Family Connections Worker – Crewe Location: Crewe Salary: £24,000 per annum Vacancy Type: Permanent Closing Date: 17 Dec 2025 About The Role We are currently recruiting for a Family Connections Worker to join our team in Crewe. We are Forward, the social enterprise that empowers people to break the cycle of crime and addiction to move forward and live fulfilling lives. We deliver a range of MOJ Commissioned Rehabilitative services (CRS) directly for Probation in three key areas: Accommodation, Personal Wellbeing and Dependency & Recovery. Accommodation is delivered in Wales, Dependency and Recovery in London and Humberside, Personal Wellbeing in East of England, Sussex, Surrey and as a subcontractor in Kent and Cheshire. These services work with Men 18+ who are under probation supervision, supporting them to navigate through their desired outcomes in order to achieve transformational change, break the cycle of offending and achieve long-term integration into society. No matter what your past, we believe anyone is capable of lasting change Role Responsibilities You will be working remotely as part of a flexible team of Family Connections practitioners, specifically delivering as part of the wider personal well-being service, Family and Significant others interventions to support adult men and young men in the Criminal Justice system. The support is tailored to individual needs, delivering a range of programmes within the categories of Family and Significant others. The Family Connections practitioner will deliver client centred support depending on the requirements of the individual client. You will be required to manage a caseload, provide appropriate advice and guidance, assess, care plan; deliver structured 1:1 and group-work sessions. You will be responsible for contributing to the targets set by Forward, Interventions Alliance, whom sub-contract Forward Trust to deliver Family and Significant Others service and local commissioners. One of the key targets will be measuring outcomes, so co-operative working relationships with other partner agencies is vital to this role. Flexibility will be required in this role, this may include some evening/weekend shift and with reasonable notice to travel to other projects in your cluster to deliver Health and wellbeing services, enabling continuity of care for the client group. Desistance, Recovery, and Wellbeing Team Leader - HMP Millsike

We’re now looking for a Desistance, Recovery, and Wellbeing Team Leader to play a key role in this mission.
You’ll lead a team of passionate practitioners delivering high-quality desistance, recovery, and wellbeing services across HMP Millsike. You’ll also support in the matrix management of Mitie Care and Custody staff working within our More Than My Past community and Incentivised Substance Free Living Unit (ISFL).
Your work will help bring our vision to life, creating a safer, more supportive environment that inspires change and growth.
Key responsibilities include:
- Line-managing staff delivering programmes such as More Than My Past, Step Back to Step Forward, and Thrive (Resettlement).
- Overseeing operations in the ISFL and ensuring compliance with MoJ standards.
- Promoting wellbeing through workshops, ambassador engagement, and annual wellbeing initiatives.
- Supporting delivery of desistance and recovery programmes across the establishment.
- Collaborating with prison leadership and partners to implement the establishment’s Drug Strategy.
- Managing the Departure Lounge, ensuring prisoners’ emotional wellbeing and smooth transition at the point of release.

Hertfordshire RECONNECT Worker (Maternity Cover)

About The Role
Are you passionate about supporting people as they transition from custody back into the community?
Join Forward Trust as a RECONNECT Worker (12-month maternity cover), delivering an inclusive, person-centred service that helps individuals access the health, wellbeing and support networks they need to thrive after release.
You will provide tailored advice, guidance and care planning for people 12 weeks pre-release and up to 6 months post-release, ensuring they connect with the right services, from substance use support to mental and physical healthcare.
What You’ll Do
- Process referrals, complete detailed needs assessments and update referrers on outcomes.
- Create bespoke care plans and offer continued support to service users after release.
- Manage a diverse caseload, supporting reintegration and engagement with local healthcare and wellbeing services.
- Assist the Team Leader with recruitment and training of prison peer supporters.
- Build and maintain strong relationships with healthcare teams, prison staff, Offender Management and other key partners.
- Keep accurate and concise case notes, assessments, care plans and risk assessments.
- Support the development of clear referral pathways across internal and external agencies.
- Contribute to safe, holistic service delivery and share best practice with colleagues.
- Refer service users to the Forward Meet & Greet service and support Meet at the Gate releases.
- Ensure individuals are fully connected with community services in preparation for release.

Health and Wellbeing Practitioner Location: East Riding of Yorkshire Salary: £24,000 per annum Vacancy Type: Permanent Closing Date: 16 Dec 2025 About The Role Please note: We have two positions available, one at 30 hours per week and one at 15 hours per week. Due to the rural location of HMP Millsike, a valid driving license and access to your own vehicle are essential for this role . HMP Millsike is a pioneering establishment built on a contemporary, evidence-based approach to prison delivery.
