As a Caseworker, you’ll work directly with unpaid carers, providing a vital mix of practical advice, emotional support, and advocacy. From conducting statutory carer assessments to organising support groups and events, your work will ensure that carers feel recognised, valued, and supported in all aspects of their role.
 
You will engage with carers via various channels - phone, email, text, face-to-face, and group settings - ensuring inclusivity and accessibility. This is a multi-faceted role that blends frontline casework with community engagement and strategic development.
 
Main responsibilities of the role include:
 
Direct Support to Carers: 

• Deliver comprehensive carer assessments in line with the Care Act 2014, and co-produce tailored support plans
• Identify and assess eligibility for Carers Personal Budgets and assist with the application process
• Provide accurate and empathetic information, advice, and guidance on carers’ rights, benefits (e.g. PIP, Carer’s Allowance, Attendance Allowance), housing, and local services
• Act as a listening ear and a source of emotional reassurance for carers navigating challenges and crises
• Liaise with Local Authorities, NHS providers, and voluntary agencies to advocate for carers and coordinate support
• Make appropriate referrals and ensure follow-ups are completed

Identification and Outreach: 

• Lead proactive outreach efforts to identify new carers, particularly those from underrepresented or vulnerable communities
• Build and nurture collaborative relationships with GPs, hospitals, primary care teams, and community organisations
• Raise awareness of unpaid carers and the charity’s services through campaigns and community presence
• Contribute to the development of internal strategies and external policies that reflect the needs and voices of carers

Administration & Case Management:

• Maintain high-quality records using internal systems (Charity Log) and the Mosaic Local Authority database
• Register new carers and ensure timely updates to service user records
• Prepare case summaries and contribute to internal and external reports for management, commissioners, and funders
• Ensure data compliance, confidentiality, and GDPR adherence at all times

Team & Organisational Development:

• Act as a lead on specific areas of knowledge (e.g., benefits, outreach) and share expertise across the team
• Attend regular team meetings, supervision, and 1:1s, contributing to a positive and solution-focused culture
• Participate in ongoing training and staff development opportunities

The Successful Candidate:

Experience of working with individuals from diverse backgrounds and with varying levels of need
Managing a personal caseload, including detailed case note keeping
Providing advice and support in a frontline or community setting
Exposure to the health and social care sector (voluntary experience welcomed)
Background working within mental health/supportive settings working with vulnerable people

We are looking for an experienced Social Worker to join our London & South East Team supporting children and young people diagnosed with cancer.

You will be part of a close-knit Young Lives vs Cancer social work team, working with an established NHS multi-disciplinary team and services in the community.

This role is part home, part site-based. Your contractual base will be both Home and Hospital.

Background to the role

We have teams embedded in four hospitals across East London where they are recognised for the warm, responsive and kind person-centred service they provide for patients. They work in close partnership with ward managers, discharge teams and social workers to ensure the safe and timely discharge of patients, connecting them back to their local communities and ensuring that their chances of a sustained and speedy recovery are maximised. Our model has proven success in reducing re-admissions.

Over the past 12 months we have worked to reorganise how our Home and Settle team works.  We have done this to reduce the pressure on individual staff members, increase the extent to which the teams work together and improve the consistency and sustainability of the services. The Discharge and Settle Worker role is key to this new model of delivery. 

The postholder will be working in the community and in patients homes to help them settle back into life and to connect with the support and services that will help them avoid another hospital visit as far as possible. Maintaining good relationships with the rest of Age UK East London’s Home and Settle team and will ensure the smooth running of the service and the best possible outcomes for patients.

Job description

Job Purpose

The home and settle service works collaboratively with hospitals to support people when they are discharged from hospital to return home. You will provide temporary practical and emotional support to service users within their home and make sure they understand the care and treatment choices available to them. The service is time limited, usually up to 6 weeks, to adjust and settle back in their homes. 

