+ £150 weekend on call retainer

Responsibilities

As a Team Coordinator you will: 30% Hands Off (to include, but exclusive of)

• Deliver One-to-Ones for Support workers.
• Complete Return to Work interviews for Support Workers
• Chair some Micro-Team Meetings where required.
• Carry out observations of social care practice, including medication assessments.
• Take a lead role in Quality Assurance audits and other management reporting duties where required.
• Contribute to the writing on care plans, including assessment and risk management plans.
• Support On Call once a month by being available to cover last minute Support Worker absence for which you will be paid £150 retainer plus your hourly rate for any support delivered. (On call start at 5pm on a Fri to 9am the following Mon)
• Support roster writing and making amendments to roster where required.
• Work 70% of your time providing direct support with 30% being management duties.
• Be open to direction from your Locality Manager.
• Build and maintain professional working relationship with your teams and colleagues.

Essential requirements include:

• Hole a Full driving licence. Have access to a reliable car with business insurance.

Duties Include:

• Deliver services based on the expressed wishes, cultural needs and support plans of those who use our services.
• Assist and encourage people to make choices and life decisions in line with the Mental capacity Act.
• Facilitate the accessing of local social and community facilities.
• Assist and encourage people to engage in everyday home, social, leisure and work activities based on their personal choices.
• Support people to manage their own homes, including supporting people to manage their own tenancies and providing safe support and guidance where necessary.
• Provide direct personal care and support in line with the individuals support plan which may include toileting and bathing if required.

• Support those who use our services to take prescribed medication as directed by appropriately qualified and trained health professionals

• Contribute toward You First’s Life Design assessment process and the development of Person-Centred Support Plans in conjunction with team colleagues, the person we support, their family and other key professionals.
• Assist and advise those who use our services with basic personal finance management and local shopping activities as directed and when required.
• To support team colleagues and respond to the immediate health and well-being needs of those who use our services by ensuring timely support and professional assistance is obtained from team colleagues and others, when required.
• Day to day contact with a range of health and social care professionals and other agencies. To pass and receive information relating to those who use our services. To plan and contribute toward and attend meetings as a member of the care support team.
• To take responsibility and joint ownership for own personal and professional development. To access training as and when required and to contribute toward effective and innovative care.
• Act as a role model and mentor to new staff. Undertakes wake in nights and sleep-in as and when required under the direction of the management team.

South West Community Chaplaincy (SWCC) aims to provide effective care and community support for people as they leave prison and re-integrate back into the locality, within the counties of Cornwall, Devon and West Somerset.  This includes mentoring, practical support, advocacy and signposting to assist the integration of ex-prisoners into their communities, in collaboration with other professionals.

The role of the SALT Project Lead is to implement and develop the charity's strategy of involving people with lived experience of the criminal justice in the design and delivery of its services.  This will include setting up a network, in which 'graduates' of mentoring support can continue to receive light touch support and be introduced to opportunities that will enable them to become more socially included, and to feel a real sense of belonging in their community. Another facet of the post will be facilitating and overseeing the 'Lived Experience Advisory Forum (LEAF)'. The role will include some direct community chaplaincy mentoring provision; giving practical and effective support to people with convictions to assist in their rehabilition and resettlement.

We are seeking an experienced and dynamic individual to join the BFYC team to support young carers in West Oxfordshire.

Working closely with the youth work team, you will help to provide support and advice to young carers by devising, developing, implementing and managing a programme of social activities for young carers (this could include trips, training, emotional 121 support and other activities).

Job Description - Key Responsibilities

• Provide one-to-one support for Young Carers, liaising with other agencies to help ensure the best outcomes for Young Carers;
• Establish relationships with schools across West Oxfordshire offering them support in how they identify and respond to the needs of young carers in their classrooms.
• Assist schools in launching Young Carers Groups and introducing Young Carer Leads in their settings.
• Contribute towards awareness raising events and activities to increase the identification and support of Young Carers, including liaison with Schools and community group;
• Liaise and work with colleagues to empower Young Carers and their families;
• Play a role in the assessment of Young Carers as part of the Common Assessment Framework (CAF), Team around the Child (TAC), Young Carers assessments, and assessments carried out by other professionals;
• Develop the active participation of Young Carers, enabling their voice to be heard;
• Support Young Carers and their families to access appropriate support services;
• Record all casework contact information and ensure that all appropriate consent forms, assessments, Baseline Forms, and other required information and administration is completed and recorded for all Young Carers;
• Ensure all tasks, procedures, and practices comply with Be Free Young Carers’ policies, including safeguarding and child protection, health and safety, confidentiality and equal opportunities;
• Assist in the production of newsletters and other communications to Young Carers and their families;
• Keep abreast of, and follow at all times, national and local policy in relation to safeguarding children and vulnerable adults;
• Monitor and evaluate work, producing records and reports as required;
• Contribute to a safe, positive and friendly working atmosphere;
• Attend training and other meetings as required;
• Provide reports and information to Trustees and other reporting as required;
• Assist in promoting and publicising Be Free Young Carers;

