Who we are
Hammersmith and Fulham GP Federation Limited (‘The Federation’) is a company owned by all 29 GP practices in Hammersmith and Fulham and covers approximately 260,000 patients.
They are the contract holder for the Out of Hospital Services element of the Enhanced Primary Care contract, let by Hammersmith and Fulham CCG, alongside all of the GP practices. The Federation supports the delivery of clinical and related patient services through the local GP practice networks, and it acts as a system manager, agent of change, and contract lead.
The Federation has grown to now host the Community Education Provider Network (CEPN). This is a registered Community Interest Company (CIC) which receives grant income from Health Education England for training, education and workforce development of primary care staff. The CEPN supports the continued improvement in the quality and delivery of primary care services and facilitates the collaborative working of GP Practices in order to drive forward new models of care and deliver primary care at scale.
The Federation also hosts the National Institute of Health Research Council (NIHRC) research hub on behalf of practices working in close collaboration with Imperial College Healthcare NHS Trust (ICHT).
The Federation works in partnership with Hammersmith and Fulham CCG, jointly agreeing to the vision of the local primary care strategy. The Federation and Partner organisations - Imperial College Healthcare NHS Trust (ICHT), Chelsea & Westminster NHS Foundation Trust (CWFT), West London Mental Health NHS Trust (WLMHT) and Central London Community Healthcare Trust - have signed an Alliance contract that sets out our shared vision and principles for place-based, population-based care for Hammersmith & Fulham via an Integrated Care Partnership (ICP).
The company is currently based in Fulham and employs 22 full and part-time staff.
