Community Mental Health Practitioner

You’ll be responsible for enhancing the care for service users with complex mental health needs by planning and implementing appropriate care using validated rating scales, risk profiles, assessment protocols, and individual care plans.

If you have:

• Experience working in mental health services.
• The ability to assess and manage risk using a therapeutic framework.
• Excellent prioritisation skills
• The ability to develop and maintain working relationships
