About the role
We are seeking a dynamic Family Finding Social Worker or Senior Practitioner wishing to play an active role in the delivery and development of a high-quality family finding service for the London Borough of Hillingdon, for children with a plan for adoption.
The post holder will be one of two professionals responsible for family finding for children with a care plan of adoption within the local area.
The post holder will be part of a small and very supportive team to develop practise and skills in this area. The post holder will also be able to have access resources and services within Hillingdon to support this role and be part of a bigger network of support from colleagues from across the RAA. This role will function as 6 month fixed contract with the possibility of an extension to offer continuity to the children we are family finding for.

Community Drug and Alcohol Recovery Services (CDARS) is a charity organisation that provides psychological and social support to drug users and their family members.
As part of this work, it has built a track record of working with service user representatives, service user Councils and networks and service user involvement teams. Duties and Responsibilities:
- Working collaboratively as part of an integral, multi-disciplinary team addressing the physical, emotional and social needs of children and young people who may be neurodivergent, as well as their families using the service

The ideal candidate will provide a Social Prescribing Link Worker service, to patients of Ascent Primary Care Network (Ascent PCN) in Milton Keynes, working to support patients and staff at each of their 3 medical centres across the area.
The Link Worker will take referrals from the PCN’s practices, and from a wide range of external agencies, and through self-referrals. They’ll work with people to identify issues that affect their health and wellbeing and co-produce a simple personalised care and support plan that connects them to non-medical community-based activities, groups and services that meet practical, social, and emotional needs.
This is a challenging role, managing a demanding caseload, working to strict quality standards and playing a full part in multi-disciplinary team of health and social care professionals. Successful candidates will be able to ensure that all work is compliant with our contract funder – Ascent PCN, and Citizens Advice standards.

- Computer-literate, although training on our case recording systems will be given.

The purpose of the Project worker is to provide a person-centred support to a caseload of Companions in particular to those who are experiencing difficulty in maintaining their tenancy due to their experiences and relationship with substances, including alcohol.

Our client is a successful charity, they have a temporary requirement for an experienced support worker who has worked with young people 16 + who have complex needs.
The successful candidate will be required to work 37.5 hours per week, working across Monday - Sunday on a rota basis, covering early (8AM - 3.30PM), mid (12.30PM - 8PM) and late (3PM - 10.30PM) shifts. There is a requirement to work 2 weekends out of 4. Temporary cover is required for approximately 2 - 3 months.
In this position, you will be required to:
- Hold a caseload of 2 - 4 young people and undertake regular key working sessions resulting in co-produced support and safety plans that are SMART and evidence progress
- Assess the needs of young people in order to identify appropriate move on accommodation
- Produce comprehensive and high quality risk assessments and risk management plans
- Assess referrals to the project to ensure suitable young people can be accepted
- Provide a safe, welcoming and high-quality standard of accommodation, ensuring when young people move in they feel comfortable and welcomed and are given information concerning the building and other local services
- Minimise risk to young people by identifying, reporting and following up any safeguarding concerns and incidents
- Work with social workers to enable young people to move back home if appropriate, or to support young people to identify opportunities for move on to appropriate supported, shared or self-contained accommodation
- Support young people to maximise benefit entitlements and secure project income through the collection of rents and service charges and the minimisation of arrears and void loss
- Run activities and support young people to participate in activities to prepare them for independence and move through
- Provide support to young people who are ready to move into work, education, or training by assisting them to access suitable courses or placements
- Work in partnership with internal departments as well as external community agencies to ensure client needs are met

Bank Support Workers have the ability to accept any shifts available on an ad-hoc basis across from all of Look Ahead's specialisms including Mental Health, Learning Disabilities, Homelessness and Young People. Bank Support Workers will provide support to customers to help them develop the life skills they require to meet the assessed needs of the customers of the service. They will work with customers to promote social inclusion and alongside other members of the team to promote independent living, encouraging customers to maximise their skills and choices. Within Learning Disability services, this will include providing physical, domestic, emotional and social care. Within Mental Health and Homelessness services they will enable vulnerable people living in a variety of different tenures to manage their tenancies/ licences and move towards greater independence within the community by providing a flexible and individually tailored support package. While carrying out activities, Bank Support Workers should deal with all customers, visitors and staff in a professional and appropriate manner and report to Look Ahead staff and management any observations relating to customers' welfare.

To help us achieve our mission, we are recruiting qualified social workers with direct experience of working with children and families, and knowledge and experience of supporting social work students. We are looking for candidates who can contribute to the delivery of teaching across the year, support participants in their local authority hubs and supervise participants during the 2nd year of the programme.
Practice Tutors (PTs) work closely as a peer group to share effective ways of working across participant practice hubs, local authorities and regions. They play a key role within the development and delivery of our social work curriculum, contributing knowledge and expertise to create a culture that promotes outstanding social work education and enables teams to do their best work.
PTs are responsible for teaching and developing participants at the Readiness to Practice stage, regional teaching days and, in participant hubs within local authorities. PTs support four practice hubs, based within statutory children’s social care services. Each practice hub is led by a Consultant Social Worker (CSW). Core to the Practice Tutor role is the ability to provide support, challenge and guidance to participants and CSWs to ensure a high-quality practice learning experience.
Some key responsibilities include:
- Use social work knowledge, theory, practice skill and experience to develop and support participants to become outstanding social workers
- Support CSWs to lead the hubs, role model best social work practice and support effective practice educators
- Teaching for year one participants is during the Readiness for Practice stage both in person and online, attend face to face regional recall days and whole cohort online teaching.. There is also online teaching for our year 2 participants on the programme through large lectures, small seminars and one to one support as necessary