As Public Engagement Officer, you’ll play an important role making this vision a reality by engaging with churches through regional & national speaking engagements, networks, events, and new fundraising products with a view to help them raise funds for All We Can/Y Care to contribute to the overall fundraising target of the Acquisition Team. This role is 12-month fixed contract with potential to extend at the end of that period depending on the performance of the role and organisational circumstances at the time.

In this role you will:

• Seek and fulfil a variety of engagements in the London and the South East which engender loyalty and acquire short- and long-term income contributing to the fundraising target of £405k per year.
• Maximise income from Churches and Events
• Participate in All We Can’s presence at several conferences and events each year, inspiring current and new supporters through stalls, workshops, and stage time. 
• Build and develop new strategic relationships with ministers, lay workers, and key volunteers in Methodist Churches across the country, encouraging them into deeper income-generating engagement with All We Can.
• Collaborate with others in the Public Engagement Team to develop new resources and fundraising products to engage supporters and generate income

To be successful in this role, you will:

• Have experience of community fundraising involving relationship-building, planning, and generating a response to a call to action with experience of community fundraising in a Christian context being desirable. 
• Be a passionate and convincing public speaker, with experience of speaking/presenting in a Christian context; being a qualified preacher and/or worship leader in a church tradition would be an advantage but is not a requirement. 
• Have experience of generating and following up on new opportunities. Experience of project management in a professional context will be an advantage but is not a requirement.
• Have excellent oral and written communication skills and proven ability to communicate both stories and data.
• Excellent interpersonal skills, including an ability and willingness to communicate to diverse audiences. To be comfortable communicating to Christian groups and individuals.

MAIN PURPOSES OF THE JOB

The Stella Maris (SM) Regional Port Chaplain will promote the vision, mission and values of SM in caring for all seafarers and fishers by helping to meet their pastoral and spiritual needs. They will recruit, train and support SM staff and volunteers for the port and will maintain good relations with local parishes and the local diocese. They will work ecumenically with members of other Christian maritime societies with whom SM works.

1. To meet the welfare and pastoral needs of all seafarers and fishers equally according to their needs and wishes. To be an advocate on their behalf, upholding the dignity of each. Through regular and frequent ship visiting, to understand the working and living conditions of the seafarer and fishers and be able to work for justice and for the well-being of all crew members without distinction.

2. To address the communication needs of seafarers, providing transport, news, and access to telephone and internet facilities.

3. To address the welfare needs of seafarers and fishers, including support with money transfers, hospitality, and crisis support, e.g. abandonment, injury and death, detainment, bereavement, advice and access to medical care.

4. To meet the ecclesial, spiritual, and sacramental needs of Catholic seafarers and fishers; to facilitate the religious and spiritual needs of others and to be a pastor to all, sharing one’s faith as appropriate.

5. To participate in SM conferences, ongoing formation and other professional courses as required.

6. To supporting seafarer centre functioning.

7. To recruit, train, develop and retain a SM ship visiting volunteering team.

8.  To represent SM on the local Port Welfare Committee(s), to include attending such meetings and submitting reports.

9. To liaise and maintain good working relationships with all port stakeholders, including the Maritime and Coastguard Agency, the International Transport Workers’ Federation, port health, harbour masters, dockers, security, and shipping agents.

10.  To work in an ecumenical and multi-faith context by respecting the differing religious and spiritual needs of seafarers drawn from diverse regions throughout the world.

11.  To encourage and support the development of an ecumenical service to seafarers through engagement with our ecumenical colleagues, including the Mission to Seafarers, The Fishermen’s Mission, The Sailors’ Society (to name a few).

