Peer Support Coordinator

This role is an opportunity to work for Mind BLMK as part of the peer support service, you will have the opportunity to use your own personal experience of mental health challenges to coordinate the running of a Peer Support Service in Milton Keynes, provide information and guidance, and implement referral pathways to the wider Recovery Service.

Our peer support and recovery services are a key element of the services we offer, and we are looking for someone who can use their own lived experiences to provide support to help individuals achieve their goals. Key Activities

• Operate a Peer Support service including peer support groups and mentoring across Milton Keynes and ensure outcomes which supports individuals with mental health and wellbeing needs in line with Mind BLMK’s agreed Peer Support model and requirements as well as contract requirements.
• Carry out assessments on individuals’ suitability for Mind BLMK’s Peer Support service in line with the Access to Service procedure
• Work with the Peer Support Manager to ensure regular involvement of service users, volunteers and staff on local service delivery, organisational matters and wider mental health strategies
• Maintain an effective workforce for the service in line with Mind BLMK’s HR policies, procedures and guidance (recruitment, line management, workload planning, support and development of Peer Support Workers and volunteers).
• Establish and maintain effective working relationships with the Mind BLMK team and relevant key post holders.
• Provide the infrastructure Peer Support Workers need to deliver the support safely and confidently.
• Hold responsibility for all Cash Handling, Health and Safety and Data Collection responsibilities as directed by the Team Leader in line with Mind BLMK’s policies, procedures, requirements and guidance.

South East London Mind are working in partnership with South London & Maudsley NHS Foundation Trust (SLaM) to deliver a Primary Care Mental Health Team (PCMHT) for Lewisham.

The integrated team provide support to people with long-term mental health problems within a primary care setting, helping people develop their independence, self-management skills and achieve their recovery goals.

We are looking for a full time Mental Health Advisor to work alongside SLaM colleagues to deliver this service. As part of an effective Information Advice and Guidance team you will have the caseload management experience and skills to deliver focused, person-centred, individual support and IAG (Information and Guidance) on a wide range of social issues including housing, benefits, employment, socialising, liaising appropriately with specialists and professionals.

Our team in Greenwich work together to offer a range of support to promote wellbeing and help people to manage their mental health. Client needs shape our services and they are at the centre of all we do.

We are recruiting a Peer Support Coordinator to lead the development and delivery of Peer Support sessions and work with Peer Support Volunteers to develop new opportunities. Groups will be held at our Greenwich centre and at venues across the borough of Greenwich. We particularly want to increase engagement of under-served communities in Greenwich.

You will have experience of group development, collaborative working across service teams and an excellent understanding of service user engagement. You will be highly organised and ensure all client data is accurately entered into our Views database. All records of groups and activities will also be entered promptly onto the database. Additionally, you will complete all client Peer Support registrations to ensure client needs are assessed and positive outcomes are achieved through engagement with the Peer Support Services.

Bromley Mental Health Hub is part of the transformation of mental health services in the London Borough of Bromley under the NHS Long Term Plan. The service will bring together the expertise of local primary, secondary and voluntary sector mental health providers with the aim of providing flexible, holistic and integrated services for adults with mental health problems.

We are looking to hire a Senior Mental Health Advisor with experience of working within mental health services, including the prevention, recovery and self-management of mental health problems. Aquarius is a charity that helps people and communities overcome the harm caused by alcohol, drugs and gambling. Right now, we’re looking for someone like you to join our NGAGE Young People’s Service in Northampton. NGAGE is an early intervention drug and alcohol service for young people aged 10-18 and we provide information, education, advice and support to young people in relation to drug and alcohol use.

Your challenge? To provide advice, assessments, brief and structured interventions and group programmes. As a Practitioner, you will be the initial contact for referrals into the service, conducting assessments via telephone, face to face and offering relevant and appropriate advice, harm minimisation, family interventions risk assessment and management, actioning onward referrals to partner agencies. You will also be required to manage a caseload.

An energetic and confident self-starter, you have a qualification in health/social care, youth and community work (e.g. NVQ Level 3 or above, DipSW, Mental Health Nursing, Counselling, Addiction Studies). Alternatively, we’ll consider candidates with experience of working in the substance misuse field with a commitment to complete NVQ Level 3 Health and Social Care. As well as good knowledge of alcohol/drug and health related issues, you're used to liaising with voluntary and statutory agencies and health professionals and comfortable engaging with clients in a variety of service delivery settings. Right now, we’re looking for a self-motivated individual to join us at our Recovery Near You service in Wolverhampton. This is a substance misuse service that works with adults aged 18+ and is run in partnership with Nacro and Birmingham and Solihull Mental Health Foundation Trust.

Your challenge? To provide advice, support, advocacy and key work for recovery plans and undertake elements of assessment and case management as directed. You’ll work not only from Aquarius offices, but also in primary care, arrest referral, hospital and youth settings and other outreach settings as required. And, all the while, we’ll expect you to operate in a way that recognises and values diversity and understands the functional model of substance misuse, the Recovery Agenda and the problems caused by alcohol and drug misuse.

Preferably with a NVQ 2/3 in Social Care, or willing to attain this, you can work within policy and procedural requirements that include child protection, vulnerable adults, confidentiality and health & safety. We’ll also be looking for good written and verbal communication skills and the ability to maintain accurate records and fulfil data requirements. You will work with young people between the age of 17-25 with needs related to substance misuse, including those that are affected by their parent’s substance misuse. The purpose of the role is to engage young people in a variety of interventions, using a young person centred approach, empowering them to reduce risk, increase resilience and achieve positive outcomes.

