Background to the role
We have teams embedded in four hospitals across East London where they are recognised for the warm, responsive and kind person-centred service they provide for patients. They work in close partnership with ward managers, discharge teams and social workers to ensure the safe and timely discharge of patients, connecting them back to their local communities and ensuring that their chances of a sustained and speedy recovery are maximised. Our model has proven success in reducing re-admissions.
Over the past 12 months we have worked to reorganise how our Home and Settle team works. We have done this to reduce the pressure on individual staff members, increase the extent to which the teams work together and improve the consistency and sustainability of the services. The Discharge and Settle Worker role is key to this new model of delivery.
The postholder will be working in the community and in patients homes to help them settle back into life and to connect with the support and services that will help them avoid another hospital visit as far as possible. Maintaining good relationships with the rest of Age UK East London’s Home and Settle team and will ensure the smooth running of the service and the best possible outcomes for patients.

Job description
Job Purpose
The home and settle service works collaboratively with hospitals to support people when they are discharged from hospital to return home. You will provide temporary practical and emotional support to service users within their home and make sure they understand the care and treatment choices available to them. The service is time limited, usually up to 6 weeks, to adjust and settle back in their homes.
Key Tasks
- Assess the needs of service users in hospital before discharge, and in their homes, and understand what barriers and enablers there might be to moving home.
- Aid the discharge process and improve patient experience through tasks such as key cutting and co-ordinating with colleagues for the setup of the home environment.
- Provide (and/or arrange) the practical and emotional support needed to enable service users to move home and/or remain safely at home.
- Be vigilant and make referrals/signpost for additional needs that our wider team, or partners can support. E.g. for benefit checks, energy advice, carers support, befriending.
- Help maximise service user independence by adopting an enabling approach.
- Develop a listening and caring relationship with the service user and their families/carers.
- Provide domestic practical support within the home e.g. light cleaning, washing, ironing (service user clothing only) etc.
- Prepare and service drinks and simple meals with or for the service user customer ensuring nutritional needs are met in accordance with individual support plan.
- Accompany service user outside the home e.g. hospital/GP appointments etc.
- Support customers with other tasks e.g. making telephone calls on their behalf, reading and responding to correspondence, completing simple forms (training will be provided) as directed by the customer etc.
- Ensure services provided by AUKEL in the community are safe and person centred.
- Report safeguarding concerns, in accordance with AUKEL safeguarding procedures.
- Work within AUKEL’s expectations of professional boundaries and confidentiality.
- Provide cover in the case of sickness and annual leave periods of colleagues.
- Support service user reviews, ensuring all visiting information is uploaded on to AUKEL’s organisational case management system.
- Meet with volunteers to provide support and guidance.

- Supporting health and social care professionals with patient discharge
- Liaising with the Hospital health and social care professionals to facilitate smooth supported discharge.
Administration
- Input all records, reviews, and visiting information to service user’s case notes in a timely manner to maintain up-to-date and accurate records on AUKEL’s case management system (currently Charity Log/Call-round App).
- Report all “no access” failed visits, or changes in service users’ condition or circumstances in accordance with AUKEL’s policies and procedures.
- Ensure service user comment sheets are completed.
- Support the return of Customer Comment sheets and general feedback in respect of your own service users.
Quality
- Ensure services provided in the community are delivered in line with the Care Quality Commission (CQC) principles.
- Deliver all work in line with AUKEL quality mark standards.
- Commit to undertaking the Care Certificate, if not already held or in possession of NVQ level 2.
- Complete essential training as required by AUKEL to provide safe services.
Liaison
- Work in collaboration with other agencies providing support within the customer's home.
- Work under the direction of the service manager and project officers.
- Represent AUKEL and participate in appropriate external meetings and events to remain aware of local, regional, and national issues affecting quality and compliance issues affecting care and home support services.
General
- Meet regularly with your line manager for support, supervision, and appraisal.
- Attend team and staff meetings, (and other meetings) as required.
- Undertake any other duties within the competence of the post holder as may be required from time to time for the continued smooth running of AUKEL.

