Within this role, you’ll play a crucial part in ensuring smooth transitions for adults with complex needs into our specialist day service. You will work closely with individuals, their families, and a multidisciplinary team to create personalised transition plans, monitor progress, and advocate for the best possible care. You’ll be the point of contact for the people they support, liaising with external agencies and ensuring everything runs smoothly.

What you'll do:

• Coordinate and implement transition plans.
• Organise pre-placement visits and reviews.
• Work with families, carers, and professionals to ensure comprehensive support.
• Advocate for the individuals' needs and preferences.
• Collaborate with a multidisciplinary team for holistic care.
