come from a vast variety of backgrounds but all have faced difficult life experiences. Beyond food, we also want people to access support to improve their situation, so we commision Citizens Advice to come to the foodbank sessions to help people maximise their income and provide advice on topics such as housing, debt and benefits.

And as a Christian project we underpin everything with prayer, and actively offer prayer to our clients too. The genuine care and compassion from our volunteers helps to build trusting relationships so we can more effectively listen, pray and signpost people to other support.

• A proven track record in working with and on behalf of children and foster families, using a trauma-informed therapeutic approach
