About the role:

We looking to appoint a Family Support Worker to deliver a high-quality family support service as part of our North East Care Team.

Reporting to the Family Support Manager out of our North East Care team base and working in partnership with health, education and social care professionals, you will take responsibility for providing needs-led emotional, social and practical support to families where a child/young person has a life threatening or terminal illness.

Having worked in a stressful and emotional environment you have a genuine interest in building supportive relationships and helping people; and having provided bereavement support to families, you understand the processes of grief, loss and change - and how best to help others deal with its impact

· An experienced child health, education or social care professional - applications will be particularly welcome from those who have worked in a community environment and those with a recognised qualification in education, health or social care

· Practiced in child protection, information sharing and the rules around data protection - you lead by example, drawing on your own professional experience and working within established guidelines

What does the Job Role involve?

As a project worker (PW1), your duties will include:

• Utilising effective communication skills to engage young people and families to build collaborative relationships.

• Supporting young people and families to identify strengths and prioritise support needs.
• Planning, facilitating and evaluating groupwork programmes and one to one support.
• Maintaining accurate recordings.
• Celebrating achievements / successes.
• Establishing, developing and maintaining productive working relationships with colleagues and other agencies.

You will have: 

• Experience of working with young people and their families.

• Experience of undertaking assessments with young people and families to co-produce holistic support plans.
• Experience of planning, facilitating and evaluating group programmes.

The Norfolk and Waveney Autism /ADHD Support Service are delighted to be hiring a Family Support Worker to join our supportive and successful team.  Our service is commissioned by Norfolk and Waveney ICB to provide support and advice, resources and signposting to parents/carers of children or young people awaiting assessment for Autism or ADHD. 

We are dedicated, committed, compassionate and extremely proud of our achievements. We have already offered support to thousands of parent/carers with children / young people on the Neurodevelopmental Assessment Pathway.

Your impact – For families awaiting assessment for Autism / ADHD in the area, to offer individual or virtual (via zoom or teams) telephone support to include giving information, advice and guidance, signposting, referring to other agencies as appropriate, attending relevant family support meetings.  This may also include occasional home visits.

Facilitating support groups for families awaiting assessment for ASD/ADHD in the area. Co-production of parent-led groups and parent forums, co-facilitating courses for parents/carers such as the 3 week Plan Bee course.

Working with the co-ordinator to develop relationships with schools, pre-schools, children’s health services and children’s social services and, in particular, NCHC Neurodevelopmental Teams to provide co-ordinated support to families.

Your skills - To join us as Family Support Worker you will have experience of working or volunteering with families in the community.  A relevant qualification in child and family work is desirable.  You will have excellent verbal and written communication skills and experience in using IT to deliver support, record casework, write reports and input data.

You’ll be an effective communicator who excels at building trusting and empowering relationships with individuals. You will be compassionate and empathic, with a good understanding of child development and in particular of Autism and ADHD and the impact this can have on children and their families.  Experience of delivering evidence-based programmes for parents and/or of facilitating group work.  Committed and willing to promote inclusion and equality of opportunity for all.

We have an exciting opportunity here at Barnardo's for experienced, enthusiastic and assertive Family Support Workers to join a brand-new team who will deliver the In Reach Hospital Emergency Department Family Support Programme.

The role is full time , 37 hours per week, permanent fixed term funded until March 2025 and will be based at the North Middlesex University Hospital, The entrance to the A&E department is in Bridport Road, Edmonton, London, N18 1QX.

Starting Salary Range - £28,565 - £30,607

Successful candidates will work in partnership with the clinical team to facilitate discharge of children aged 0-12 years from the Emergency Department where the clinical presentation is not acute and can be managed at home by local services.  

