East London Apartments provides 24-hour accommodation-based support for up to 31 individuals with complex histories, multiple traumas, complex personality disorder(s) and a varied, disrupted housing history with experience of homelessness.
East London Apartments implements and promotes the Psychologically Informed Environment (PIE) model. This is an approach to supporting people out of homelessness, in particular those who have experienced complex trauma or are diagnosed with a personality disorder. It also considers the psychological needs of staff: developing skills and knowledge, increasing motivation, job satisfaction and resilience.
Working with individuals with complex needs, who may display challenging, chaotic behaviours, regularly test boundaries and have experienced multiple traumas as a result of abuse, neglect and years of street homelessness, it is integral that the post holder is motivated to support individuals to progress. They should continually hold aspirations for individuals despite multiple setbacks, and display emotional resilience, able to process challenging incidents and remain motivated to deliver excellent support.
The successful applicant will have experience working closely with a range of services to support individuals to address and/or recover from their dependence on drugs and or alcohol. You will have excellent knowledge of harm reduction techniques and various treatment approaches for addiction and be confident in the use of these approaches. These should include but are not limited to medication treatment, behavioural counselling and relapse prevention. You will also provide information, social inclusion and vocational opportunities to support people to recover and stay well; exercise choice and control in their care and lives; and participate on an equal footing in the community. Provision of care and support is also provided to develop a model of re-enablement towards self-advocacy, self-management and empowerment, incorporating the principles and practice of recovery, co-production and peer support.
Post holders will deliver trauma informed and asset based support to a caseload of up to 6 complex service users with substance misuse support needs. - The caseload is subject to change depending on the service needs.

What you'll bring:
NVQ Level 3 or equivalent in a relevant area (e.g. substance misuse, addiction, psychology, Health and Social Care,).
Experience building and developing strong professional partnerships with external providers.
A minimum of 2 years experience in complex needs services, community development, addiction counselling, drug and alcohol work or other related fields.
Good written and oral communication skills
2-3 years experience providing targeted recovery focussed support to individuals with needs around trauma, mental health, substance use and dual diagnosis.

We provide 17 spaces across 8 properties with 1-1 key working support and community activities, with a training and employment project underway to assist residents transition successfully in to indendant living when they feel ready to do so.

MAIN PURPOSE OF JOB:
- To support residents to maintain and continue their recovery from drug and/or alcohol problems.
- To support residents to engage in positive activities (volunteering, work or education) to underpin their recovery.
- To manage the maintenance, upkeep, and management of all properties.
Main Outcomes
Residents remain substance free and in recovery.
- Overseeing incoming referrals to obtain new residents.
- Undertaking drug and alcohol testing.
- Providing advice/signpost residents to additional recovery support and other issues such as housing, health, benefits, and finance.
- Assisting residents in times of crisis, offering them advice and support as appropriate.
- Developing the quality and effectiveness of the service, including utilising service user feedback.
- Attending training as appropriate and/or necessary.
Residents follow care plans and comply with licence agreements so that they grow in self-worth, self-confidence, and resilience.
- Undertake weekly keywork sessions, tracking individualised goals as agreed on care plan.
- Co-facilitating groupwork.
- Monitoring the adherence to property and programme rules and helping to develop a healthy resident community.
- Keeping care plans and files up to date.
- Offering a high level of care and support to all, always maintaining appropriate professional conduct and boundaries.
The properties are kept to a high standard of repair and meet all legal and Landlord requirements.
- Through key working residents and regular visits and checks. Identify issues that would need resolving with the properties.
- Maintain relationships with Landlords to address their needs and repairs required.
- Arrange trades to deal with maintenance issues and make repairs
Working conditions (e.g., hours of work, any travelling required etc):
- Full Time (40 hours) per week, 12 month contract with a view to be made permanent.
- 25 days annual leave per year pro-rata, plus bank holidays.
- Reporting to Head of Key Working.
- You will be expected to travel to see residents (and employers, voluntary placements, etc) within the local area as part of your role. Full driving license, vehicle and relevant insurance required.

In this role you will work across two services, our Newcastle Supported Housing and Personal Budget Service.
Working for our Newcastle Support Housing service you will support individuals aged 18 and upwards who have a mental health diagnosis to move into one of our tenancies helping them to ensure the transition is as smooth as possible, you will then meet with them each week to support them both emotionally and practically to develop life skills which enable them to sustain their tenancy and eventually move on to independent living.
This can include a number of tasks such as money management, managing bills, reporting repairs and supporting them to keep on top of the cleanliness of their property. You will support them with all aspects of the move on process. You will then support the team to get the property ready for our next move in. The individuals that we support though our housing service have multiple and complex needs, this can include a struggle with addiction or an offending history as well as other support needs.
In our Personal Budget Service you will support individuals aged 18 and upwards with a mental health diagnosis who have block hours of support and whose mental ill health is causing them concern and/or affecting their lives in various way. You will meet these individuals either in their own homes and out in the community and provide tailored support around areas that they currently struggle with to help them achieve their goals. The individuals that we support through this service live across Tyne, Wear and Northumberland.

