About the Accelerating Reform Fund

The Accelerating Reform Fund is a new initiative which aims to transform social care services, making them more responsive to the needs of the community. It is a national programme, with 42 regions allocated to have their own projects. Greater Manchester is one of the ARF regions. The ARF has a particular focus on unpaid carers and aims to boost short-term breaks for carers and improve hospital discharge processes. This work is made possible by Department for Health and Social Care grants. As an organisation, Gaddum has supported and collaborated with unpaid carers for many years, so we’re excited to bring our experience to the table. This role will be based in the Manchester team working to improve hospital discharge processes.

Job Summary

To develop and ensure safe discharge planning and information for patients and unpaid carers. To embed procedures to raise awareness of the needs of carers in a secondary care setting, working in partnership with the discharge, virtual and enablement teams. To empower carers to be involved in the discharge process.

Job Purpose

The purpose of this role is to provide operational leadership and oversight of the Carers Hospital Discharge Pilot in Manchester. The role involves line-managing a team of three Hospital Discharge Workers across multiple hospital sites, ensuring effective collaboration with hospital staff and community services to support carers. The aim is to enhance the identification and referral of carers, ensuring timely and appropriate support to prevent carer breakdown and facilitate smoother, safer, and more efficient hospital discharges. By fostering strong relationships with health teams, discharge teams, and local services, this role bridges the gap between secondary care and community resources, contributing to improved outcomes for carers and patients alike.

Administration, Reception & Asian Carers Support

21 hours per week £17,635 incl. of Inner London Weighting.

The successful applicant will be required to provide a full range of administrative support services to our small busy Centre.  You will need excellent reception and first point of contact skills, as well as a friendly and efficient approach and experience of reception and office systems. Experience of using Microsoft Office 365 and experience of using a cloud-based CRM database (desirable). The successful candidate will require Asian language skills, Urdu, Hindi, Punjabi and the ability to facilitate a Carers Support Group.

This role will deliver tailored, one to one support and case work to support and resolve both immediate, crisis situations experienced by people and on a longer-term basis in the community. In the main people we support are Irish or of Irish descent including the Traveller Community. However, our services are open to all.

Additionally, part of the role will include working across the project team and wider role of ICCM in early intervention and prevention.  Informing and supporting community-based advocacy, awareness, and public campaigns to inform and increase knowledge and education of the community on how to access help, and the availability of support which is open to people as needs develop. Thus, increasing the wider public and social value of ICCM and brining more service users in to receive one to one support when needed. 

This will attract and support our community by making our services local, appropriate, and accessible to those who mainly do not access mainstream services. This will include advocacy and representative services in order for people to secure welfare benefits, enabling people to access the financial benefits they are entitled to.

This will support people experiencing mental and physical health issues, substance misuse, facing legal issues, those who require debt management, threatened with eviction, domestic abuse, and bereavement. This will be done mainly on an outreach basis across Manchester, supporting people in their own homes, in care homes, hospital and where they are resettled to their home environment.

The overall aim of the service is to improve quality of life, access to appropriate support services and improved health and wellbeing outcomes by acting as an advocate and to negotiate with other agencies, where necessary, on our client’s behalf.

Key Responsibilities:

• To make contact with members of the Irish and Traveller communities across Manchester.
• To provide Advocacy and Support via outreach, accompanying to appointments, advocacy to other agencies to address presenting needs who are vulnerable and ‘at risk’ through age, poor physical or mental health, homelessness, risk of homelessness, drug and alcohol use, offending, abuse, and social and cultural isolation.
• Undertake an initial assessment of service users’ needs, presenting issues and risks and to develop a care plan working alongside other statutory and voluntary agencies.
• To offer comprehensive information, advocacy, advice and support services to our community to meet individual needs and achieve positive outcomes.
• Make referrals to statutory and community and voluntary organisations working in partnership to achieve the best level of care and support.
• Develop care plans in order to improve the health of our communities, working with service users and relevant agencies
• To attend and participate in meetings and forums highlighting community issues, cultural needs and experiences.
• To support volunteers and students at ICCM. Volunteers will also support service users to meet presenting needs as highlighted in the assessment and care plan.
• To maintain accurate and up to date records of all areas of work.

The Women’s Recovery Service is a dedicated 22-bed accommodation designed to provide a safe, psychologically informed space for women with complex needs. Our mission is to support recovery and empower our clients by offering a nurturing environment that addresses the unique challenges faced by women experiencing multiple disadvantages.

About the role:

As the Team Manager, you will play a vital role in leading and supporting our frontline staff, ensuring the highest standards of trauma-informed, recovery-focused care for our residents. This position requires a balance of strong interpersonal skills and the ability to build and sustain partnerships across diverse stakeholder groups. Working closely with the Service Manager, you’ll provide day-to-day guidance and strategic support, maintaining SHP’s standards and fulfilling contractual requirements.

In this role, you’ll deputise for the Service Manager when needed, sharing responsibility for delivering compassionate, effective support that empowers women on their journey to recovery. A high-level understanding of the issues faced by women affected with homelessness, substance misuse, and the effects of complex trauma—including violence against women and girls (VAWG)—is essential. You will engage with clients across the spectrum of recovery, from those in pre-contemplation stages to those who are committed to positive change.

This role requires a strong commitment to making a meaningful impact, combining strategic oversight with empathetic, hands-on leadership to drive effective service delivery.

