Community Connect is a well-established Social Prescribing service and you will empower and support residents to access charities, community organisations and groups to enable them to improve their own health & wellbeing. You will work closely with local organisations and healthcare professionals to continue to develop the project and provide the best service to residents.
Duties and Responsibilities
- Be responsible for incoming resident referrals. You will meet with the resident, understand their needs and what matters to them to develop a personalised action plan to provide relevant support.
- Be responsible for referring patients to onward voluntary and community groups for continuing support to their health and wellbeing.
- Provide ongoing support to the resident for approximately four sessions, which could vary from follow-up phone calls or attending appointments with the resident, centred around what is best for the resident.
- Be responsible for developing exit strategies and ensuring residents are receiving appropriate support, and aware of the right services before discharge.
- Actively feedback key themes and insight to ensure continuing development and support of Bexley’s Voluntary Sector.
- To assist in maintaining the database of local services and activities in Bexley.
- Meet residents in a variety of locations including community locations in Bexley and Bexley GP Practices.
- To provide regular updates and reports on caseload management, ensuring that the CRM system is kept sufficiently up-to-date and records all information accurately.
- Work in partnership with GP Surgeries, including providing patient updates, regular outreach sessions and attending multi-disciplinary meetings.
- Be a champion of Social Prescribing, and the voluntary sector, locally, regionally, and nationally.
- Undertake any other duties and tasks which may reasonably be required, as discussed with line-manager.
- Adhere to and actively implement BVSC’s policies and procedures such as Equality and Diversity, Health & Safety and Safeguarding.
- Identify, report, and monitor any safeguarding risks and concerns for Children, Young People, and Adults.
- Commit to undertaking any training considered necessary to ensure and develop own knowledge and skills to effectively deliver the role.
These are the normal duties which are required of the position; however, we do require that all staff be flexible and may be required to perform other duties to ensure the efficient running of services. It is envisaged that this post will be community based but this will be regularly reviewed.

Title: Consultancy for mid-term review of INSPIRE refractive error project
Location: Pakistan / Global
Contract: Approximately 30-45 workdays over six months
In Pakistan, eye health services are an integral part of the government health system, led by the National Committee for Eye Health (NCEH). The NCEH, which includes government and civil society stakeholders such as Sightsavers, has developed the ambitious IPEC plan 2020-2030. This plan highlights the need for strategies targeting disadvantaged communities, including women, the elderly, and people with disabilities. It also recognises the necessity for a comprehensive national refractive error (RE) strategy and a national training programme for opticians.
- Conduct a Mid-Term Review to assess the project's progress, successes, and challenges.
- Conduct primary data collection in the project areas (initial sampling suggests that around 116 individuals should be consulted across the stakeholder groups, although final details can be agreed between Sightsavers and the consultant during inception).
- Evaluate the effectiveness of implementation approaches, including equity strategies, social and behaviour change (SBC) activities, and public-private partnership models.
- Validate initial findings through a workshop with implementing partners.
- Provide recommendations for adjustments to the project and future similar projects.
- Contribute evidence towards answering Sightsavers' thematic learning questions for refractive error.
- Coordinate with various stakeholders to ensure timely delivery of activities and high-quality outputs.
- Extensive experience in monitoring and evaluation for international development projects/programmes, with strong skills in both qualitative and quantitative approaches.
- Thematic expertise in eye health evaluations, preferably with experience in refractive error, inclusive health, and primary eye care.
- Comprehensive understanding of preferred practices and policy issues at global and national levels.
- Strong understanding of equitable access to inclusive healthcare for women, people with disabilities, and other marginalised groups.
- Prior experience working in Pakistan, preferably within the project regions, including data collection experience.
- Ability to travel within Pakistan, mainly within the project districts (Multan and Sheikhupura).
Timeline
We anticipate the work will require approximately 30-45 person days' input, depending on the methodology and approach proposed. The work should be conducted between August and December 2024. About RLS
RLS is a UK charity and a non-profit company in Greece (AMKE).
Refugee Legal Support works in solidarity with people who migrate. We operate in the UK, France and Greece, promoting and protecting rights through legal support, casework, litigation, engagement, training and partnerships. We advocate for safe migration and oppose discriminatory immigration systems and policies.
RLS has held a legal aid contract in the UK since 2023.
About the Afghan Pro Bono Initiative (APBI)
The Afghan Pro Bono Initiative is a specialist casework project delivered by RLS with funding and pro bono assistance from 14 leading commercial law firms. It is a project delivered in partnership with SPI.
Firm pro bono lawyers are trained and supervised by the project’s two full-time supervising lawyers so that they can then assist with legal casework.
APBI has two core components. The team delivers legal information and support to and within the Afghan community, working together with the Community Engagement and Research Lead to ensure wide access. While the project also provides high quality legal representation in a range of cases for this community. This includes family reunion, the ARAP scheme, the Afghan Citizens Resettlement Scheme and others.
APBI then aims to respond to the increased need for legal information, advice and representation of Afghan nationals and their family members forced to seek protection and reunion with family in the United Kingdom following the Taliban takeover. The project is now going into its 3rd year, and won the Most Effective Pro Bono Partnership Award at the Law Works Pro Bono Awards in 2022 as well as being nominated and commended for numerous other awards in 2022 and 2023.
The casework of this project feeds into strategic research, policy and advocacy efforts to make existing ‘safe’ routes to protection (in particular refugee family reunion and resettlement schemes) more accessible and effective, advocate for their expansion and defend the fundamental rights of people on the move by influencing policy and decision making.
Litigation arising out of the project is actively pursued (where eligible) under RLS’s legal aid contract to push for better decision-making, secure results for individuals and pursue improvements in the relevant law, policy and practices.
RLS recognise that pro bono legal work is always an adjunct to, and not a substitute for, an adequate system of publicly funded legal services.
Key responsibilities and objectives
The post holder will be a staff member of Refugee Legal Support. They will be a participant within the Afghan Pro Bono Initiative project team, which comprises two lawyers (the other being at SPI), a coordinator and a Community Engagement and Research Lead in addition to the volunteer lawyers from the individual commercial firm partners.
Key responsibilities