An exciting opportunity has arisen to join us as an Outreach Worker supporting individuals who are homeless with complex needs and have been excluded from services.
Richmond Fellowship works as part of the Bridge the Gap Alliance within a well-established, multidisciplinary team set up to meet the needs of homeless people based in East Surrey. We are currently seeking an Outreach Worker who will be responsible for engaging with individuals with multiple and complex needs and working alongside internal & external agencies. You will support people through their journeys into statutory and voluntary services. The post holder will be expected to navigate local service provision, building strong relationships with a range of different professionals whilst advocating on the client’s behalf.
We will work with local partners to co-ordinate the care of clients to influence change in the way that organisations provide services to individuals facing multiple needs and exclusions. Applicants must have empathy with the client group and the ability to work within the ethos of Richmond Fellowship.
You will need to enjoy working with people, have a “can do” attitude and show commitment, resilience, empathy and good basic admin and IT skills. to effect system changes that can more effectively support those with complex and multiple needs and ensures that positive changes are made in the way that local services are provided.
You will work with individuals and their support networks to create personalised service plans, with the long-term aim to secure sustainable wellbeing, attend Operational Meetings to discuss clients’ needs and situations, enabling access to appropriate services and ensure that barriers are addressed so that service users can access suitable services. They will work with voluntary and statutory sector organisations to develop a personalised case management approach for homeless individuals with multiple and complex needs.
You will have experience of helping vulnerable people to identify personal goals and supporting them through a process of change, building supportive, trusting relationships with clients and working proactively with other members of the team to handle the service caseload and administrative responsibilities. They will support clients to set personalised goals in the form of a Support Plan, conducting regular key work sessions that are innovative and engaging to achieve Support Plan goals and ensuring ongoing assessment and management of risks associated with clients within an attitude of ‘positive risk taking’.
This is a fixed term post that ends on 31 March 2025. There are two part time posts available requiring the post holder to work 18 hours per week. This role includes evening and weekend working.

About the role:

Fixed Term Contract Until March 2025
The role involves working in multi-agency settings in Camden and Islington, working in partnership with existing delivery structures, local networks and developing collective practices. Navigators are able to manage their own their own time, working flexibly on the go, including some working from home.
The Navigator will carry a caseload of 10 client who are rough sleeping and experiencing multiple disadvantage, and will provide end-to-end holistic, coordinated support for each individual, from first contact in a street setting through to helping them to achieve a more settled and fulfilled lifestyle.
Navigators will provide personalised solutions which demonstrate innovation, creativity as well as iterative learning.
The approach taken with clients will be psychologically informed, incorporating an understanding of acute mental health conditions, complex trauma, and addiction.
As a Navigator, you will provide support such as:
- Assessing their needs and identifying their goals.
- Helping them to find and maintain suitable accommodation.
- Supporting them to access health, social care, and mental health services.
- Assisting them to apply for benefits, grants, and other entitlements.
- Linking them with education, training, and employment opportunities.
- Empowering them to develop their skills, confidence, and resilience.
- Advocating for them with other agencies and professionals.
- Providing crisis intervention and harm reduction advice.
Please note: We will also be considering applicants from this vacancy for a permanent Navigator position for one of our Services in Hackney. This role is permanent but paid at a different salary banding starting at £30,342.85 and rising incrementally to £33,228.99 per annum.
About you:
- Previous experience of working with either rough sleepers or people with complex mental health, alcohol or substance use support needs.
- An understanding of working with people using person-centred approaches and working collaboratively within a harm minimisation and recovery framework.
- The ability to manage a caseload and deliver positive outcomes, utilising your knowledge of the health and social care sector is also needed.
- A flexible and creative approach to working with a sometimes hard to reach client group.

The role

- NVQ/QCF Level 2 or 3 in a social care field.

About the Service
This is a pilot working as a Multi-Disciplinary Team (MDT) with housing teams and Drug and Alcohol Services across District and Borough councils. These teams will be working with individuals who are presenting as homeless and have a number of Complex Needs.
Purpose of Post
Providing advice, information, onward referral and holistic support to clients who are experiencing mental ill health or need support with their mental wellbeing. These will be clients who are presenting to District and Borough councils as homeless and need intervention and support with their mental health and remain a source of independent support for all clients. Ensure that the safety and wellbeing of the client is monitored and reviewed regularly and that a person centred recovery orientated and trauma informed approach is embedded approach in all aspects of the roles.
Service Objectives
- To improve the mental wellbeing of people experiencing mental ill health.
- To increase early access to help for people experiencing mental ill health in, by providing a clear and effective pathway to services provided by Herts Mind Network and other third sector and statutory providers.
- To contribute to an improvement in individual mental wellbeing.
- To remain a source of independent support for all clients.
- To reduce the use of police, ASB, tenancy enforcement, housing needs and environmental health services by clients whose primary need is related to mental ill health.
- To reduce the use of crisis services by people experiencing mental ill health without positive outcomes for the individual.
- To provide an assertive outreach approach for clients who are in crisis and/ or those that may have complex or multiple needs.
- To work collaboratively within an MDT consisting of CGL, District and Borough councils, housing teams and other professionals.

The Successful Candidate will

- Be an independent source of support for all clients.
- Embed a person-centered and recovery orientated approach in all aspects of the roles. The Recovery approach includes empathy, warmth, acceptance, authenticity, compassion and humanity.
Key Accountabilities
- To receive referrals a wide range of referral pathways.
- To undertake initial contact with clients and complete a holistic needs assessment, making use of assertive outreach work to achieve the initial client contact, if required.
- To provide holistic support both outreach and in our centres and advocacy services to clients experiencing mental ill health.
- Ensure all person-centered risk assessments and support plans are completed as appropriate.
- To accompany clients to initial visits and appointments as necessary in order to ensure the sustained use of existing support services.
- To liaise with other agencies to ensure that clients receive appropriate support and continue to remain engaged with the service.
- To assist clients to access appropriate mental health services, drug and alcohol services, housing/tenancy services and other services according to assessed needs.
- To establish effective pathways across a variety of agencies to maximise and provide effective partnership working.
