Age UK Lambeth’s vision is to work closely with residents, closely to where they live, remain relevant and to respond locally to rising needs. Hospitals need to free up beds by discharging patients who do not need to remain in hospital. Despite the pressures we want discharges to be safe, successful and as painless as possible for patients. We also want to ensure a patient is not readmitted into hospital.
The Hospital Discharge Community Support Coordinator will match Age UK Lambeth (AUKL) volunteers and partner organisation volunteers to support a patient's discharge. Volunteers could provide support with transport, practical support such as food shopping or completing home safety checks. Upon discharge volunteers may continue supporting patients for up to 4 weeks, providing support in person (at the individual's home or in the community) and over the phone, linking them into support services and helping them access the community to prevent hospital readmission.
What you’ll be doing?

Oversee the development of a new, inclusive volunteer programme that offers a range of diverse and challenging volunteer roles to support patients leaving hospital and once settled home.

Work in partnership with the VCS to create 100 volunteering opportunities to support hospital discharge.

Using the volunteer platform, Care for Me, track allocated tasks to ensure they’re completed. Monitor volunteer reports to review the support and work delivered under the project.

Develop and maintain strong working relationships with King’s College Hospital and Guy’s and St Thomas’ Hospital to ensure appropriate referrals are made into the service.

Sensory Services by Sight for Surrey has been established for over 100 years. We enable and empower people who are Deaf, hard of hearing, blind, partially sighted, and deafblind to overcome barriers to be as independent as possible and thrive in their communities. We are a countywide charity based in Fetcham, near Leatherhead.
We are seeking a hard-working, driven and flexible individual to develop, lead and grow our volunteer and community engagement team and lead and collaborate with our marketing and communication team to raise awareness and increase engagement to grow our vital work.
Main duties include:
¨ Be an effective manager, line managing our Volunteer, Community Outreach and Marketing and Communications teams.
¨ Working with various partner organisations, building relations while exploring possible funding/joint working/event opportunities, and supporting our Outreach Team to make sure they are best placed to have the biggest impact for the people we enable and empower.
¨ In collaboration with the Marketing and Communications team, oversee the production of high quality communications to raise awareness, increase engagement and grow support for our vital work.
¨ Organise and facilitate our Breaking Down Barriers Workshops three times a year.
¨ Lead on key projects around Communications, Volunteering and Engagement, including:
- Co-produce a strategic volunteering plan for the team aligned to our strategy
- Review and develop the charity policies for volunteering and ensure volunteers are up to date with our relevant policies, legislative requirements and our needs and requirements
- Co-produce a strategic communications plan for the team, aligned to our strategy
- Review all current communications and revise annual marketing plans to strengthen our presence in the community
¨ Working with colleagues in other teams and departments, develop a strategic Community Engagement plan aligned to our strategy
¨ Ability to work flexibly according to the business need which may include evening and weekends
Skills and Qualifications:
- Direct experience of line managing a team of staff
- Relevant experience in the management of a volunteering programme, or extensive experience of directly managing volunteers with the ability to step up into this role
- Knowledge and understanding of volunteering best practice, strategy and policy
- Demonstrable experience of community engagement and building relationships with partner organisations
- Planning and organisational skills, including ability to plan, prioritise and deliver a complex workload
- Strong presentation skills, sufficient to deliver engaging training to staff and volunteers
- Strong communication skills – verbal and written, and ability to relate well to people from all backgrounds
- Experience in MS Office and confident in using a database
- Understanding of data protection requirements and confidentiality
- Able to champion the role and contribution of volunteers
- Ability to engage and influence colleagues
Competencies Required:

Think ahead and develop, lead and grow a busy programme of recruitment and encouragement of volunteers to meet the needs of the charity. Develop innovative approaches to recruitment and service delivery and make recommendations and suggestions to senior managers. Implement a communications plan to improve and increase our visibility and awareness.

Create new roles and tasks for volunteers working with colleagues across the charity. Increase the potential of the Outreach team as they build relationships across the county. Develop imaginative and effective ways to raise our profile and reach new audiences. Value and motivate volunteers to help them provide quality services and support for the charity.

Plan resources effectively and manage any crises effectively within laid down policies. Demonstrate a consultative and informed approach to problem solving, using patience and understanding. Liaise with charity colleagues and senior managers, and externally with other groups.
4. Record keeping
The people we support are often vulnerable so it is essential that all volunteers have an appropriate Disclosure and Barring clearance, and that our records of all other relevant paperwork such as driving licence details and insurance is kept up to date and properly recorded.

The post holder must be open to new ways of working, adapt well to change and be able to feed back the impact of changes to volunteers, their team and to senior managers.