Key Tasks

• Assess the needs of service users in hospital before discharge, and in their homes, and understand what barriers and enablers there might be to moving home.
• Aid the discharge process and improve patient experience through tasks such as key cutting and co-ordinating with colleagues for the setup of the home environment. 
• Provide (and/or arrange) the practical and emotional support needed to enable service users to move home and/or remain safely at home. 
• Be vigilant and make referrals/signpost for additional needs that our wider team, or partners can support. E.g. for benefit checks, energy advice, carers support, befriending.  
• Help maximise service user independence by adopting an enabling approach. 
• Develop a listening and caring relationship with the service user and their families/carers. 
• Provide domestic practical support within the home e.g. light cleaning, washing, ironing (service user clothing only) etc. 
• Prepare and service drinks and simple meals with or for the service user customer ensuring nutritional needs are met in accordance with individual support plan. 
• Accompany service user outside the home e.g. hospital/GP appointments etc. 
• Support customers with other tasks e.g. making telephone calls on their behalf, reading and responding to correspondence, completing simple forms (training will be provided) as directed by the customer etc. 
• Ensure services provided by AUKEL in the community are safe and person centred. 
• Report safeguarding concerns, in accordance with AUKEL safeguarding procedures. 
• Work within AUKEL’s expectations of professional boundaries and confidentiality. 
• Provide cover in the case of sickness and annual leave periods of colleagues. 
• Support service user reviews, ensuring all visiting information is uploaded on to AUKEL’s organisational case management system.

Administration

• Input all records, reviews, and visiting information to service user’s case notes in a timely manner to maintain up-to-date and accurate records on AUKEL’s case management system (currently Charity Log/Call-round App). 
• Report all “no access” failed visits, or changes in service users’ condition or circumstances in accordance with AUKEL’s policies and procedures. 
• Ensure service user comment sheets are completed.
• Support the return of Customer Comment sheets and general feedback in respect of your own service users. 

 Quality

• Ensure services provided in the community are delivered in line with the Care Quality Commission (CQC) principles. 
• Deliver all work in line with AUKEL quality mark standards. 
• Commit to undertaking the Care Certificate, if not already held or in possession of NVQ level 2. 
• Complete essential training as required by AUKEL to provide safe services. 

 Liaison

• Work in collaboration with other agencies providing support within the customer's home. 
• Work under the direction of the service manager and project officers. 
• Represent AUKEL and participate in appropriate external meetings and events to remain aware of local, regional, and national issues affecting quality and compliance issues affecting care and home support services. 

 General

• Meet regularly with your line manager for support, supervision, and appraisal.

Make a Change 

Make a Change is a community-wide, early response approach to people using abusive behaviours (or who are concerned that they might be) towards a current or ex-partner. The model has been developed by Respect in partnership with Women’s Aid Federation England, drawing inspiration from their Change that Lasts approach. It includes three strands: expert support programme for perpetrators with parallel support for survivors, workforce development and community outreach.

Improving the safety, freedom and wellbeing of adult and child survivors of domestic abuse is a key outcome for our work with perpetrators.  The Make a Change model is a multi-partner project offering local areas a framework for delivering perpetrator work. Where feasible, we aim to establish partnerships with local survivor domestic abuse service to deliver parallel support for survivors (referred to as Integrated Support Service) as part the expert support strand.

Purpose of the role: 

The Senior Communications and Policy Officer will work with the Make a Change team at Respect to advance the development of communication and policy activities for the Make a Change model in project sites and nationally to encourage uptake in new areas. They will also work with Respect’s Communications and Influence leads to ensure alignment with the organisation's broader messaging and advocacy efforts, maximising the impact of Make a Change initiatives. A waking Night Support Worker will help to provided support to the Young People in the service through the night. You will report to the team leader who will supervise your engagement and support of our Young People through the night. Partnership is fundamental to our way of working. We are second-tier organisation focusing on the continuous improvement of service models, sharing best practice and supporting specialist service providers to deliver. 

 We have three core strands of work: 

The Drive Project is our flagship intervention working with high-harm, high-risk and serial perpetrators of domestic abuse to prevent their abusive behaviour and protect victims. The Drive Project challenges perpetrators to change and works with partner agencies – like the police and social services – to disrupt abuse. It is currently being delivered in 9 police force areas. 

