Job description
We are looking for an exceptional individual to join our vibrant, passionate, and friendly Dementia Support service in Derby City

About the role

Location: Community based in Derby City with some homeworking elements. 

As a Dementia Adviser you will have the rewarding opportunity to provide support and guidance to people with dementia and their carers; helping to maintain their independence, improving their sense of well-being, and putting them in more control of their lives. The service is unique to each service user as it is based upon their personal circumstances and support needs.

You will offer support and guidance to your clients in a variety of ways, whether in the client's own home or at other locations, face to face, by phone, letter, or email. You will assist service users to access other services, providing signposting and referrals. Your ability to provide person centred assessment of need, along with a clear understanding of the need for client confidentially is essential to providing a valuable and worthwhile service.  You will also be delivering groups within the Derby City area enabling people to create support networks and build friendships in a safe and fun environment.

You will need to have access to a strong Wi-Fi connection at home.

About you

We are looking for an exceptional individual, with an understanding of memory loss, dementia and the needs of those affected with these conditions.

You will:

- Have an understanding of the needs of people with dementia and those who care for them and be able to demonstrate a person-centred approach.
- Need a broad understanding of statutory and voluntary services, and how they can support people affected by Dementia, be comfortable connecting with others and talking to a wide range of other health care professionals and related statutory and voluntary organisations.
- Have good communication and organisational skills, along with a sound knowledge of IT systems to record data, write support plans and communicate by email. 
- Possess the ability to travel independently around Derby City, be a team player, supporting colleagues across Derby, and be able to ask for help yourself when needed.
