This is a very exciting time to be involved with Social Prescribing. It has been identified as a key component of NHS England’s Universal Personalised Care objective contained within their Long Term Plan, as well as London’s Health Inequalities Strategy for healthy communities published by The Mayor’s Office in 2018. Both strategies recognise the influence of social, emotional, economic and cultural factors on health and well-being and the importance of community connections to improving health and wellbeing, self-care and self-management of long term conditions.
Family Action is proud to be the provider of the existing local Social Prescribing Service commissioned by City & Hackney CCG delivering since 2014, which recently won “Best Larger Project” at the inaugural Social Prescribing Awards 2019. We are now delighted to be expanding the provision through additional funding provided by City & Hackney Primary Care Networks/Neighbourhoods.

You will work as part of a Primary Care Network Multi-Disciplinary Healthcare Team, providing 1:1 personalised support to Children and young people,ndividuals, and families utilising a holistic and strength-based approach, to help identify the wider issues that impact on their health and wellbeing and work together to co-produce a wellbeing plan to address these needs. You will have excellent holistic assessment skills, as well as interpersonal and communication skills that enable you to build trusting relationships which support individuals feel empowered with their wellbeing.
You will forge strong links with local VCSE organisations, community and neighbourhood networks, building on what is already available, to create a menu of community groups and assets to connect individuals to as part of their wellbeing plan.
You must be a good listener and have experience of working positively with individuals facing complex, social and emotional challenges that impacts on their wellbeing. Being creative as well as having a solution focussed attitude to removing barriers to health and wellbeing is essential.
You will also have strong leadership qualities , experience of supervising staff and report writing to support strategic and organisational requirements .

Havering Complex Needs Pathway: Offers support to 23 residents across 5 services for individuals with low to medium level complex needs. The pathway is designed to support residents and participants in the Havering Borough to reintegrate back into the community and prepare for independent living. This role would be a floating officer based in the Havering borough supporting across the numerous services providing Intensive Housing management support.
Shift Pattern: 37.5 hours per week, 9:00AM -17:00PM or 13:00PM - 21:00PM Monday - Sunday, Onsite.

Location

As Head of Fundraising, you will have overall accountability for Trust’s fundraising strategy. Reporting directly to the Chief Executive Officer, this pivotal role will work closely with the wider senior management team (SMT) and inspire and lead the Trust’s fundraising, marketing and communications team.
This role’s primary responsibilities will be as follows:
Strategic Development and Implementation
· Provide leadership, strategy and direction for the Trust’s fundraising, marketing and communications team comprising 16 in the UK and 3 in Nepal.
· Be part of the Senior Management Team (SMT), contributing to the Trust’s wider vision and strategic direction as well as providing updates and reports for the SMT and Trustees.
· As the organisational expert on fundraising, work closely with the Trust CEO and SMT to ensure that all fundraising activity reflects the vision and agreed priorities of the Trust.
Fundraising
· Implement the Trust’s five-year fundraising strategy (income c.£14m pa), and be accountable for the delivery of operational plans, defining targets and identifying new sources of income.
· Develop and implement fundraising policies and procedures ensuring all fundraising remains safe and legal, including all data held within the CRM
· Create a donor-centric culture within the Trust, implementing an accurate and robust system to deliver effective supporter journeys across all disciplines – individual giving, major donor, legacy, trust and corporate, and community fundraising.
· Keep abreast of current fundraising trends and standards as well as historic analysis of our own activities to optimise all fundraising approaches.
Budgeting and Reporting
· Preparation of the annual fundraining income and expenditure in line with the GWT budget cycle and management of in-year income and expenditure, including variance analysis of financial performance against budgets and forecasts for the 12-month period.
· Work closely with the Head of Finance GWT and the Finance team in Nepal to agree the details and split of unrestricted and restricted projects to be funded each year.
Leadership and Line Management
· Provide strategic direction to all areas of fundraising, nurturing team members to develop their knowledge and expertise, and maintaining a passionate and engaged group of staff committed to delivering excellence for the Trust.
· Coach, inspire and motivate direct reports and other members of the team as well as providing guidance and direction to deliver their objectives and develop their skills and expertise in their roles.

Based from home (must be UK resident), with ad hoc client and team meetings/socials (all travel paid), mostly within England. All IT equipment provided. Full-time or part-time (minimum 0.8 FTE - 28 hrs).

