About the role

The service address the needs of homeless individuals with complex requirements. They are currently seeking a motivated and empathetic Complex Needs Project Worker to join the team on a temporary basis. The Accommodation based hostel service provides support to 14 service users enduring Complex Needs whom previously experienced sleeping rough.

Key Responsibilities:

• Support/Well-being Plans: Develop and implement individualised support and well being plans for each client in collaboration with the wider team.
• Needs & Risk Assessments: Conduct comprehensive needs and risk assessments to identify and address the unique challenges faced by each client.
• Liaison with External Support Services: Work closely with external support services, including health care providers, social services, and community organisations, to ensure holistic care for our clients.
• Income Maximisation: Support clients in maximising their income through housing and welfare benefits, providing guidance on financial literacy, and facilitating access to relevant resources.

• Previous experience working with homeless individuals with complex needs.
• Strong understanding of support planning, needs assessment, and risk management.

The Project

An emotional and wellbeing service that supports adults aged 18+ in primary care who are experiencing a range of mental health difficulties. Patients are offered extended consultation times, quick and easy access to practical advice and tailored support for their mental health needs from a multidisciplinary team.

Patients can explore the situation affecting their wellbeing – whether that is an ongoing mental or physical health problem, loneliness, debt, or other issues. They can then be guided to appropriate resources that may help, including talking therapies, benefits advice, or an introduction to a local community group.

The role of the Community Connector is fundamental to the developing of these innovative teams and mental health services based within networks of GP Practices (Primary Care Networks – PCNs). The role supports the ongoing development and mobilisation of integrated primary care mental health services.

The Role

Providing a primary care mental health service within this context refers to:

• First level of intervention
• Ease of access
• A multidisciplinary approach working a range of professionals across PCN’s inclusive of GP’s, Practice Nurses, Mental Health Practitioners, Pharmacists and Psychiatrists.
• Supporting patients to access to a wide range of community services and resources including social care, housing, family, debt and employment support that contribute toward maintaining good health and wellbeing.

About You

Below is the essential criteria of the role:

• A diploma or degree in appropriate subject i.e. counselling, psychology, social work, probation, mental health, Health and Social Care (Level 3) or NVQ Level 3+ and/or a minimum of 2 years community experience of mental health work.

• Understand confidentiality, consent, and GDPR
• Ability to travel within Surrey and local area if required.
• Ability to work flexibly across operational hours and evenings and weekends, where required.
• To work co-operatively as part of a multidisciplinary team

Summary of role:

As the NCS Project Officer, you will play a crucial role in supporting the smooth running of each Community Experience. You will lead and inspire cohorts of young people aged 16 to 17. You will be responsible for delivering Employability, Life Skills and Social Action sessions, coordinating programmes.

This is a fulfilling and challenging role where you will make a real impact on the lives of young people. As part of this role, you will need prior experience of working with young people and programme coordination and delivery. Whilst on the programme you will support and work closely with existing Groundwork provision, with staff in schools, colleges, and other local youth provision.  The Youth Project Manager and the Team Leader will support you with coordination and delivery of NCS. As the NCS Project Officer, you will be required to work the entire duration of one or more of the 16 Hour Community Experiences, work evening and weekends to support the delivery of NCS. We are looking for a Recovery Support Worker to join our passionate, and dedicated team in Hounslow. In this role you will support some of the most vulnerable individuals in the borough, helping them identify their individual strengths and aspirations.

The Hounslow LIFE service is a large multi-site service with a large team of colleagues with specialisms in Homelessness, Substance Use and Mental Health to provide a recovery based approach to 86 clients across our different sites.  

In the role of Recovery Support Worker, you will: 

• Work with clients on their road to recovery, aiding clients to gain the necessary skills to move on from our services and into suitable accommodation.  

• Provide support to maintain independence in the community, build their skills for employment, manage their health needs, and look towards a brighter future.  

• Oversee housing management responsibilities to ensure we deliver safe, high-quality services. 

In this role you will work on a weekly shift rota including early and late shifts, weekends and bank holidays. Staff can claim an additional 20% pay premium for hours worked between 8pm and 8am Monday to Friday, and all hours worked on Saturday and Sunday.

This is an exciting opportunity to join our team as a Senior Support Worker to make a positive impact on the lives of service users by co-ordinating the provision of direct housing related support and housing management services. This will be achieved by working with area operation and central support teams and liaising with a range of external agencies and partners. You will report directly to Service Lead for your region. 

The successful applicant will demonstrate a desire to coach and support their team and to lead by example in driving quality and accountability whilst delivering the contractual aims of the service.

You will have experience of working with stakeholders in the Criminal Justice System and be able to communicate effectively to partners and providers. This will include forming working relationships in the MoJ and with HMPPS, and also with external community agencies to better support the service.

You will have previous experience of supporting others and guiding them within a team as you will be required to support, coach and develop a staffing team and deputise for the Service Lead in their absence. 

This role provides an excellent springboard into other criminal justice professions including experience of working with a range of criminal justice agencies. In addition, you will gain experience in liaising with local authority Housing Benefit departments and the DWP to resolve financial issues/queries. 

This role requires regular travel and a Full Driving License with access to your vehicle are an essential criteria. 

An Enhanced DBS check is required for this role, as well as registration on the DBS Update Service. These will be completed as part of the Pre-Employment Screening process. 

This role also requires a Prison Clearance.

About the role:

As a Team Leader you will lead a team of Independent Victim Advisors ( caseworkers) providing support to victims across the county. You will hold a small caseload but your main focus will be on the performance management of direct line reports including the allocation/ auditing of cases and the collation of service performance reports, measuring KPIs and quality compliance. You will need a good understanding of the criminal justice sector and able to work collaboratively with partners to achieve the best outcomes for victims. Welcome to Richmond Fellowship’s Lincolnshire Crisis Service as a Recovery Worker.

Lincolnshire Crisis Service have two houses in the county, one in Boston and another in Lincoln, we provide a range of support for people with mental ill health and both work to a recovery model. We have been operating in the area for a number of years and offer a staffed service to our residents aged 18 and upwards. This role does require you to work varied hours as the service is 24/7 and may include some sleep in shifts, these are paid in addition.

We know that recovery can look and feel very different to each and every one of us. But we also know that if we provide the right support, at the right time, we can inspire recovery nationwide and that recovery is possible for everyone. That’s where you come in. Responsible for helping and inspiring the people who use our services, we’ll rely on you to work with them to come up with a personal support plan that will see them achieve their goals and aspirations. Whether it’s developing their domestic and finance management skills or accessing work, leisure or educational opportunities, one thing’s for sure – you will have every chance to shine.

This is the perfect role for a press officer with great news judgement, who is proactive about hunting out media opportunities, and tenacious about seeing ideas through until coverage appears.

This exciting Public Mental Health Officer role will work alongside community partners to identify, test, and embed the best approaches to improving mental health in communities. You will support the development of the evidence base on mental health, and generate compelling evidence of how systems can act to support good mental health.