“This role requires workers to have experience of holistic ways of working with families and young people. Including assessment, support, signposting and making referrals to appropriate services where needed. The successful applicant will be able to work as part of a team as well as on their own initiative and manage the pressures of working in both a busy emergency department and the community”

Some but not all of the key responsibilities are:

• Initial face-to face conversation or triage call to identify presenting issues and the level of intervention required.
• Delivering support flexibly and tailored to the needs of the family unit including telephone, virtual and face-to-face support and home visits where necessary. Contact with families could be a one-off or over weekly sessions based on level of need targeted. 
• Signposting to partner agencies, based on needs identified.
• Supporting families to navigate their way through referral pathways and enable them to access universal & targeted services.
• Where required advocating on behalf of families, supporting parents and carers to communicate with professional/Services.
• Supporting families to access other local services.

Ideal Candidates must have's

• Experience of working with families with complex needs and the ability to build and maintain rapport through effective communication and listening skills, using a whole family approach.
• Ability to be clear and realistic about what support the service and you can offer to families.
• Ability to maintain a visible presence and sustain relationships with clinical staff, being mindful of the various working patterns/ professionals within the department.  
• Ability to navigate a high pressure, fast paced acute healthcare setting. 
• Competent IT skills to record and maintain accurate data. 
• A basic understanding of Public Health, health inequalities and wider determinants that underpin health and wellbeing. 
• Knowledge of safeguarding and promoting the welfare of children
• Relevant Level 3 Qualification (Health and/or Social Care)

More about the Programme

 

The In Reach Hospital Emergency Department Family Support Programme model offers a robust way of supporting often vulnerable families to uncover and address unmet need. Through identifying the underlying factors causing the clinical presentation and attendance, and addressing holistic needs of the family unit, we hope activity to unscheduled, primary and emergency care is dramatically and sustainably reduced.

The model is based on Family Support Workers based within Children's Emergency Departments, offering holistic, family-oriented support in a flexible manner either in-person, telephone- based and home visits to be able to address the underlying causes of repeated / avoidable ED attendances.

*Although this contract has a permanent status, please be aware that this post is subject to funding currently to March 2025 and therefore should this funding not be extended further, you may be subject to a redundancy consultation or a TUPE arrangement.

** Please note the hours for this role will be required over a 5-day week and may be done in longer day shifts or with set times i.e. 8am to 4pm (to suit Emergency Department and community/family needs). The role can adopt flexible working arrangements to meet service and personal need but will NOT provide 24 hour provision in the ED settings. This can be discussed further at interview.

This is an exciting opportunity to join TACT London & The South in a new role as Family Support & Community Engagement Worker and be part of an exciting project embedding community support, parent and child activities and deepen engagement across the region.  The Family Support & Community Engagement Worker will be leading on the establishment of community hubs for foster carers where we can provide sustained local assistance,  encourage peer to peer support,  host family activities, training and identify other interventions that will consistently enhance the skills, knowledge and engagement of our foster carers.

This is an exciting opportunity to join TACT East Midlands in a new role as a Family Support & Community Engagement Worker and be part of an exciting project which aims to promote community engagement for TACT as a fostering agency, through developing links with local healthcare providers, religious leaders, education establishments and leisure groups in the Nottinghamshire/Derbyshire area, to develop better awareness and visibility of TACT as a fostering agency and promote our foster carer recruitment in the area. 

In addition, the Family Support & Community Engagement Worker will work alongside other professionals, in developing links and relationships with the foster families in the Nottinghamshire/Derbyshire area to identify and respond to their support needs, and will be leading on the establishment of a TACT Carer Support Hub which will support carers who look after children with complex needs with stay over breaks.

Overall Duties of the Family Support & Community Engagement Worker will include:

• To support the foster carer recruitment in the Nottinghamshire/Derbyshire area to develop a ‘TACT Carer Support Hub’ that will bring foster carers together to support each other and provide more family support and breaks from caring.
• To engage foster carers to raise TACT’s profile in the community and recruit more carers in the locality.
• To support carers in the Hub who look after children with complex needs so that children can benefit from stable family life.
• To offer practical and emotional support to TACT foster families, including trauma-informed and solution-focused interventions and strategies which will involve supporting foster families to strengthen and develop their skills and knowledge relevant to supporting children with complex needs.

