Job description

Age UK Lewisham and Southwark (AUKLS) is a local, independent, self-funded charity that exists to improve the lives of older people in Lewisham and Southwark. As part of the social prescribing team, you will work in partnership with community organisations, health and social care and statutory services to provide a social prescribing service to people aged 50 and over, and/or self-identifying as a carer of an older person living in the London Borough of Southwark. The role will be based at our Healthy Living and Learning Centre, as well as working from our Stones End Day Centre, from partners' sites across the borough, in clients' homes nad in other venues that clients' might choose.

We are looking for a passionate person who enjoys a varied working day and is passionate about supporting older people to maintain their wellbeing and independence.