About you:

• Demonstrable experience and knowledge of complex issues effecting women including DV, VAWG, Mental Health, Women’s Health and Substance Misuse.
• Experience of, or a strong awareness and understanding of staff and service management and the ability to lead, motivate and develop the skills of both staff and service users, and to motivate and manage a quality service committed to achieving excellence.
• Understanding of Housing Management and compliance, especially in relation to License Agreements, Excluded License Agreements and Assured Shorthold Tenancies.
• Strong networking skills, and proven ability to build positive working relationships with internal and external stakeholders, including those with statutory services, in order to establish or improve services to service users.
• The ability to respond calmly to crisis and deal promptly, effectively, safely and creatively to complex and challenging situations. About the role:

This role is home-based and is open to applicants living in Devon and Cornwall who have access to an appropriately confidential space in which to work. Regular travel will be required both to our office in Plymouth & throughout Devon and Cornwall. Your role will embrace customer service, merchandising, staff supervision and everything else you’d expect in a retail environment, plus overseeing house clearance services and a fleet of five vans that play a crucial role to collect donated items and deliver customer purchases.

We’re looking for a self-motivated, empathetic, and dynamic Community Mental Health Recovery Support Worker to join our one-to-one mental health support service.

You’ll be primarily responsible for supporting people to identify and meet their self-defined mental health needs and aspirations, to understand the range of options available to them, to support them emotionally, and to connect them with appropriate community resources and services.

The post involves working directly with service users, volunteers, and in partnership with colleagues across different sectors in Camden. You’ll be working collaboratively alongside partners and statutory providers to adapt the service offers to meet changing local needs. You will be contributing to the transformation of community mental health services across Camden.

The role requires someone who: is passionate about people being at the heart of their individual mental health recovery; is an excellent communicator – both orally and written; and enjoys working within a diverse and growing team. Do you want to work in a role where no day is the same and make a real difference to the lives of people experiencing homelessness in London?

We are looking for people to join this exciting pilot as Assessment and Reconnection Workers. The service delivers a new approach to preventing rough sleeping, through the provision of accommodation with onsite specialist move-on support to those who are at immediate risk of sleeping rough. The service will provide a route and support for single homeless clients who are presenting to the Local Authority Housing Options or day centres who have not been sleeping rough yet but who are at imminent risk of this happening.  

Where homelessness cannot be prevented by the council a referral can be made into the Rough Sleeping Prevention service, which is run across London to support as many people as possible. The service will work in partnership with the local authority to secure onward move-on options e.g. access the council private rented sector (PRS) scheme, advocating for access to supported housing.

In these roles, you will work across 2 of our London locations in Islington (24/7 Assessment hub space for up to 15 clients), and Lambeth (medium support accommodation).

Each site will have a team delivering casework and support, providing each person with a comprehensive assessment of their needs and circumstances, and intensive specialist support to secure move on. In the rewarding role of an Assessment and Reconnection Worker, you will: 

• Be part of a busy team environment working closely with those new to, or at risk of rough sleeping. 
• Provide a personalised case management service to clients, turning assessment recommendations into practical action plans which fully involves the client.
• Build and maintain effective relationships to support clients throughout their recovery. 
• Work on the 7 day weekly rolling rota including early middle, and late shifts.

About you

These roles will suit people with experience of working at pace in a public facing environment with the ability to adapt to new situations.

• An understanding of the issues faced by homeless or vulnerable people and the difficulties they experience in accessing services.
• Good communication skills and the ability to work with others whilst managing your own time.

The Junction is an out of hours service, open 365 evenings a year, to support and assist individuals experiencing a mental health concern. If so, join St Giles where you will be dual trained as a Support Worker and Activity Facilitator working on our CFO Evolution programme, which will see you provide support to a caseload of participants in custody to prepare for release and successfully resettle into their local communities by providing practical help, advice and guidance, as well as motivation and understanding.

About this key role

Our successful candidate will carry out initial and ongoing assessments of the support needs of individual participants, before providing them with essential advocacy, mentoring, guidance and training interventions that meet their needs, plus engage and case-manage eligible participants on the CFO Evolution programme by providing ongoing resettlement support.  You will be expected to create an Action Plan, plan and design appropriate activities, training programmes and interventions, and establish and deliver high-quality, engaging one-to-one and group sessions, while also monitoring participants’ progress and providing timely monthly reports.

We will also rely on you to develop strong relationships with HMPPS/agencies offering support, where appropriate provide holistic wrap-around with general elements of resettlement support and working collaboratively plan for and facilitate effective Through the Gate and warm handover to the relevant Support Worker in Community.

What we are looking for

• Working intensively with individuals and supporting their progression
• Working with offenders, or those disaffected and marginalised from mainstream services, in training, guidance, advice and counselling environments would be advantageous
• High level guidance skills, with the ability to deliver one-to-one and group sessions
• Knowledge of the issues affecting the rehabilitation of offenders and desistance theory
• Knowledge of HM Prison environments and/or training, education and employment opportunities locally and nationally, and of training and guidance
• Demonstrable ability to support and motivate learners with multiple complex needs
• The ability to effectively assess evidence and identify assessment criteria using a range of assessment methods
• Excellent interpersonal and communication skills, both verbal and written
• A flexible, collaborative and professional approach to your work.

Please note this role requires Enhanced Adult DBS checks.
