Job Purpose

To manage and develop the home from hospital settle service in the hospitals that you are responsible for, to support timely and successful patient discharges from hospital and prevent re-admissions.  You will work in collaboration with hospital teams and community teams to lead and manage your team to support the smooth transition from hospital to home for service users.  The service works with people for time limited support (usually up to 6 weeks) to adjust and settle back in their homes, and to prevent readmission into hospital. 

Key Tasks

• Work in partnership with the hospitals and other health and social care professionals to coordinate safe and timely discharges for referred clients.  This involves attending meetings with hospital personnel including Discharge Co-ordinators and Ward Managers to discuss discharges and plan who and how individuals will be supported as appropriate. 
• Manage a team working across two hospitals and ensuring the service is available between the contracted hours.
• Work collaboratively with other agencies providing support services.
• Conduct initial assessments of people referred to the services as necessary.
• Plan and develop person centred interventions to provide short term support for people after hospital discharge.
• Work with GPs as appropriate in identifying and supporting patients to reduce unnecessary hospital admissions.
• Work with people with a range of needs, dealing with issues ranging from social isolation and keeping people engaged in their community, to preventing unnecessary admission to hospital.
• Support the hospitals with delayed transfer of care (DTOC).
• Coordinate and monitor the caseload of referrals allocated to team members to ensure a fair and manageable distribution of work.
• Rota planning to ensure staff flexibility and availability, enabling planned rosters that best suit both hospital and patient needs.
• Develop your team which includes; supervision, appraisal, planning of continued professional development opportunities and training needs.
• Conduct regular management audits to ensure a consistent way of working and identify development and training needs in the team or individual.
• Develop and deliver a flexible service that can respond to peaks and troughs in demand.
• Ensure the service is meeting key performance indicators, and identify any concerns e.g. demand for the service exceeding the levels in the contract and taking proactive action to address this.
• Monitor performance of the services against KPIs and produce reports to demonstrate performance.
• Deliver front-line services when needed, e.g. sickness or staff shortages.  This is by exception rather than you regularly doing support work.
• Manage the budget, sign-off expenses, and ensure the project is delivered to budget, liaising with the Finance Department as necessary.
• Support the directorate with generating funding e.g. being actively involved in consultation of possible projects, providing activity data, opportunity spotting, and/or supporting the production of tenders.
• On a rota basis, provide weekend on call cover for the services you manage, and other hospital settle services provided by AUKEL.

Administration

• Lead on contract reporting, this includes quarterly, annual and ad hoc reports as necessary.
• Carry out annual customer service surveys.
• Enable the impact of the service to be assessed and inform the improvement of other local services by keeping electronic records of all referrals and interventions, and producing reports on activity and outcomes, in accordance with KPIs.
• Meet and produce reports as required by the Head of Service.
• Ensure patient records are regularly updated through AUKEL database, and support staff to complete their administration via training etc.
• Work with the Communications team and the Head of Service, market and promote the service.
• Build and circulate a resource pack for the wider AUKEL team to generate referrals between services delivered by the charity.

Quality

• Be familiar with and implement AUKEL policies and procedures.
• Ensure the service is delivered in accordance with Care Quality Commission principles.
• Ensure a cycle of continuous improvement by taking a “lessons learnt” review approach.
• Ensure that the requirements of the contract specification for the services are met.
• Ensure integration with other AUKEL projects, and service users are referred/sign-posted to appropriate support services available to them.
• Respond to safeguarding or other concerns and take appropriate reporting and management action.
• Lead on recruitment of staff for your team, and support others at AUKEL.

 Liaison

• Represent AUKEL and participate in appropriate external meetings and events and remain aware of local, regional and national issues affecting AUKEL clients.
• Develop external relationships with stakeholders, and build positive connections with health and social care partners
• Attend contract management meetings with commissioners alongside the Head of Hospital and Home Services.

 General

• Meet regularly with line manager for support, supervision and appraisal.
• Attend team and staff meetings, (and other meetings) as required.
• Complete any training required to fulfil the post.
• Carry out the duties of the post in accordance with AUKEL’s policies and procedures including Equal Opportunities, Health & Safety, Confidentiality, Complaints, GDPR, Safeguarding Vulnerable Adults, Safeguarding, and Care Quality Commission (CQC) guidelines.

Experience

Essential

• Minimum of 2 years’ experience of people management including line management and team leadership.
• Minimum of 2 years’ experience of delivering services within a health or care setting at a level reflecting the responsibilities of the post.

Desirable

Regional Fundraising Manager (West Wales)

Join Wales’ leading cancer charity as a Fundraiser and manage raising funds across an already established West Wales region. There’s loads of scope to generate more income and grow connections.

You’ll be joining an experienced Wales fundraising team who generate over £2.8million of annual income. Your fundraising impact is seen in people affected by cancer through the services we run, the difference to lives we make and the campaigns that make meaningful change.

You’ll be managing and building relationships across West Wales with our supporters, Friends of Tenovus groups and establishing new fundraising connections and work with teams of volunteers. You can expect to manage events such as our annual Lovelight concerts, Singathons and Breast Cancer Awareness Month to name just a few.

Working Hours: 35 hours per week with flexible working

Due to the nature of our fundraising work you’ll need to be flexible at times to work evenings and weekends. Around this we work core 11am-3pm hours to flexibly suit work around your lifestyle and personal commitments and operate our work flexibly between 7am – 9pm so you can do things like the school runs or fit in wellness activities and hobbies with your working life.