Restart is an innovative pilot project providing earlier intervention for families experiencing domestic abuse. It brings together domestic abuse services, children’s social care and housing teams to identify and respond to patterns of domestic abuse at an earlier stage. Restart is currently being delivered in five London Boroughs.  

The Drive National Systems Change programme works across the domestic abuse specialist sector, public sector partners and beyond to develop sustainable, national systems that respond to all perpetrators of domestic abuse.  We identify systemic gaps and build solutions that keep survivors safer by addressing those causing harm.

Background for the role

In April 2021 the Domestic Abuse Act received Royal Assent. The Act introduces a new civil Domestic Abuse Protection Notice (DAPN) to provide immediate protection following a domestic abuse incident, and a new civil Domestic Abuse Protection Order (DAPO) to provide flexible, longer-term protection for victims. DAPOs can impose both prohibitions and positive requirements on perpetrators. Positive requirements can be in the form of interventions aimed at reducing and managing risk, meeting the needs of an individual (for the factors that are not the causation of abuse but impact on risk e.g. mental ill health, substance misuse) and behaviour change interventions.

We were commissioned by the Home Office to design a triage model that will assess individuals for the suitability of these interventions, this triage model launched in November 2024 and will be tested and evaluated in order to prepare for national roll out in 2026.

 Purpose:   

The DAPO Service Manager will manage the operational, and strategic delivery of the DAPO team pilot working closely with the Practice and Development Lead and Programme Manager.

The postholder will have responsibility for managing all DAPO triage teams who are working locally and remotely in the DAPO pilot sites.

This role will require 

a) the effective line management of Triage Team Leaders (who in turn manage triage workers and IDVAs), in providing a high-quality frontline service triaging DAPO referrals for positive requirements

b) the development and maintenance of a multi-agency infra structure that actively engages with the triage team and the triage process 

c) working with the Practice and Development Lead and Programme Manager to ensure safe and effective delivery of the DAPO pilot triage process. 

d) support the development of the DAPO triage model through learning and analysis of the pilot delivery e.g. to initiate, develop, maintain and monitor multi-agency links through procedures and protocols, and to keep safety central to all services for perpetrators and victims of domestic abuse.

The Heaney Cluster an intensively supported housing scheme for the most challenging customers in the borough. The majority of the customers come with a history of entrenched rough sleeping; often coupled with enduring mental health difficulties. About the Role

We're looking for 2 kind, compassionate and resilient Specialist Support Workers to join our Mental Health service in Southwark.

£29,940.00 per annum, working 40 hours per week.

Specialist Support Workers within Look Ahead are required to have either an extra dimension of experience or a higher qualification level as determined relevant for the post.
The shift pattern for this role includes: Early 8-4pm and late 2pm-10pm - Monday to Friday and double or, 8am-3pm/3pm-10pm Saturday and Sunday. With some mid shifts. Working days also includes bank holidays.

What you'll do:
* Building supportive, trusting relationships with customers and creating a positive atmosphere
* Carrying out holistic assessments of new customers which incorporate relevant statutory referral information
* Conducting regular key work sessions that are innovative and engaging in order to achieve Support Plan goals
* Key-working the most complex service-users with needs aligned with Drug and Alcohol dependency
* Creating support plans for providing appropriate services based on on-going assessment and reflecting the services and resources available to enable customers to progress in their recovery
* Ensuring ongoing assessment and management of risks associated with customers within an attitude of 'positive risk taking'
* Providing support and access to relevant services to enable customers to continue their recovery and empower customers to self-manage their medication regimes through individual re-enablement programmes and informed decision making

What you'll bring:

Essential:
* NVQ Level 3 or equivalent in a relevant area (e.g. substance misuse, addiction, psychology, Health and Social Care,).
* Experience building and developing strong professional partnerships with external providers.
* Good written and oral communication skills

Desirable:
* knowledgeable in "Recovery" with an excellent understanding of addiction,
* capable of completing comprehensive assessments, managing a high caseload of complex cases, creating care plans and risk assessments,
