About us

Who we are

We provide a palliative nursing service for people who have chosen to spend their final days at home, nursing the patient and supporting their carers. We don't work within an expected time frame, but look after people expected to be living their final weeks, providing much need support for family and carers. Working closely with the district nursing service, gps, discharge teams and other professionals our team of nurses, experienced in community nursing, the provision of palliative care and effective symptom control is fully supported to look after our patients and support their carers throughout our time with them. Our service is a 24 hour service and is provided free of charge . We plan our care around the patient and their family’s and carers requirements, fitting in with their needs. We are able to support carers well because our nurses visits to people's homes are a minimum of two hours, therefore we are able to spend time with carers to find out their needs and concerns and are able to work with them to ensure they feel supported, informed and confident to help care for the person
at home.
We work hard to ensure that our nurses, as lone workers, are safe and very well supported. We invest in good mandatory as well as varied training programmes for our nurses. Our Nursing Co-ordinator as our nurses’ first point of contact is available 24 hours a day, for patients and carers too. We tend to have around 22 patients on our casebook at any one time. Part of our service involves pop in visits for people who are not yet ready for our full nursing service, but who benefit from the support we can give to the patient and their carers in particular. Because we have witnessed the difference it makes to carers to have this contact before our nursing care starts, this is an area that we plan to develop as pre bereavement support.
We are occasionally able to care for people registered with other GP practices and we do this on a case by case basis dependent on the information we receive about the patient and their current requirements for care and our availability of nurses. This is something we wish to continue.

Our services are delivered free of charge to all those who need them. 

We are committed to continuing to provide an excellent palliative nursing service and to supporting improvement in end of life care across Gloucestershire and Burford in Oxfordshire. Our work supports patients and their families and carers across the community, developing local partnerships to support individual needs. Our stated aim is not to increase the area we currently work within but to maintain the quality of the service we offer within our current working area.
