Mental Health Homelessness Outreach Team Leader

Working base: Flexible to one of our seven Wellbeing Centres across Hertfordshire, dependent on location.

About the Service

This is a pilot service working as a multi-disciplinary team with housing teams and Drug and Alcohol Services across District and Borough councils. These teams will be working with individuals who are presenting as homeless and have a number of Complex Needs. The Complex Needs Housing Team Leader plays a significant role in the co-ordination and running of the Complex Needs Housing Service.

The objectives of Hertfordshire Mind Complex Needs Housing Service are:

• To improve the mental wellbeing of people experiencing mental ill health.
• To increase early access to help for people experiencing mental ill health in, by providing a clear and effective pathway to services provided by Herts Mind Network and other third sector and statutory providers.
• To contribute to an improvement in individual mental wellbeing.
• To remain a source of independent support for all clients.
• To reduce the use of police, ASB, tenancy enforcement, housing needs and environmental health services by clients whose primary need is related to mental ill health.
• To reduce the use of crisis services by people experiencing mental ill health with positive outcomes for the individual.
• To provide an assertive outreach approach for clients who are in crisis and/ or those that may have complex or multiple needs.
• To work collaboratively within an MDT consisting of CGL, District and Borough councils, housing teams and other professionals.

This role is key to the development and support of the team and the wider organisation. 
The post holder will need to:

• Support the Services Manager with effective running of the Complex Needs Housing Service.
• Provide advice, information, onward referral and holistic support to clients who are experiencing mental ill health or need support with their mental wellbeing. These will be clients who are presenting to District and Borough councils as homeless and need intervention and support with their mental health.
• Line manage staff and where necessary volunteers.
• Lead a team of Mental Health Outreach Workers. Ensuring the team are using databases and outcome tools as required to report for the project.

Key Responsibilities

• To receive and process referrals from the local district and borough Councils for those who have presented as homeless with a mental health need, as well as other appropriate referrals pathways across Hertfordshire and make direct contact within 72 hours of referral.
• To establish effective pathways across a variety of agencies to maximise and provide effective partnership working.
• To manage a caseload of up to 5 clients.
• Embed a holistic, trauma informed and person-centred service
• Proactively recognise the indicators of deteriorating mental health and facilitate appropriate action, whilst liaising with relevant agencies.
• Ensure a direct community service is in place which encourages, motivates and builds patients self-esteem and confidence. This will include goal setting and support with social and practical skills development.
• Ensure all person-centred risk assessments and support plans are completed as appropriate.
• To assist clients to access appropriate mental health services, drug and alcohol services, housing/tenancy services and other services according to assessed needs.
• To work with partners both internal and external to ensure that vulnerable person’s cases are monitored and risks evaluated in order to promote community safety.
• Support clients to build their emotional resilience and crisis planning tools, promoting self-management through building an understanding of their own triggers/coping strategies.
• To work with HMN staff in other services in order to make onward referrals to other projects.
• Liaise with appropriate HPFT staff to ensure that individuals are assessed for personal budgets to support with any longer-term needs.
• Support individuals to develop sustainable local social networks, both through our HMN Wellbeing Centres and other community services and resources.

The successful candidate will play a significant role in the co-ordination and running of the Complex Needs Housing Service.

As the Head of Development & Communications, you will be responsible for developing, implementing, and achieving EECF’s income generation strategy. Key responsibilities:

· Lead the strategy setting and annual planning and budget setting for Comic Relief’s Digital Marketing & Fundraising, including building financial models, developing business cases, monitoring budgets and updating forecasts

· Manage and coach a team of digital fundraisers to execute fundraising, (acquisition and engagement), activity across our key digital channels to deliver against agreed plans and objectives across the entire Fundraising team, specifically focussed on the UK public audience: individuals and communities of individuals, including schools and workplaces

· Operate as a champion of digital innovation within the wider Fundraising team and organisation, proactively identifying, sourcing and helping to embed new technologies, ways of working and tools and approaches that drive digital marketing/fundraising performance and income generation. · Lead the management and performance management of our media agency to develop a digital media strategy that delivers against agreed KPI’s, working closely with the Paid Media Manager to continuously optimise and evaluate performance across the full suite of campaigns and always on activity plan

· Oversee the email marketing strategy, including oversight of critical email marketing tools and platforms, ensuring the effective management of audiences and messages, closely with the Senior Digital Fundraiser and Email Marketing Executive to continuously optimise and evaluate performance across the full suite of communications

· Play a lead role in working with stakeholders across the organisation to unify separate digital marketing activity (owned and earned) to ensure all Comic Relief efforts are working together to maximise effectiveness and efficiency

· Play a lead role in working with colleagues managing Comic Relief’s website and associated platforms to ensure they are optimised for User Experience and conversion to action across all agreed initiatives and activities

The Information Manager will coordinate and develop a range of health information needed by the migraine community and people living with migraine to raise awareness to both members of the public and health care professionals. The role is also responsible for ensuring the information on the website is up to date and coordinating and managing Patient Information Forum renewals.

Key responsibilities

• Produce and maintain quality health information: You will manage and update health information, scope, plan, and create new content in consultation with stakeholders, and identify target groups to produce materials to meet their needs. You will work within the Information and Support team constantly communicating with colleagues working on front line services to ensure the information being produced is in line with service users needs. You will also be responsible for managing the organisations PIF processes and assessments.
• Engage with the migraine community: You will hold focus groups to understand information needs, build strategic partnerships to disseminate information effectively, and support staff learning and knowledge-building about migraine.
• Support communication and dissemination: You will work with the Communications team to create accessible information formats, provide content for videos, blogs, and other communication channels and regularly monitor and report on the impact of information provision.
• Research: You will act as the key point of communication for the research committee, manage administrative duties for research funding applications and grants, and coordinate research meetings and liaise with funded researchers for project updates.
• Represent and support the charity: You will represent The Migraine Trust at external events and build relationships with healthcare professionals and stakeholders.

The Information Manager will have solid experience of communicating complex health information to audiences in written and online formats, as well as proven experience of producing information from the research stage through to writing, external review and liaising with designers. The successful candidate will have the ability to gain an in-depth understanding of medical and health related information, including research and critical analysis skills to develop an evidence base. You will have experience working within a health communication environment, with excellent verbal and written communication skills, and meticulous attention to detail with demonstrable editing and proofreading skills. The foundation is currently based in a Hong Kong family office, with personnel in Hong Kong and the UK, as well as Cameroon, Brazil, Australia, Papua, and the US. We are seeking a hands-on, seasoned, and globally-minded leader to serve as the foundation’s Director of Operations (DO) as the institution continues to evolve and develop other vehicles fit for its mission and activities.

Role

As Director of Operations, you will be a key member of the foundation’s leadership team jointly responsible for strategic planning and overall management of the institution to ensure organisational health and the fulfilment of the foundation’s mission. You will report to the Executive Director (ED).

Director of Operations Responsibilities

 Finance

 Information Technology

Risk Management

Qualifications