Manage a small caseload and provide high quality legal advice and representation to clients;

Supervise the work of volunteer pro bono commercial lawyers on cases;

Deliver training to pro bono commercial lawyers;

Ensure that all legal work is promptly and accurate recorded on RLS’s case management system and keep relevant time recording;

About the role
As a Wellbeing Assessor, you will use your skills and experience to work directly with people with mental health issues in one-to-one settings, including telephone, face-to-face and online engagement. About the role
The NextGen Community Producer will play an integral role at Youth Music shaping and developing our support for young adults, particularly helping them to close the gap between learning and earning in music.

About the opportunity
Critical to our mission, we are looking to recruit a South West and South East Programme Manager to be responsible for three geographical areas and the schools within them, and to support further geographical expansion. You will oversee your region’s Action Tutoring programmes, liaising with our partner schools and developing new school partnerships, whilst supporting your team of Programme Coordinators to engage pupils and support them to build relationships and retain a strong pool of volunteer tutors.
As a Programme Manager, you will line manage Programme Coordinators to ensure they effectively manage their caseload of schools, whilst driving the quality of programmes and ensuring that the delivery of programmes meets organisational KPIs. This role will take a lead on new school recruitment, regularly pitching to Senior Leaders within schools and managing school partnerships within your region to ensure Action Tutoring’s growth targets are met. Additionally, you will contribute to the wider leadership of the Action Tutoring programme team, lead on programme-specific projects to support the programme department's priorities and will be expected to support the wider learning and development needs of the Programme Coordinator team.
This position would suit someone who enjoys working with people who have a frontline role, gets energy from pitching to schools and wants to increase their management experience in an exciting and rapidly developing charity. The successful candidate should be able to manage their own workload to a high standard as well as support others, have excellent attention to detail, be adaptable, relational and used to problem-solving and be able to work well with a range of stakeholders and be confident carrying out administrative tasks to support programme delivery.

· Lead on reviewing and improving programme delivery in the South West and South East through regular visits to schools and programmes. Using data to reliably assess programme performance and engaging with quality assurance processes to drive impact.
· Develop and strengthen relationships with key stakeholders in Action Tutoring’s partner schools in the South West and South East, including members of the Senior Leadership Team, ensuring strong retention of school partnerships each academic year.
· Take a lead, with support from others in Action Tutoring, on networking in the South West and South East. This could include connecting and attending events with other charities, universities and other organisations to build Action Tutoring’s presence locally.
· Work closely with the Marketing, Communication and Engagement teams to establish and maintain volunteer recruitment opportunities and channels in the South West and South East.

Leaders Plus is an award-winning social enterprise dedicated to supporting leaders with babies and young children progress their leadership careers. Their flagship intervention is a 9 month award winning Fellowship Programme for working parents which leads to promotion and therefore gender equality at the top. Their employer partners include varied organisations such as RSPCA, King's College Hospital Trust and HSBC Global Internal Audit.
As Business Development Manager, you will lead the growth of their employer partnerships, and manage a pipeline of applications from working parents, reporting to their CEO and Founder. This will help them increase the number of parents they support, so that in 2023, they have 1000 woman in executive director roles who wouldn't be there without their support.
You will join Leaders Plus at a crucial stage of growth, and play a crucial role in helping them to scale further. You will join a small team who value making a difference, excelling at work and enjoying their time off - at the moment all the team work part-time and at the moment all have Friday's off.

Some of the main responsibilities include;
- Generate an income through offering the Leaders Plus Fellowship to employers.
- Develop new employer partnerships through referrals, cold leads, LinkedIn and events.
- Manage a pipeline of potential employer partners who are interested in their Leaders Plus Fellowship Programmes and wider work.
- Manage a pipeline of potential applicants, answer questions and have calls with interested applicants.
- Develop long-term purpose-driven partnerships with current employer partners to advance their mission of supporting career progression for working parents.
- Grow the scale of their existing partnerships and increase the value for everyone involved.