You will provide capacity-building in the form of advice, advocacy, support, and briefings across the London boroughs to statutory agencies and community organizations around the needs of Eastern European domestic abuse survivors, to embed best practice across our communities.
It is desirable for candidates to be proficient in one or more Eastern European languages.
You will have proven experience of providing direct emotional and practical support to women as well as up-to-date knowledge of legislation relating to survivors of gender-based violence. You will have excellent casework skills, good written and verbal communication skills, clear professional boundaries and be a proactive team player. All candidates must demonstrate a commitment to the feminist values of empowerment and equality which underpin all of our work.
The post holder will provide a high-quality personal welfare support service to Eastern European survivors of domestic abuse and other forms of violence and abuse and their children. The post holder will empower Eastern European survivors by providing them with emotional and practical support, including support to access safe accommodation.

Your Role
Programme Leadership & Delivery
- Coordinate Power UP! - Plan, manage, and deliver activities including youth-led research, grant-making, internships and leadership opportunities.

Role Context and purpose
Following recent and projected growth of the organisation, Gaddum is looking to recruit additional resource within the Central team to bolster support; ensuring delivery of safe and high-quality services. Gaddum acts as an administrator for smaller trusts and charities, who do not have internal finance teams, and this role will act as the key point of contact for those trusts in delivering financial support. In addition to this, Gaddum also redistributes grant funding – this role is to act as the Finance lead for the financial management of the funding.
The post holder, line managed by the Finance and Resources Director, will join the finance team and work alongside another Finance Assistant responsible for Accounts Payable and Receivable. This is a 28 hours per week role, working flexibly to suit the needs of the organisation and individual.
Main Duties and Responsibilities
Trust Funds
• Key point of contact for trusts
• Administration of trust fund applications against agreed policies between Gaddum and the associated trusts
• Coordination and administration of trust meetings on behalf of the organisation and trusts
• Maintaining accurate, timely and relevant written and financial records, pertaining to the administration of the trust funds
• Respond to enquiries and requests by phone, email and in person as required
• To assist with collating and inputting of data, e.g. accounts for reporting periods
• To liaise with referring organisations, referrers and beneficiaries where necessary during the day-to-day administration of trust funds
Grant Funding
• Working with the existing Grant Coordinator in ensuring the regular invoicing of grant funding
• Accurate and timely processing and payment of this funding to organisations within the region

OVERVIEW OF THE ROLE
We have worked in partnership with the Young Roots charity for the last five years providing a service to young, unaccompanied asylum seekers aged between 16 and 25 and we are looking for a Senior Clinical Psychologist to manage and support this work.
As a Senior Clinical Psychologist within the Young Roots psychology service, you will report to a Senior Clinical Psychologist at HBF/Head of Therapies and manage the clinical services of the Young Roots Partnership.
This role will include management of the overall partnership project, delivery of highly specialised psychological assessments for both medico-legal and clinical purposes, provision of appropriate evidence-based therapies, delivery of clinical supervision to qualified clinicians, assistant and trainee clinical psychologists on final year specialist placements, and non-clinical members of staff. The role will also include input into policy, research and fundraising and delivery of consultancy and training.
We are looking for someone who is committed to working therapeutically with young people who experienced human rights abuses and post-migration stressors and present with trauma spectrum disorders, depression, and anxiety as a result. In addition to proven experience in working therapeutically with this client group, the candidate will have a passion for human rights work beyond the individual client work, and a desire to contribute to effecting change at a policy level. This role requires the ability to react and respond creatively and appropriately to the needs of the charity.
MAIN DUTIES AND RESPONSIBILITIES
Under the supervision of senior clinicians at the Helen Bamber Foundation, work independently to ensure the delivery of a range of evidence based psychological therapies to clients.
· Provide specialist psychological assessments and deliver trauma-focused psychological therapies to clients of the Helen Bamber Foundation.
· Train in the preparation of medico-legal reports and where relevant, contribute to the preparation of these reports for clients.
· Develop and carry out audits and research projects.
· Provide internal and external training in collaboration with your line manager.
· Supervise junior staff. This may include newly qualified psychologists, trainee clinical psychologists, assistant psychologists, and research assistants.
· Provide supervisory support for non-clinical staff where needed.
· Liaise with other non-statutory and statutory organisations on matters of clinical care, safeguarding and policy.
· Monitor the effectiveness of the interventions provided and provide feedback to funding bodies in the form of data and reports.

