Social Prescribing Link Worker - Health and Wellbeing 

GP practices across North Birmingham, office (Castle Vale) - Agile 

£26,802.47 increasing to £30,457.35 after passing probation   

Full Time 

Fixed Term Contract – up to 31st March 2025

They have an opportunity for a Social Prescription Link Worker to work across GP practices in North Birmingham, as well as their office in Castle Vale plus some home based. 

 

The Link Worker role is incredibly valuable and often involves supporting people through the most challenging times of their lives. The services eases the pressure on the NHS and allows patients to receive a wide range of support in a timely manner.   

 

You will support patients with social, emotional and wellbeing needs. You will create personalised support plans and empower individuals to work through complex cases. This will enable and empower individuals to enhance their overall wellbeing and quality of life, in areas affected upon assessment and referral. 

 

Patient engagement is through face-to-face appointments in GP surgeries across the Erdington constituency and telephone consultations.  

 

About you 

 

They need a confident self-starter who can easily build strong relationships with a variety of health and social care professionals. 

 

Previous experience in a similar role would be advantageous and you will ideally have experience of delivering services in a community setting, particularly to vulnerable adults. 

 

You will need to be proactive and organised with the ability to manage a busy workload.
