Community Connect is a well-established Social Prescribing service and you will empower and support residents to access charities, community organisations and groups to enable them to improve their own health & wellbeing. You will work closely with local organisations and healthcare professionals to continue to develop the project and provide the best service to residents.
Duties and Responsibilities
- Be responsible for incoming resident referrals. You will meet with the resident, understand their needs and what matters to them to develop a personalised action plan to provide relevant support.
- Be responsible for referring patients to onward voluntary and community groups for continuing support to their health and wellbeing.
- Provide ongoing support to the resident for approximately four sessions, which could vary from follow-up phone calls or attending appointments with the resident, centred around what is best for the resident.
- Be responsible for developing exit strategies and ensuring residents are receiving appropriate support, and aware of the right services before discharge.
- Actively feedback key themes and insight to ensure continuing development and support of Bexley’s Voluntary Sector.
- To assist in maintaining the database of local services and activities in Bexley.
- Meet residents in a variety of locations including community locations in Bexley and Bexley GP Practices.
- To provide regular updates and reports on caseload management, ensuring that the CRM system is kept sufficiently up-to-date and records all information accurately.
- Work in partnership with GP Surgeries, including providing patient updates, regular outreach sessions and attending multi-disciplinary meetings.
- Be a champion of Social Prescribing, and the voluntary sector, locally, regionally, and nationally.
- Undertake any other duties and tasks which may reasonably be required, as discussed with line-manager.
- Adhere to and actively implement BVSC’s policies and procedures such as Equality and Diversity, Health & Safety and Safeguarding.
- Identify, report, and monitor any safeguarding risks and concerns for Children, Young People, and Adults.
- Commit to undertaking any training considered necessary to ensure and develop own knowledge and skills to effectively deliver the role.
These are the normal duties which are required of the position; however, we do require that all staff be flexible and may be required to perform other duties to ensure the efficient running of services. It is envisaged that this post will be community based but this will be regularly reviewed.

- Be kind, approachable and empathetic in your approach to supporting residents with their health and wellbeing
- Excellent IT skills including previous use of Microsoft Office (especially Microsoft Excel) and CRM database systems.

- Excellent communication skills, enabling you to confidently receive referrals on the phone & email; extracting relevant information whilst ensuring people feel supported and informed

- Able to work on your own initiative and meet deadlines

Contract

Headway East London is a charity supporting people living with brain injury. Working across 13 London boroughs we offer specialist support and services for survivors and their families. About the Role
As HR Officer you will support the HR Manager in a busy HR function. You will play a vital part in ensuring a high-quality HR service and smooth running of the organisation, including overseeing the recruitment process and organising staff training. By focusing on the operational and administrative aspects of HR, you will help our creative, diverse community to thrive and to make a difference in the life of people living with brain injury.
Principal Duties and Responsibilities
- Act as first point of contact on all HR queries from the staff team, pass on complex HR issues to the HR Manager and manage the HR and recruitment email inbox.
- Work with the HR Manager to update or create HR policies and procedures following best practice and keeping up to date with current legislation.
- Oversee the end-to-end recruitment process, ensuring all elements are completed efficiently and effectively. Headway’s recruitment procedure includes: placing adverts, receiving applications, arranging interviews, taking up references, maintaining the diversity monitoring information, processing DBS, liaising with managers to organise inductions, and ensuring the smooth transition of new starters.
- Laise with Line Managers to ensure smooth transitions for leavers, confirming last date, working out outstanding leave entitlement and arranging exit interviews.
- Work with the HR Manager on employee relation issues including disciplinary, capability, grievances, and absence management.
- Be responsible for the administration of Headway HRIS software and collate HR reports from this as and when needed by the Senior Management Team and the Board of Trustees.
- Work with the HR Manager to maintain and improve the staff training and development and ensure staff complete all mandatory and necessary trainings.
- Maintain accurate HR records and information such as job descriptions, staff absences, changes to contracts, work patterns ensuring confidentiality and compliance with GDPR regulations.
- In the absence of the HR Manager work with the Finance Team to provide up to date payroll information to our payroll provider.
- Manage the administration of the pension, salary sacrifice, and staff benefit schemes.
- In conjunction with the Volunteer Co-ordinator provide administrative support in the recruitment process for volunteers, including taking up references, processing DBS and issuing agreements.

They will work within the team to make proactive contact and provide high quality advocacy and support based upon a client led needs and risk assessment to women from aged 13, focusing on working with those aged 18 and over who access the domestic abuse service. They will advise women on criminal justice and civil remedies and related matters, support women to attend court where necessary, and coordinate the provision of multi-agency support. Part of the role will to be to establish positive, proactive, and innovative working relationships with services providing services to clients and partner agencies within the Angelou Partnership. As a Core IDVA will work across the boroughs mentioned above, under the guidance of the team leaders and project manager.

To be successful as a Core IDVA, you’ll need to have an excellent understanding of domestic abuse and its effects on women and children and of best practice within the domestic abuse sector. As an experienced domestic abuse advocate who has worked with complex and multiple needs, you will be skilled in risk management and safety planning, remaining calm in a crisis and in handling sensitive information on a daily basis. Experience of direct work with female survivors of domestic abuse, of supporting women involved in mental health services, and of working within safeguarding procedures is essential for this post, as is the need to adopt and promote a strong partnership approach to service provision.
