Job description

In this role, you will provide tailored, high-quality housing-related support to clients with complex needs, including those who may be homeless, isolated, or facing challenges such as substance misuse, mental health issues, or isolation. You will manage a caseload of clients, delivering personalised support plans designed to maximise independence and facilitate successful transitions into independent living.

 

If you have:

 

  • The ability to assess and identify individual support needs.
  • Strong communication skills when networking with relevant agencies for specialised support
  • Experience in assisting clients in securing and maintaining accommodation.
  • A full UK drivers licence and access to a vehicle for work.
