Senior Care Navigator (Frailty Service)
The Bridge Renewal Trust, in partnership with the NHS, deliver Care Navigation services for older people in Haringey.
As the Senior Care Navigator, you will provide supervision and support to the existing Care Navigation Team, leading on the coordination of work, allocating resources and ensuring every team member fulfils their role to the best of their ability through effective delivery, monitoring, and reporting.

Are you a passionate and dynamic individual with excellent organisational and interpersonal skills; can you motivate, support and coordinate the Care Navigation Team that sits within a wider multi-disciplinary team, working closely with GP practices and other healthcare professionals, the public and voluntary sector?
We are currently recruiting a Senior Care Navigator who will focus on leading a small yet dedicated team of Care Navigators who are helping patients to make full use of community and voluntary sector services available locally. You will be responsible for allocating workloads, championing a personalised approach to providing support, and modelling exemplary working practises to motivate the team to achieve their very best.