It’s an exciting time to join The Institute of Pre-Hospital Care, the dedicated team work collaboratively with a range of stakeholders to deliver clinical and degree course programmes over a variety of subjects and faculties. Clinical Courses are mainly delivered to our own team of doctors and paramedics, while the degree programs consist of an undergraduate intercalated BSc for medical students and an MSc open to a multi-professional cohort. 

We are seeking to appoint a Head of Institute to lead the development and delivery of a detailed IOPHC strategy that responds to the new LAA strategy. Working closely with the Medical Director and other senior clinicians, this role will ensure that the IOPHC remains well aligned with the clinical work of the service.  

The successful candidate will lead the creation of a clinical innovation function, ensuring that the IOPHC acts as a key enabler for new clinical developments, and continues to expand opportunities for LAA to influence pre-hospital care nationally and internationally.  

The Head of Institute will provide leadership and management of the IOPHC team, working closely with the Senior IOPHC Manager to ensure the delivery of high-quality education and training. They will also support the paramedic leadership team with the further evolution of paramedic training at LAA.   

Care & Support Digital Technology Officer

• Support teams across Care & Support (C&S) Service delivery to make the best use of technology in their work
• Collaborate with the Digital Technology team to design and deliver comprehensive guides for digital tools and systems, including Sona and the Care Planning system
• Assess the digital proficiency of staff and create tailored training plans to bridge skill gaps
• Act as the primary point of contact for C&S staff seeking support with digital tools, ensuring systems are used effectively to enhance care delivery, record-keeping, and operational efficiency

Purpose of the Collections Care and Conservation Manager role:

We are looking for an experienced Collections Care and Conservation Manager to join the Museum team at an exciting time as we prepare for a major store move and develop vibrant and engaging new exhibitions.

You will manage the development and delivery of collections care and conservation projects and facilities across the three Museum sites as well as a range of activities to ensure the preservation of collections while promoting wider public access to the RAF story and volunteer engagement.

Key responsibilities of our Collections Care and Conservation Manager include:

• Lead the collections care and conservation team, integrating staff and volunteers to condition assess, document, clean and conserve objects ranging from aircraft and uniforms to personal papers and art works so they can be shared with audiences on site, offsite and online.
• Plan, manage and monitor the delivery of programmes for the conservation of and access to the Museum’s collections, planning activities, managing risk, time and resources in accordance with professional standards, policies, procedures and legislative requirements.
• Ensure the safe, secure, effective and efficient management of the conservation team, volunteers, contractors, conservation materials, equipment and facilities. 
• Ensure an effective balance is made between long-term preservation, restoration and the Museum’s access and interpretation objectives.
• Manage the Collections Disaster Response and Salvage Plan, acting as lead contact in emergencies relating to collections, liaising with the Visitor Experience Supervisors to ensure effective incident management, damage reporting and to support wider emergency planning through the Museum’s Incident Management Plan.
• Support and facilitate the Museum’s commercial activities

What we are looking for in our Collections Care and Conservation Manager:

• Relevant experience in managing, maintaining and conserving museum collections, notably in relation to stored collections, collection moves and exhibition projects.
• Demonstrable experience of implemented collections care in line with professional standards and best practice.
• Communicates in a straightforward manner, demonstrating respect and acting with integrity and impartiality.
• Recognises scope of own authority for decision making and empowers team members to make appropriate decisions.
• Relevant conservation or collections management qualification.

Reporting the Head of Institute, the successful candidate will ensure the delivery of high quality and effective education and training across the clinical courses and degree courses portfolios. They will maintain effective and sustainable working relationships with key stakeholders, and support the implementation of process improvement across the institute. 

The Senior Institute Manager will manage a team of course managers and coordinators, as well as having direct involvement in a program delivery where required. The post holder will ensure good liaison between all parties in the delivery and future development of these programs.

Assistant Manager

PLUS is delighted to be able to offer an exciting opportunity for an experienced individual to progress their career into a junior management role supporting a registered service manager to meet the challenges of supporting adults with learning disabilities and complex health conditions at one of the following, supported living or registered service

You will support the line manager of the service with the day to day service provision to meet service users’ aspirations.  Your role will be to guide and assist support worker staff and be responsible for the day to day provision of the service as directed by the Registered Manager. This is an excellent opportunity to take the first steps on a career path for those staff who aspire to be future managers. 

You would be required to ensure the smooth running of the services within the legal requirements of CQC and any commissioner requirements. You will also attend care/support planning and review meetings and liaise with health and social care professionals involved in service users’ care as well as supporting service users in the same way as other support staff across the full range of duties.

This is a full-time post, so applicants must be prepared to work the full range of shifts including weekends, sleep-in, waking nights etc.  You must be physically capable of pushing wheelchairs and supporting service users who may be physically dependent. Key Responsibilities

Overview

The Olive Tree Cancer Support Group (the Olive Tree), is a registered Charity, dedicated to the relief of the physical and/or emotional distress of persons with cancer and their partners, families, friends and carers, through the provision of services, operating from its support centres in Crawley and Horsham.

A Centre Manager is being sought to manage all aspects of the Horsham centre, working closely with the Crawley Centre Manager and reporting to the Board of Trustees, delivering the charity’s goals and long-term strategy. The responsibilities include staff and volunteer management and recruitment, onboarding new patients / service users, engaging with external stakeholders and all other operational and day to day financial matters, including supervision of team members responsible for fundraising activities, social media and communications.

How are staff supported to work remotely?