12.  To engage with local maritime colleges.

13.  To engage with the local press, in particular the local Catholic diocesan press.

14.  To represent and be a proactive advocate for SM at Deanery meetings, in local parishes, in schools, with diocesan Justice and Peace networks, with migrant communities, in the local Catholic community and in the local community as a whole.

15.  To support the Fundraising and Communications team with the development of local fundraising initiatives.

16.  To promote SM internally and externally so as to ensure a highly motivated workforce and a positive external image.

Home based role and living within the relevant region or close enough proximity to be able to travel and meet the requirements of this role’ (subject to meeting homeworking assessment requirements, which includes a minimum broadband speed of 18Mbps) 

In this key role you will be responsible for assisting the Senior Negotiating Officer to manage and support our stewards, safety and equality representative networks across West Midlands and part of Cheshire, advising them in providing individual and collective industrial relations support to members, working both in and outside of the NHS.

You will work in collaboration with other health trade unions across the region.

You will work with the CSP regional team to recruit and organise members, influence on local workforce issues and promote physiotherapy.

You will provide representation for members at disciplinary hearings, grievances and disputes with employers, and provide general advice and information to representatives and members on issues such as pay, terms and conditions, and employment legislation.

With significant trade union experience at a senior level, and an understanding of NHS structures and government policy on health, you will have excellent communication, negotiation, training and presentational skills, combined with a strong collaborative approach and a thorough understanding of, and commitment to, equality and diversity principles and the ability to put them into practice.

Home based role and living within the relevant region or close enough proximity to be able to travel and meet the requirements of this role’ (subject to meeting homeworking assessment requirements, which includes a minimum broadband speed of 18Mbps)

You will undertake work with the Wales Government, working in collaboration with other health trade unions.

In this key senior role, you will be responsible for managing and supporting our stewards and safety representative network across Wales, advising them in providing individual and collective industrial relations support to members, working both in and outside of the NHS.

You will represent the CSP on the Wales Partnership Forum, as well as working with the CSP regional team to influence on local workforce issues and promote physiotherapy.

You will provide representation for members at disciplinary hearings, grievances and disputes with employers, and provide general advice and information to representatives and members on issues such as pay, terms and conditions, and employment legislation.

With significant trade union experience at a senior level, and an understanding of NHS structures and government policy on health, you will have excellent communication, negotiation, training and presentational skills, combined with a strong collaborative approach and a thorough understanding of, and commitment to, equality and diversity principles and the ability to put them into practice.

Care Coordinator

Position: Care Coordinator
Location: Leeds Office (Beeston)
Job type: Permanent, Full-time 9-5pm

About the role:

You will plan and organise care, establishing effective rotas for well-trained care teams, minimising travel, and maximising care continuity, enabling care colleagues to spend as much time as possible with the people they support. You will be a skilled communicator, sharing important information, ensuring people receive the care they need.

Your key responsibilities will include:

Coordination of Care

• Create consistent runs and rotas, matching customers, and colleagues to maintain high levels of care continuity.
• Ensure care calls are covered in advance, to minimise the impact of unplanned colleague absences.
• Work with the Trainer to schedule training and shadowing for care colleagues, providing cover for their care calls or negotiating additional time for training compliance.

Maintaining accurate records and information for:

• Care calls, ensuring they are scheduled to meet the needs, wishes and preferences of the people we support.
• Colleague records, their availability, absences, location and travel, and other important information to ensure you know who is available to work each day.
• Contact logs and other records of communication on the care system.

Care Quality and Customer Satisfaction

• Communicate with the people we support, their families and care colleagues regarding new care services and changes to care services.
• Establish and maintain professional relationships with the people we support, their families, colleagues, and external partners/services.
• Communicate in a timely manner, responding to concerns and issues - intervening early, resolving issues before they escalate, keeping colleagues informed and requesting support when needed.

About you:

We are looking for an exceptional and credible candidate with demonstrable knowledge, experience, and skills related to:

• Coordinating and scheduling care calls, runs and rotas. Creating efficient runs, minimising travel to enable care colleagues to spend more time with the people we support.
• Prioritising your workload, working under time pressures, keeping calm and focusing on the task at hand, being proactive pro-active to prevent concerns and complaints.
• Compassionate communication with the people we support and their families, care colleagues, GPs, District Nurses, Social Workers, and others.
• Collaboration with colleagues, using information and data to help inform your understanding, discussions with others, and decision-making - being responsive to complaints from the people we support and their families).