A fantastic opportunity has arisen to join our highly successful, passionate, and driven Family Support Hub Team.

As a Family Support Hub Officer, you will work within our Family Support Hub, liaising with identified partners to provide enhanced support to families caring for a child with a life limited illness. In addition, you will support those families contacting Together for Short Lives through the Helpline, website and from partner referrals. The purpose of this role is to help Together for Short Lives reach and support more families - responding appropriately to their needs by listening, signposting or referring onto internal and external support eg grants, peer support, events and engagement opportunities.

The Family Support Hub Officer will help us to deliver our programme of work for children’s health and social care services to improve and expand the range of support that they can offer to families.

We are looking for an experienced young people's practitioner. You will be confident in your work with young people and join our dynamic, ambitious team.

CHHAT provides a programme of targeted support services to identified, young people/young carers. The CHHAT practitioner will provide support to, and improve the physical, mental and emotional wellbeing of young carers and their family network. You will address a number of target areas for children/young people who are caring for/affected by parental/familial substance misuse and mental health.

Our engagement with young carers covers early to high intervention needs. We also support cases of historical parental substance use, where the young carer may, or may not still be residing with their parent, but continues to be affected by the experience.

In order to be successful with this role, you will need to be able to demonstrate that you have extensive experience of: -

Delivering direct interventions with vulnerable young people (8-25) on both a one to one and group work capacity. As well as working to support the family network.

Co-producing care and safety plans with young people that are young people led, focusing on both a relational & strength-based approach.

Extensive partnership working in a multi-agency environment to ensure that the needs and wishes of the young person are heard and acted upon.

You will work as part of a team and need to be able to work in a flexible way which includes flexing their day and work location as need indicates, including being based in the locality office in Chelmsford and working in the home environment to increase convenience and accessibility

You will need to demonstrate that you have:

-A relevant professional qualification working with vulnerable children and families is desired e.g., Diploma/degree in social work, counselling qualification, professional youth worker or related NVQ or degree. Or Substantial relevant experience of working with vulnerable young people facing significant challenges within this sector.

-Substantial experience of working with vulnerable young people and families that are hard to reach and from diverse backgrounds on both a one-to-one basis and through group work interventions.

-Experience of supporting adolescents who are dealing with complex and sensitive issues through offering advice and advocacy.

-Experience of partnership and multi-agency working.

-Experience of working on one's own and within a team.

-Experience of using IT based case management systems.

Tower Hamlets Domestic Abuse Service is a refuge that provides short-term accommodation for 15 women and their children with a range of support needs, who are at risk of or fleeing domestic abuse.

You will be responsible for overseeing and coordinating families' move-on pathway from the refuge into independent living. The post holder will be responsible for sourcing accommodation in the private rented sector - liaising with landlords and sourcing new move on routes for families. You will directly support customers to move into their new accommodation and ensure support staff are fully equipped to inform customers about the move on process. The post-holder will also a central contact point for local authority housing departments.

All applicants must be legally eligible to work in the UK by the start of employment as Look Ahead are not able to offer sponsorship.

What you'll do:

Develop links with private sector landlords and estate agents in order to source private rented stock and promote Look Ahead as a partner organisation
Work in partnership with local authorities' move on co-ordinators, creating innovative schemes to support long stayers in accommodation
Attend and support customers to complete all relevant documentation for housing placement meetings
Develop and maintain an information resource to support the team in the resettlement of customers and their children. This will include (but not limited to) Local Housing Allowance Rates, Partner Landlord and Estate Agency details and Rent Deposit Schemes
Act as the main point of contact for information and resources on the private rented sector
Support a caseload of families to ensure they move on within their licence agreement timeline