About the Service
This is an exciting and reasonably new project, ‘Flourish’ – Mental Health for Refugees in Hertfordshire, which is a highly flexible and responsive outreach service developed to meet the needs of refugees fleeing the war in Ukraine & other refugees in Hertfordshire. The aim of the service will be to support refugees in the community with multiple needs who have mental health support as a primary need.
The countywide holistic service also provides emotional wellbeing support for families who are hosting refugees.
The Flourish Team Leader plays a significant role in the co-ordination and running of the Flourish service. This role is key to the development and support of the team and the wider organisation.
Service Objectives
- To provide advice, information, onward referral and holistic support to clients with mental ill health.
- Support to build social networks and reduce isolation whilst settling into the Hertfordshire community this includes building a sustainable and empowering peer support network and building confidence and self-esteem.
- To ensure that the safety and wellbeing of the client is monitored and reviewed regularly.
- To remain a source of independent support for all clients.
- Embed a person centred and solution focused approach in all aspects of the role.
- To work in a trauma informed way including awareness of the 6 principles of trauma informed practice, putting the clients emotional and practical needs at the forefront of this work.
Key Responsibilities:

About the Role
Our Lancashire Infant Feeding Support project is commissioned by Lancashire Council, to offer infant feeding support across the region in both community and hospital settings. The support is delivered by staff and volunteer peer supporters.
We are seeking a capable Peer Supporter who will support parents at Royal Preston Hospital and on occasion East Lancashire Hospital Trust or Ormskirk District General Hospital as well as community groups and home visits in Central and East Lancashire. This role will mainly be based at out in the community, with some homeworking. Expenses will be reimbursed to/from a base (Royal Preston Hospital).
You will be responsible for offering breastfeeding support for parents within Royal Preston Hospital and on occasion East Lancashire Hospital Trust and Ormskirk District General Hospital and the community.
Some of the key responsibilities of the role include:
- Providing breastfeeding support to families on the wards at the hospitals, in service user homes and community settings within Lancashire.
- Support in delivering inductions for volunteer peer supporters.
- To report any concerns or additional support needs of women to the NCT management team and/or Infant Feeding Lead for referral onto partner agencies as appropriate.
- To attend regular supervision/support sessions.
- Having a good working knowledge and understanding of the local demographics in the Lancashire area and ensuring all support is accessible and inclusive.
- Ensure that data collection and feedback is collected in a timely manner in an inclusive way.
You must have good communication skills and be able to build relationships and create a rapport with a wide range of people easily. This is a large project and covers multiple locations so you must be able to confidently prioritise your own work, be highly organised and able to work independently.
Being able to make decisions and respond appropriately to our staff, peer supporters and parents and other external stakeholders is essential, alongside good IT skills– including Outlook, Word and Excel – and you must have good attention to detail.
It is essential for this post that you undertake the training to become an NCT Breastfeeding Peer Supporter. This requires breastfeeding experience of at least 3 months. The training can be undertaken within your working hours.
The role is 17.5 hours per week which will be worked Monday Tuesday, Friday 09:00 – 15:00. This will be split across the two hospitals, in service user homes and at community settings across Lancashire, however there is also some home-based working required.

Veterans Outreach Support (VOS) has secured funding from the Armed Forces Covenant Fund Trust to expand the Thrive Together programme (formerly VPPP) across the South East. This initiative connects veterans' support agencies, ensuring seamless pathways, safe spaces for veterans, and training for volunteers and staff working in veterans' mental health. They are now seeking a Families & Carers Outreach Officer to explore the challenges of reaching out to families and carers of veterans to improve the way that this support can be delivered.
VOS is an award-winning charity that offers welfare, wellbeing and mental health support and services to veterans of the UK Armed Forces and Merchant Navy in Portsmouth, Hampshire, and the Isle of Wight. VOS provide a drop-in model of support, a welfare advice service, peer-supported wellbeing activities and social groups, with access to psychology, psychiatry, and therapy services.
The Families & Carers Outreach Officer will collaborate with the Thrive Together Portfolio Manager and VOS team to identify and improve outreach to families and carers of veterans, many of whom may not know they’re entitled to support.
The successful candidate will be able to demonstrate:
- Experience with identifying, planning, running, and evaluating pilot projects, research groups, and focus groups, while managing timelines.