Based in the Transitions team  you will support young people with multiple risk factors and substance use. Our Complex Needs Practitioners work flexibly across the community and within our multi-agency partnership settings. The caseload will include a mixture of community and multi-agency referral pathways including health, social care, education and criminal justice.

A core part of the role will be working with young people and their families to address holistic support needs across all aspects of the young person’s wellbeing. This could include diverse work related to homelessness, family work, exploitation, physical and emotional health as well as substance misuse.

The skills and knowledge that are important to us are:

• A relevant professional qualification in Social Work, Counselling, or Youth & Community Work (at graduate level).

• Awareness of the impact of mental health on the lives of young people.

Community Navigator (Mental Health)

This role is an opportunity to work for Mind BLMK as part of the Milton Keynes NHS mental health service for someone with experience working in health, social care or information and advice, in direct contact with service users.

Working alongside colleagues from Central Northwest London Foundation Trust (CNWL) this role will provide referral management to and from Secondary Services. This will include signposting patients to statutory services including talking therapies and wider community based & voluntary sector support.

The Community Navigator will work as part of the team to deliver on the aims and objectives of the Mental Health Hub whilst representing the Voluntary and Community sector with the NHS.

Service Delivery

• Work within the NHS Mental Health SPA (Single Point of Access) Team to review and process referrals received from GP Practices and other sources including those to be subsequently triaged and assessed by mental health practitioners.
• Actively call patients with a view to assisting them through the process and making their onward appointments into community or secondary care service providers.
• Support the hub to signpost patients and direct them to appropriate services including secondary care, the local Voluntary and Community sector services and activities that support the patients’ wider determinates of their mental health and wellbeing.
• Answer calls, ensure sufficient phone cover, respond to messages/voicemails, provide administrative support to the Mental Health SPA for all services and deal with queries in a sensitive and timely way liaising with clinical or team lead as is appropriate.
• Monitor service inboxes during core office hours, responding to any queries professionally providing helpful information.
• Monitor and action tasks within SystmOne and maintain records of all activities carried out, i.e. outbound calls, calls received, archiving any received correspondence.
• Build good working relationships with the voluntary and community sector, relevant services and activities to ensure signposting pathways are kept up to date as well navigating and developing the setup of new signposting pathways through networking.
• Provide clinical and non-clinical information in appropriate chronological templates to other services.
• Carry out health and safety responsibilities as directed by Managers in line with Mind BLMK’s H&S policies, procedures, and guidance (risk assessments, reviews, reporting, safeguarding, incidents/accidents, premises).
• To bring VCSE experience, knowledge and culture into an NHS team.

About the Peer Support Services

Our Peer Support Service started 10 years ago, with 2 Wellbeing Centres offering this new service. Since then, we have seen our service grow and develop in ways we could not have imagined. This service is the key service most requested by people and we now offer Peer Support across all of our Wellbeing Centres across Hertfordshire.

Working in co-production with clients, staff, volunteers and stakeholders, we were able to create our own Peer Support Charter Values (this can be found on our website). These values form the ethos we work towards within our peer support team. These values also complement HMN’s values of Hope, Courage, Togetherness and Responsive, which form the basis of all the work we do as an organisation. 

About the Role

Working with the team to offer 1-1 peer support to anyone accessing HMN. The Peer Support service provide tailored support for individuals to support their needs. This includes using different tools and techniques to help an individual improve areas of their life they may be struggling with. Having a lived experience of mental health issues or supporting someone who has is a key element to what makes this service so unique.

The Peer Support approach includes empathy, warmth, acceptance, authenticity and compassion. Peer Support is based on the recognition that there is no better person to support the path towards recovery, than someone who has walked a similar path as that person.

To meet client need, and work to this personalised approach, we do support people out in the community, within their home, as well as in one of our Wellbeing Centres. Being able to drive and having access to your own vehicle (or equivalent) is essential for this role.

The successful candidate will need a personal understanding of mental ill health, either from your own lived experience or by caring for someone with a mental health condition and have awareness of issues in mental health service provision.

Peer Support Workers will also provide support within our Peer Support Groups, Peer Support Learning Sets and Peer Support Academy when needed.

Princess Court is a mental health, supported housing, 24 hour supporting housing and community-based service, working to a recovery model. We provide a range of support services for people with mental ill health to access in their local community, including outreach support, peer support and supported housing. We have been operating in the area for several years and offer a staffed service to our residents aged 18 and upwards.

We know that recovery can look and feel quite different to every one of us. But we also know that if we provide the right support, at the right time, we can inspire recovery nationwide and that recovery is possible for everyone. That is where you come in. Responsible for helping and inspiring the people who use our services to maintain their tenancy and independence in the community, we will rely on you to work with them to produce a personal support plan that will see them achieve their goals and aspirations. Whether it is developing their domestic and finance management skills or accessing work, leisure, or educational opportunities, one thing is for sure ‐ you will have every chance to shine.

No relevant experience is required as full training will be provided, although candidates with an understanding of mental health issues and/or lived experience would be welcomed. More important is your caring and compassionate nature and empathy and enthusiasm for helping others. Whatever your background, you will need to be happy to work both independently and within a team and willing to be part of a weekly rota system and available for on call duties.