Job Purpose
The Carer Hospital Project Officer will play a crucial role in helping unpaid Carers to get the right support at the right time to help them access the appropriate Carer related support in the respective boroughs. This role works with individuals in a person centre way and support Carer needs by making it Personal, Coordinated and Enabling.
They will work closely with ASC Duty Social Workers and other partners in Health & Social Care to support Carers in the hospital and ensure they have the necessary information and support.
Key Tasks

Being present and visible within their allocated Hospital sites – to both residents/carers and staff

Being ‘expert’ in the statutory and non-statutory Carers support services available in their allocated area – and across NEL more broadly – recognising that many carers access Hospital sites outside of the borough in which they live (we will be developing a ‘directory’ of NEL wide Carers services as part of the project to support this)

Having a comprehensive understanding of the current and emerging legislation surrounding carers, their entitlements and the Statutory Carers Assessment process

Building and maintaining good working relationships with hospital staff – to promote best practice around the identification, support and recording of Carers (in line with the Carers Hospital Discharge Tool kit’). This could include the delivery of Carer Awareness training/sessions for Hospital Staff

Attending Multi-Disciplinary Team meetings as / when appropriate.

Effectively explaining what a carer is as many people don’t see themselves as a carer

Identifying carers within the hospital – ideally at the earliest possible stage/ the point of admission

Actively promote local carer support services within the hospital

Informing the Carer, they have the right to a carers assessment and supporting a referral to the relevant Local Authority/Provider where necessary

Helping to give Carers a ‘voice’ within the Hospital and support them to be engaged in care / discharge planning wherever possible/appropriate

Ensuring that any identified carers have been informed of the broader support available to them and ‘intelligently signposting’ them

Attending monthly team meetings with the Project Manager, evaluation partner and other Carer Hospital Project Officer to review impact data, share learning and identify opportunities to maximise impact

Attending quarterly project partnership board meetings with all members involved in the project

Attending any meeting/ events as appropriate within their carers support service

Proactively escalating any issues and or ideas about how services can be improved to Hospital leads and the Project Manager

Co-producing and delivering the TEC pilot

Working with the evaluation team on key findings and data from the project

Keep accurate and up-to-date records of contact with clients, including the use of relative databases (relevant training will be provided) and the Age UK East London Charity Log data base.

Provide additional data as required e.g., case studies, outcomes monitoring

Each Carer Hospital Project Officer/provider will be required to submit monitoring data to the Project Manager monthly.

Data from the first 3 months of delivery will be captured and base-lined, and Carer Hospital Project Officers will be expected to increase their activity/reach by 5% in each additional quarter of project delivery.

In this role, you will work closely with Primary Care Networks, GP practices, local authorities, social workers, voluntary sector, community centres, and faith groups to help identify and engage with Unpaid Adult Carers, Young Adult Carers and Young Carers.

As life becomes ever more difficult for migrants across London, the UK and the world, Waltham Forest Migrant Action (WFMA) are committed to fighting back in solidarity and supporting our fellow human beings who are often suffering in the most extreme of circumstances.
WFMA is the only provider of free immigration advice in the borough. Our mission is to provide support for local migrants and refugees, and to advocate and campaign for the rights of migrants and refugees in the UK. Since 2013, our weekly drop-in centre has offered visitors a warm welcome, free advice on immigration, accommodation and welfare issues, and opportunities to socialise with the local community. Last year we helped 366 people from 71 nationalities, and our numbers continue to grow.
THE ROLE
WFMA is looking for a compassionate Welfare Advisor with a strong interest in migrant justice to support our visitors with Welfare and Housing issues, and make a tangible difference to the lives of migrants and refugees in Waltham Forest. This is a new post at an exciting time for the charity as we seek to expand our services at the drop-in centre and provide more immigration advice and casework in-house.
1. Supporting drop-in centre visitors with welfare and housing issues
- Determine eligibility and advise on all Public Benefits and how to apply for them, and in some cases help them to apply
- Advise on Housing issues, solutions and consequences, and when appropriate help the visitor find and brief a housing solicitor
- Advise on and help visitors access healthcare services
- Guide, inform and support volunteers to assist visitors
- Maintain records of advice and actions taken using our case recordkeeping software, scanning and securely storing documents shared with us
2. Maintain current knowledge and understanding of public benefits and housing systems
- Keep abreast of new developments in the public welfare and housing systems
- Liaise with other agencies and organisations as and when appropriate
- Attend training courses as and when needed, and proactively seek opportunities to develop professionally
3. Work collaboratively with WFMA staff, volunteers and professional advisors
- Understand and implement WFMA policies and procedures
- Participate actively in staff and board of trustees meetings, and other meetings as may reasonably be required
