Who we are
Transitions UK provides support that changes the lives of vulnerable and disadvantaged young people aged 13-24 yrs. Founded in 2014 from a frustration that the needs of our young people were not being properly addressed, let alone met, we launched our first project in Luton. We are now working in 18 location hubs across Herts, Beds, Bucks, Leicestershire, Birmingham and North London across four specialities – learning disabilities, at risk of offending, mental health challenges and care experienced young people.
We have a breadth of knowledge and experience working with young people presenting one or more of 4 key support needs. All our beneficiaries are experiencing at least one of these needs, however most experience multiple.
Previously, each beneficiary was referred to a specific project (Achieve, Affirm, Aspire or Attain) based on their primary presenting need. In response to growing numbers of beneficiaries with multiple needs, we have taken the decision to restructure our services to recognise the overlap of support our young people need. This will allow us to better share our expertise across the charity and provide better support.