Who are we looking for?
Ideal candidates will have a successful track record of meeting income targets. You will have good knowledge of budgeting and financial control and an interest in researching and preparing imaginative funding packages. You will have previous experience in researching potential Trust and Foundations to approach as well directly applying for funds and writing proposals. Experience of planning, analysis and understanding monitoring and evaluation is also essential for this role. You will be well organised, self-motivated and comfortable using Microsoft packages and be familiar with the use of databases and spreadsheets. Experience with databases such as Donorfy would be a bonus however it is not essential for the role. Your approach to work will be collegiate and you will work well within a team environment as well as taking your own initiative. This is a great opportunity to further develop your fundraising skills in a supportive environment.

About the role:
This is an exciting opportunity to join a busy corporate partnerships team. The postholder will work closely with the Corporate Partnerships Manager and wider Charity team to deliver the corporate strategy.
Areas of support include management of incoming enquiries, team support for events, administration tasks for the team and supporting corporate partners.

Some management of low value corporate partnerships as agreed with the Corporate Partnerships Manager.

Assist with the arrangements to ensure that all donors receive outstanding stewardship to establish long term commitment to the Charity.

*The charity is currently conducting a four-day working week trial, due to end on 31st March 2025. This means that, for example, staff previously working 37.5 hours a week are now working 30 hours a week to enable a four-day working week. We are confident that by embracing a more flexible and balanced approach to work, we can continue to create a thriving and fulfilling work environment while driving growth and success for our charity. Next steps will be agreed before the 31st March 2025, based on an assessment of the trial period; the four-day working week trial may be ended, extended or adopted permanently.
This fixed term contract is for 14 months at 28.13 hours per week. As part of our 4-day working week trial (working week Monday to Thursday) working hours required will be 22.5 hours per week if you opt to sign up to the trial and its associated conditions. If the trial was to end after 31st March 2025, the working hours required would return to the contracted weekly hours for this role of 28.13 hours per week. As part of our staff charter, we have a flexible working policy and can consider accommodating a flexible working pattern such as school working hours if requested.

As our Strategic Evidence Assistant, you will support the Strategic Evidence team in gathering, analysing, contributing, and mobilising evidence, data, and insight to help build and curate a strategic evidence base that is built on the most recent and best available dementia evidence. You will also be part of an even larger Evidence, Policy and Influencing function, which is laser-focused on ending the devastation of dementia through changing policy. About this key role
Our successful candidate will provide important administrative support to one individual Caseworker, including organising their hand-written notes, helping enter them accurately into different platforms and databases, plus ensuring all data inputted is done so in a timely and accurate manner. The Development and Alumni Relations Office (DARO) exists to support The University of Birmingham's academic and student community by engaging, inspiring, and celebrating alumni, individuals, and charitable funders who give their money, time, and networks to support the University’s strategic priorities. The Office, which is comprised of five teams, is focused on fundraising and volunteering from alumni, organisations and individuals who are passionate about changing lives, through funding various research projects, supporting student bursaries, mentoring students, and providing internships, as well as providing a versatile programme of engagement opportunities for our global alumni community.
This new role in DARO will lead on providing insight and intelligence utilising reporting tools to guide, influence and drive strategic and operational activity. Long-term, the role will lead on the development, delivery and maintenance of a suite of live reports (using PowerBI). In the short term, the role will produce regular static reports on key KPIs and metrics whilst working with the Head of CRM to develop the capabilities of CRM and related integrated systems in readiness for BI reporting. The role will also be responsible for developing skills across DARO to utilise the functionality of PowerBI whilst building an understanding of the importance of data as an asset through analytics and reporting. Alongside this, the role will provide direct support to stakeholder teams through the provision of complex queries and data support to enable key DARO activity such as fundraising campaigns and mailings. This role will therefore require a robust combination of adaptable technical proficiencies, data storytelling techniques, and relationship building and communication skills.

About us:
Hospice at Home’s core service is its skilled team of Registered Nurses and Healthcare Assistants who provide exceptional care and support in people’s last year of life and at end of life, also providing additional services such as Lymphoedema, Bereavement and Family Support, Befriending, Occupational Therapy and Complementary Therapy.
We provide individualised care and support to improve quality of life and our end of life care helps facilitate a peaceful and dignified death in the home and support to those who are bereaved.
Although we receive some funding from the NHS, we need to raise over 85% of our running costs in order to ensure that this vital help is available when needed and remains free of charge to patients and their families. Our Fundraising team plays a huge part in raising these vital funds.
Our clinical service area covers a mixture of urban and rural communities, a region of approximately 1,500 square miles throughout North and East Cumbria.
